3 Welcome to Alexandra Hospital The new experience is designed to meet your needs at every step of your healthcare journey from prevention to early intervention, consultation, admission, discharge and post-discharge. Here, care is anchored by a Doctor, leading a One Care Team comprising Nurses, Allied Health Professionals, Service Associates, Care Managers, Pharmacists and supported by Specialists. We work closely with our patients, their caregivers and families, as well as those who are well at home and in the community. In inpatient care, services will revolve around a patient who stays in the same bed at the same hospital, with one care team providing seamless acute care, sub-acute and rehabilitative care from admission to discharge. This contrasts with the conventional model of patients needing to be transferred from an acute hospital to a community hospital. The intensity and type of care are seamlessly matched to the trajectory of the patient s illness. Our CareHub will run a tele-triage and consultation hotline centre, which draws on the resources and expertise of the entire NUHS and its partners to identify and match care needs with the appropriate sites and providers of care. The hub will help the patients navigate the complex system, facilitate transfers between institutions, institute measures to prevent re-admissions and anchor care at home and in the community. Beyond, seamless handovers with one identified care team in primary care and the community will become the norm. While being prepared for transition to the community, upon discharge, patients can expect seamless handovers to an identified care team in primary care and the community that will continue to provide holistic care

5 A new model of comprehensive care drawn out at the new under the NUHS, recognises the important role of the family doctor and how a seamless connect with hospital care would provide better patient outcomes. From specialisation to holistic, same doctor, one appointment, same bed and one-stop care of the patient rather than patient s conditions, at, we don t address our referral memo to whom it may concern. Our care team makes sure that our patient s care is discharged to someone we know and trust from the dozens of GPs in the south-western area. Community-based primary care and hospital-based care provided by family doctors can be strengthened and integrated with opportunities to collaborate at a health district. The family physician has a role as patient advocate in hospital care. 6 Foreword A/Prof Jason Phua CEO D ear Friends, Singapore has one of the fastest ageing populations in the world, and noncommunicable chronic diseases account for an increasingly large proportion of the burden of illness. There is a need to maintain the health of those who are well, and to provide care that is seamless and holistic for those who are not. We have our work cut out for us at the new, as we take over the campus on 1 June We are here to stay for good, to serve the residents of the southwestern region (including Mei Ling, Mei Chin, Tanglin Halt, Holland Drive, Commonwealth, and Queensway), and Singapore as a whole. Approximately one in five of these residents are 65 years and older, and this percentage is set to grow. Many have multiple comorbidities, and yet available data show that a large proportion have not gone for health screening. Thus, if we truly want to help build a healthy community, we cannot be content with a hospital that merely looks inwards and focuses on episodic admissions. Instead, will be a health empowering environment with peaks of excellence in integrated care for chronic diseases and healthy ageing, across the entire care continuum and into the home. It will act as a dynamic step-up and step-down bridge between the National University Health System s Kent Ridge Campus and JurongHealth Campus and the community. It will serve as a site for test-bedding innovative solutions as it redesigns healthcare for Singapore s health problems. Even as we take over the current hospital, we are working with the Ministry of Health on a master plan to redevelop the 42,000 square metre green campus over the next decade, while preserving its healthcare heritage. Facilities currently planned include a new hospital, a nursing home, a hospice, a mental wellness facility, academic centres of excellence, and staff accommodation. Yet, while new infrastructure is always welcome, we believe that processes and partnerships are even more crucial. We are actively streamlining our processes as we create a new model of health and care, key features of which include a one-patient one-care team approach, a one-patient one-bed concept, a breakdown of barriers between home and community and hospital, use of technology as an enabler, and zero-based planning. As for partnerships, we are actively reaching out to our friends in the community, the most pivotal of whom are you, the stalwarts of primary care. We believe that nothing that we do at will make a sustainable difference if we don t work with you. You are and rightly so the go to persons for health and healthcare for most people, and we see ourselves playing a supportive role for you. There is so much we can do together, for those in the pink of health, those who are acutely ill, those who are feeling the burden of chronic illnesses, those whose hair is graying, and those at the twilight of their lives. Some of these people are well to do and lead happy lives with their families, while others are financially and socially disadvantaged and live alone in rental flats. Regardless, imagine the possibilities if together, we can build a seamless healthcare continuum which every person can navigate easily without ever being dropped from the system. 7 Foreword A/Prof Khoo See Meng Chairman Medical Board D ear Colleagues, is a hospital which has been imagined differently. Conventional hospital departments which are based on the training backgrounds of the doctors will not exist in. In its place are programmes and services that are organised around the needs of the patients and created according to the types of patients that the hospital will care for. This first edition of our s clinical directory presents you a broad picture of how we organise ourselves into five clinical programmes over a single continuum of care, from the well to those requiring end-of-life care. These five key programmes, Well, FAST, Chronic, Healthy Ageing and Palliative Care will form the pillars of the hospital s clinical organisational structure, which will in turn be supported by the surgical, rehabilitative, and urgent care services. Together, they will deliver care through an innovative model where one care team takes charge of each patient and manages the multiple complex comorbidities to ensure that the care experienced by our patients is seamless, complementary and coherent across care settings. Our ambition is to draw on the comprehensive resources and expertise of NUHS, and embrace the necessary complexity of the community of care created by the tight collaboration between hospitals, primary care providers and community partners, to deliver care to our patients with the paradoxical simplicity of a single care team, one bed and a named care provider. This is a hospital that sees its unique mission as generating thought-provoking hesitations in the routines of everyday care delivery rather than simply servicing those routines to deliver care because this to us, is healthcare redesigned. 9 Hospital Leads A/Prof Jason Phua Chief Executive Officer A/Prof Khoo See Meng Chairman Medical Board Associate Professor Jason Phua is the CEO of, and is a specialist in Respiratory Medicine and Intensive Care Medicine. He served as the Head of Division of Respiratory and Critical Care Medicine, NUH, from 2012 to 2016, and is the past President of the Society of Intensive Care Medicine (Singapore), the Deputy Chair of the National Intensive Care Unity Repository (NICUR), the Chair of the Asia Ventilation Forum (AVF), and the Secretary of the Asian Critical Care Clinical Trials (ACCCT) Group. He is a specialist in Respiratory and Sleep Medicine, and is the Director of the Sleep Medicine service, NUH, and the Head of Division of Acute Care, University Medicine Cluster, NUH. A/Prof Khoo served as the Programme Director for NUHS Internal Medicine Residency Programme from 2009 to 2017 and during this time, he received multiple teaching excellence awards. In his role as a clinician educator, A/Prof Khoo continues to contribute actively to both undergraduate and postgraduate teaching and remains a mentor to many today. Prior to this appointment, Grace was the NUHS Group Chief Infrastructure Officer from 2015 till 2018, and also served as the COO of National Healthcare Group (NHG) Polyclinics, as well as a Senior Director with the NHG Regional Health System (RHS) Office from 2010 to She has also been the Project Director for the developments of NUH Medical Centre and the Khoo Teck Phuat Hospital, and oversaw the development of various NHG Polyclinics. Grace's works over the years has covered infrastructure development, hospital planning, inpatient and surgical services operations, specialised support services, operational support services, and human resource in other public hospitals. 11 Hospital Leads Ms Hooi Pik Yee Head of Pharmacy Mr Melvin Poon Head of Allied Health Ms Hooi Pik Yee is the Head of Pharmacy at. She leads the Pharmacy Division, working with clinical teams to develop, implement and maintain high standards of care through medicine. Pik Yee has been the Chairperson of NUH Pharmacy and Nursing Quality Improvement Committee, implementing initiatives that addressed safe and effective medication management. She has also served as the co-chairman of NUH Insulin Safety Committee, leading a team to work towards improving insulin safety, and was a member of the MOH Workgroup for Job Redesign of Pharmacy Technicians, as well as the MOH National Pharmacy Landscape Workgroup. From our Urgent Care Centre, to the inpatient stay, to rehabilitative care, our model of care is one that facilitates a seamless transition of care provided by one dedicated care team in the integrated ward setting. 15 Better Programmes Chief of Urgent Care Centre Dr Keith Ho Consultant Chief of Rehabilitation Services Dr Effie Chew Senior Consultant Dr Keith Ho is the Chief of Urgent Care Centre at, and a Consultant in the Emergency Medicine Department at NUH. Dr Ho s area of interest is in Disaster Medicine. He did a fellowship with the Centre for Disaster Medical Services at the University of California, Irvine, and has served as the director of emergency preparedness for the Emergency Department, and as the chief hospital planner and co-ordinator for mass casualty incidents. As a member of the core faculty for NUHS Emergency Medicine Residency programme, Dr Ho contributes actively to both undergraduate and postgraduate teaching through medical simulation. 16 Better Programmes Director of Intensive Care Unit Dr Liew Mei Fong Consultant Dr Liew Mei Fong is the Director of the Intensive Care Unit (ICU) at Alexandra Hospital. She is currently an Associate Consultant with the Division of Respiratory and Critical Care Medicine at NUH. Dr Liew underwent specialist training in both Respiratory Medicine and Intensive Care Medicine in Singapore and has a keen interest in Critical Care Medicine, bronchiectasis and pulmonary hypertension. 18 Better Programmes Director of Active Centre Dr Lingaraj Krishna Senior Consultant Dr Lingaraj Krishna is the Director of the Active Centre at. He is the Head and Senior Consultant of the Division of Sports Medicine and Surgery, as well as a Senior Consultant at the Division of Hip and Knee Surgery at NUH. A trained orthopaedic surgeon, Dr Lingaraj deals with the entire spectrum of knee and hip conditions. He is also an Assistant Professor with the Yong Loo Lin School of Medicine at the National University of Singapore, and the Associate Programme Director of the Orthopaedic Surgery Residency Programme at the National University Health System. He is a Consultant with the Division of Geriatric Medicine, NUH. Dr Santhosh underwent specialty training in Geriatric Medicine and General Internal Medicine in London, and took a year to pursue his interest in Stroke at St. Thomas Hospital, London. Upon completing his training, he worked at The Royal Berkshire NHS Foundation Trust, Reading, before eventually returning to Singapore, where he is now also the Programme Director of NUHS Geriatric Medicine Senior Residency Programme. Head of Palliative Care Programme Dr Noreen Chan Guek Cheng Senior Consultant Dr Noreen Chan is the Head of the Palliative Care Programme at, aimed at championing compassionate care and quality of life for patients receiving palliative care. Dr Chan is the Head and Senior Consultant in the Division of Palliative Care, at the National University Cancer Institute, Singapore. She was also Medical Director and CEO of Dover Park Hospice, before joining the NCIS, where she also serves as Lead of the Supportive and Palliative Care Group. 21 Surgical Services has a team of experienced and dedicated surgeons and anaesthetists who offer comprehensive general surgical and orthopaedic care services including minimally invasive surgery, surgical endoscopy, ENT (Ear, Nose, Throat), Ophthalmology, Joint Replacement and Reconstruction Surgery. The hospital is equipped with an Urgent Care Centre opened 24/7, Integrated Care Clinics (I-Care Clinics), integrated wards, major operating theatres, day surgery centre, Anaesthesia Outpatient Consultation Clinic and supported by diagnostic radiology, clinical laboratories, day surgery facilities and the endoscopy centre.
