7 Welcome to South Country Health Alliance South Country Health Alliance (also referred to as South Country or SCHA) is a county-owned health plan that serves 12 Minnesota counties. Through South Country, these counties have joined together to help improve how you receive your health care. s, doctors, dentists, eye doctors, chiropractors, hospitals, nursing homes, and home care agencies are working with us to see that you get the best care. The 12 counties in our service area include: Brown, Dodge, Freeborn, Goodhue, Kanabec, Morrison, Sibley, Steele, Todd, Wabasha, Wadena, and Waseca. We work closely with county health care providers, human service agencies, and county public health departments in these counties to support your wellness. South Country supports five different programs to meet our members needs. South Country s Programs Families and Children** Families and Children is a managed care program that covers health care for the following people who have Medical Assistance: Children under the age of 21 Parents and caretakers of a dependent child Pregnant women Low-income adults without a dependent child Our Families and Children program is called Medical Assistance. MinnesotaCare** MinnesotaCare is a managed care program that covers health care for people who do not have access to affordable health care coverage. Some members may be required to pay a premium to the State. Minnesota Senior Care Plus (MSC+) MSC+ is a managed care program that covers health care and Elderly Waiver services for people who: are age 65 and over, and have Medical Assistance. If you are enrolled in MSC+, you may have another option. Check with your county financial worker to see if you can enroll in the Minnesota Senior Health Options (MSHO) program. MSHO is a voluntary program. Minnesota Senior Health Options (MSHO) MSHO is a voluntary managed care program that covers health care and Elderly Waiver services for people who: are age 65 and over, have Medical Assistance, and have both Medicare Parts A and B. Our MSHO program is called SeniorCare Complete (HMO SNP). There are no added costs for you to join this program. While a member, you will get your Medical Assistance and Medicare Parts A, B, and D through the plan. For more information or to enroll in SeniorCare Complete, call Member Services at (toll free), TTY users call or 711. If you choose to leave MSHO, you will be enrolled in MSC+ for your Medical Assistance services. Special Needs BasicCare (SNBC) SNBC is a voluntary managed care program that covers health care for people with disabilities who: are age 18 through 64*, have Medical Assistance, and have either both Medicare Parts A and B or Medical Assistance only. *People in SNBC who turn 65 will be enrolled into a senior managed care program if they continue to meet the eligibility criteria to enroll in managed care. South Country has three SNBC programs: AbilityCare (HMO SNP) integrates Medical Assistance and Medicare Parts A, B, and D for people who choose this option. SingleCare (SNBC) provides Medical Assistance benefits for those not eligible for Medicare. SharedCare (SNBC) provides only Medical Assistance benefits for those that are eligible for Medicare benefits but get them from other health plans. ** South Country Health Alliance Medical Assistance and MinnesotaCare are not available in Freeborn County. 8 Important Information CLINICS The clinic information printed in this listing was correct as of January You need to choose a primary care clinic (PCC) when you enroll. Your primary care clinic (PCC) can provide most of the health care services you need and will help coordinate your care. Please confirm with Member Services that your primary care clinic (PCC) is still a provider with our plan before signing up. How do you choose your Primary Care (PCC)? When you become a member of our health plan, you must choose a primary care clinic (PCC) from our Primary Care Network Listing. You must write your choice on the enrollment form. If you cannot find a PCC listing for your area, or have questions, call Member Services. Elderly Waiver (EW) Obligation (For MSC+ and SeniorCare Complete members) If you get EW services and your income is over the income limit, you may have to pay a monthly amount. This is called the EW waiver obligation. Your county will tell you about any EW waiver obligation when your eligibility is determined. Eye Care Services South Country members can receive eye care services. These services include eye exams and glasses. You can get more information on eye care services and providers by visiting our website at or calling Member Services. Health Education Ask Mayo is a nurse advice line available for your health questions 24 hours a day, 7 days a week. Call the helpline at (toll free) or TTY users call Want to quit tobacco products? In the EX Program, you will get expert advice and tips from Mayo and create a customized quit plan that learns and grows with you. Home and Community-Based Waiver Services for People with Disabilities who are members of our SNBC programs. We do not cover Home and Community-Based Waiver Services for people with disabilities who are members of our SNBC programs. But you may be eligible to get them through your county or tribe. Call your county or tribe to apply for these and other waiver services under the following waiver programs: Community Alternatives for Disabled Individuals (CADI) Brain Injury (BI) Community Alternative Care (CAC) Developmental Disability (DD) These waiver programs offer more services than regular Medical Assistance. Services include extended home care, assisted living, supported employment, and more. Home Care Home Care services include the following: Skilled nursing Rehabilitation therapies (for example speech, physical, respiratory, and occupational) Home health aide Medical equipment and supplies Personal Care Assistant (PCA) Home Care Nurse (HCN) services We do not cover PCA or HCN services for AbilityCare, SingleCare, or SharedCare members. But you may be able to get them through your county or tribe. Call your county or tribe to apply for these services. If you are currently eligible for and getting PCA or HCN services, you will not lose those services by enrolling in AbilityCare, Shared- Care or SingleCare. s Use a hospital associated with your primary care clinic. There is a list of hospitals after the primary care clinic section in this booklet (beginning on page 37). If you need to go to a hospital that is not in the listing or is not connected with South Country, call Member Services before you get services. If you are admitted to an out-of-network hospital, call Member Services within 48 hours of your admittance. Call your primary care clinic as soon as possible after receiving services. You can get more information on hospital services by calling Member Services. Interpreter Services American Sign Language (ASL) and language interpreter services are available to help you get services. Oral interpretation can be given to you in your language. You may be able to get written information in your language. Call Member Services to get interpreter services. Medical Equipment and Supplies Medical Equipment and Supplies is any medical equipment you can use in the home to improve quality of living. 10 tions or illnesses. Examples are nebulizers, wheelchairs, crutches, and many others. You can get information on durable medical equipment or DME providers by visiting our website at or calling Member Services. Mental Health Mental health services include care and treatment for depression, anxiety, or other mental health issues that interfere with your daily functioning. If you need mental health services, you can see any provider in the South Country network without a referral. If you need help finding a mental health provider call Member Services. Members should call Member Services before treatment begins as some treatments may require prior authorization. Nursing Home and Skilled Nursing Home Care (For MSC+, SeniorCare Complete, AbilityCare, SingleCare, and SharedCare members) For MSC+ and SeniorCare Complete members, we will cover a total of 180 days of nursing home care. If you need nursing home care for more than 180 days, the Minnesota Department of Human Services (DHS) will pay for your care. This applies unless you were already in the nursing home when you joined our plan. For AbilityCare, SingleCare, or SharedCare members, we will cover 100 days of nursing home care. If you need nursing home care for more than 100 days, the Minnesota Department of Human Services (DHS) will pay for your stay in the nursing home. The health plan will continue to pay for all health care costs. This applies unless you were already in the nursing home when you joined our plan. You can get more information on nursing home services by calling Member Services. Open Access Services For some services, you may choose to get care from any physician, clinic, hospital, pharmacy, or family planning agency even if it is not in our network. These are called open access services, and they include the following: Family planning Diagnosis of infertility Testing and treatment of sexually transmitted diseases Testing for AIDS or HIV-related conditions. You can get more information on open access services by calling Member Services. Pharmacy Members must use our network of pharmacies for drugs covered by our health plan. Remember to have the following available when you call to refill your prescription: South Country Member Identification (ID) Card Minnesota Health Care Programs card Cards for any other health coverage you have, such as Medicare Part D or private insurance It is important to remember to show both your Member Identification (ID) Card and your Minnesota Health Care Programs card when picking up your prescriptions. To see South Country's drug formulary, go to and choose "Find A Drug" to search the formulary. If you are enrolled in Medical Assistance, MSC+, or SingleCare (SNBC-Medicaid Only) and have Medicare, you must enroll in a Medicare Prescription Drug Program (Medicare Part D) to get most of your drugs. You can get more information on pharmacy services and network pharmacies by visiting our website at or calling Member Services. Specialty Care Specialty care describes services you can get from doctors or surgeons whose training focuses primarily in one specific field of medicine. Examples include neurology, cardiology, and oncology, among others. You can see any of the specialists in our network without a referral. You can get more information on specialty care services and specialists by visiting our website at or calling Member Services. Substance Use Disorder Services Substance use disorder (SUD) services provide care and treatment for substance use disorder. This can include abuse of alcohol, narcotics, or other drugs. If you need SUD services, you can see any provider in the South Country network without a referral. 11 Members should call Member Services before treatment begins as some treatments may require prior authorization. Transition of Care When you join South Country, we will help you continue to get necessary health care services that were authorized before you joined our plan. Some examples are the following: Orthodontia Mental health services Substance Use Disorder services Services for at-risk pregnancy Prescriptions If you are currently getting care from a provider who is not in our provider network, we will help you transition to a participating provider. If you would like more information please call our Member Services. Transportation (For Families and Children, MinnesotaCare*, MSC+, SeniorCare Complete, AbilityCare, SingleCare, and SharedCare members) *Medical transportation is available for MinnesotaCare Children under age 19. We are not required to provide transportation to your primary care clinic if it is over 30 miles from your home if another primary care clinic is available within 30 miles. Exceptions to this rule can be made in certain circumstances. If you need a ride and there isn t a closer clinic in our network, please call our Member Services. Members will get a Member Handbook for Families and Children, MinnesotaCare, MSC+, MSHO, and non-integrated SNBC or an Evidence of Coverage for integrated SNBC with more information about transportation. Urgent/After Hours/Emergency Care When possible, call your primary care clinic before going to the emergency room. They can help you decide if you need urgent or emergency care. Call Ask Mayo if you need help deciding if you need urgent or emergency care.
