4 Central East LHIN Integrated Health Service Plan (IHSP) Advancing integrated systems of care to help Central East LHIN residents live healthier at home 4
5 Background: Residential Hospice in Central East LHIN A residential hospice is a healthcare facility and registered charity that provides palliative care services by an inter-disciplinary team with palliative care expertise 24 hours a day, 7 days a week in a home-like setting for the individual and their significant others at no cost to the user. A residential hospice provides an option for adults and children with life-limiting illnesses to receive end-of-life services where death is imminent and a home death is not possible or preferred but hospital level care is not required. The Ministry of Health and Long-Term Care (Ministry) provides operational funding for residential hospice beds at $105,000 / adult bed / year. The Central East LHIN has one Ministry-funded 3-bed Residential Hospice: - The Bridge Hospice located in Warkworth. 5
6 Background: Residential Hospice in Central East LHIN (cont d) As part of 2005 Ministry s End-of-Life Care Strategy, operational funding for three 10-bed residential hospices (30 beds total) in the following Central East LHIN sub-regions was provided: - Peterborough City and County: o Lead Residential Hospice Provider: Hospice Peterborough - Scarborough North: o Lead Residential Hospice Provider: Yee Hong Centre for Geriatric Care - Durham West: o Lead Residential Hospice Provider: Victorian Order for Nurses (VON) Durham 6
7 Background: Residential Hospice in Central East LHIN (cont d) Minister Hoskins mandate (Nov 2014): - oversee the expansion of hospice care to include 20 new hospices across Ontario. This will almost double the number of beds for quality endof-life care and will provide palliative patients the opportunity to spend their final days in a home-like setting. Through capacity planning done in early 2016/17, the Ontario Palliative Care Network (OPCN) identified the need for 94 residential hospice beds in Central East LHIN. The Ministry has confirmed new operational funding support for 26 residential hospice beds in Central East (to be operational by 2018/19): - Total bed complement in Central East LHIN = 56 (30+26). 7
8 Central East LHIN Residential Hospice Strategy (July 2016) To provide a framework for stakeholders who are interested in the planning, development and implementation of residential hospice in the Central East LHIN. 8
9 Central East LHIN Residential Hospice Strategy Central East LHIN Residential Hospice Strategic Aim To expand options available to palliative patients in the Central East LHIN by increasing the number of operational residential hospice beds to 56 by 2019, by: - Pursuing development of beds across LHIN sub-regions; - Advancing collaborative multi-sector partnerships to make best use of public investment; and - Recognizing residential hospice as a key element to advancing Hospice as Hubs strategy. 9

11 Residential Hospice: Call for Expressions of Interest The Central East LHIN is requesting expressions of interest from health service providers, organizations and/or individuals who are interested in working with local stakeholders and the LHIN in the development of a residential hospice by 2018/19 in the following LHIN sub-regions and identified target communities: LHIN Sub-Region Target Community # of Beds Scarborough South Scarborough South 10 Durham Northeast Durham East 5 Northumberland County West Northumberland 3 Haliburton County and City of Kawartha Lakes City of Kawartha Lakes 3 11
12 Residential Hospice: Call for Expressions of Interest (cont d) Purpose of the Expression of Interest (EOI): 1. To identify health service providers, organizations and/or individuals who are interested in working with local stakeholders and the LHIN, and supporting the capital fundraising, planning and development of a residential hospice by 2018/ To identify a Lead Residential Hospice Provider who will be responsible for leading the planning, including the capital campaign collaboratively with local partners. **While the LHIN has targeted specific communities in this EOI, this in no way inhibits ongoing discussions and partnerships with other interested proponents in other Central East LHIN communities. The LHIN continues to welcome collaboration outside of the EOI targeted communities.** 12
13 Residential Hospice Model The Central East LHIN is open to different models for residential hospice beds including a free-standing building or co-location in another healthcare setting (e.g., retirement home, other) depending on the needs of the community and available resources. 13
14 Residential Hospice Costs Capital costs - Currently, there is no available provinical funding for capital planning or construction costs, as such, capital requirements must be fundraised through a robust fundraising/capital campaign. Operating costs - Nursing and personal support services are funded at $105,000 / adult bed / year, although the annual operating cost of a residential hospice exceeds funding. The table below provides a summary of the anticipated funding gap based on number of funded beds. 15 Linkages with Community Partners It is an expectation that residential hospice models will be integrated with community stakeholders, including but not limited to: Local visiting hospice services; Central East LHIN Palliative Care Community Teams; Primary Care and Palliative Care physicians and clinicans; Community Care Access Centre (CCAC); Hospitals; Municipality/County/City; and/or Other. 15
