3 WHAT is the SHRTN Knowledge Exchange? The SHRTN Knowledge Exchange is a Province wide network of people involved in seniors health care SHRTN promotes the exchange of knowledge generated through: Research explicit knowledge Practice and experience tacit knowledge

17 Background Limited resources and growing demand require that program and policy decisions are based on best possible evidence Most LTC and community care providers do not have access to the collections and services of an academic library to access this evidence nor the time to sift through it if they did SHRTN Library Service levels the information playing field by connecting all paid caregivers to high quality, clinically relevant information, suited to their needs

18 SHRTN Library Service Model Information Specialists deliver services from host/partner sites: Ottawa (Bruyère Continuing Care ) Guelph (St Joseph s Health Centre) Toronto (Baycrest) Hamilton (St Peter s Hospital, Hamilton Health Science Centre) Kingston (Providence Care MHS new as of this year) Library Coordinator (Baycrest) oversees services SHRTN Library Service is available: for free to all paid caregivers of seniors in Ontario by , telephone, in person or through the website

19 SHRTN Information Services Reference Services & Literature Searching Direct you to best evidence or resource to inform a practice innovation, project or quality improvement initiative Current Awareness Regularly distribute recommended reading lists, comprising recent and relevant articles and resources as well as new practice tools and evidence based fact sheets developed by members of the SHRTN collaborative Disseminate information on upcoming events of interest

24 Education SHRTN Information Services (cont d) Building information literacy skills and information finding capacity within organizations through one on one consultations or group training sessions on topics such as: Formulating a research question Finding the evidence using databases Using Google to find health information Understanding copyright Welcome feedback and ideas for sessions that would be helpful and relevant to our clients

25 SHRTN Information Services Networking, Outreach & Promotion Connect people with others interested in similar topics, both within and outside SHRTN Communities of Practice Support the development of information tools by clients, SHRTN Communities of Practice and community groups. Promote the SHRTN Library Service at meetings, events, conferences etc.
