5 In March 2016, the Ministry of Health and Long-Term Care announced the creation of the Ontario Palliative Care Network (OPCN). Led by LHINs, Cancer Care Ontario, Health Quality Ontario, and the Quality Hospice Palliative Care Coalition of Ontario developing a coordinated, standardized approach for delivering palliative care services in the province. OPCN is developing a 3-year Action Plan, which is expected to be finalized in December

7 As OPCN indicators are finalized, coordination of local strategies, initiatives and indicators will be reviewed to ensure strategic alignment. 7

9 Inaugural meeting in January 2017 Joint accountability to the Central East LHIN Chief Executive Officer and the Central East Regional Cancer Program Regional Vice President (similar across all 14 LHINs) Competency-Based Membership: Includes members from each of the seven Central East LHIN subregions, and across sectors. Includes individuals with the following areas of expertise: Clinical Leadership; Program Leadership; System Expertise; Research/Academic Expertise; Quality and Performance Improvement; Enabling Technology and Integration; Change Management; Indigenous Expertise; Francophone Expertise; and Caregiver Expertise. 9

10 Mandate: Advance high quality, integrated, patient-centred hospice palliative care across all sectors based on best practices in accordance with Ontario Palliative Care Network (OPCN) direction, and in alignment with both the Central East LHIN Integrated Health Service Plan (IHSP) and the Central East Regional Cancer Program Strategic Plan. 10

11 Central East LHIN Integrated Health Service Plan (IHSP): Continue to support palliative patients to die at home by choice and spend 15,000 fewer days in hospital by increasing the number of people discharged home with support by 17% by 2019 Central East Regional Cancer Program Strategic Plan: Quality of Life and Patient Experience to deliver responsive and respectful care that optimizes quality of life for patients across the cancer continuum Integrated Care collaborating and working together to develop stronger links with primary care providers Safety adherence to evidence-based best practice guidelines with respect to all treatment protocols Equity promote health equity and better access to all patients receiving services in the region Sustainability continue to provide cancer care options for care close to home, ensuring resources are optimally allocated and utilized Effectiveness to ensure the provision of cancer care services are delivered based on best evidence across the entire cancer journey 11

12 Purpose: To be the principal advisor within the Central East Region to: Provide leadership and structure to facilitate the development of a high-quality, comprehensive, integrated and coordinated system of hospice palliative care informing Central East LHIN and Central East Regional Cancer Program decision-making; and, Support the Ontario Palliative Care Network (OPCN) vision and direction for hospice palliative care as outlined in Advancing High Quality High Value Palliative Care in Ontario; A Declaration of Partnership and Commitment to Action (Declaration). 12

14 Two half-day facilitated sessions (Feb 28 th and April 6 th ) of the Central East Regional Palliative Care Steering Committee Identification of 11 priorities/themes Survey developed and sent to stakeholders to prioritize the 11 priorities/ themes 117 local stakeholders were targeted 31% return rate Four priorities identified and draft high-level action plans created 14

15 1. Establish common and consistent palliative care terminology to be utilized across Central East region. 2. Establish a purposeful and deliberate coordination of care process at points of transition across all care settings. 3. Establish and implement an Advance Care Planning Strategy. 4. Review and continue implementation of an education strategy for health service providers 15

16 Adoption of the Ontario Palliative Care Network (OPCN) Key Palliative Care Concepts & Terms document to ensure consistency across the Central East LHIN and alignment with the province. Next steps: Spread and implementation of the document.

17 Utilization of the Ontario Palliative Care Network (OPCN) Journey Mapping Exercise. Future action items: Establish a Coordination of Care Working Group Review current state and identify sub-set of priorities for focus Consider coordination of care at all points of transition Hospital, community, long term care, hospice, other 17

19 Align with existing palliative care education funding Learning Essential Approaches to Palliative Care (LEAP) Fundamentals, Comprehensive Advanced Palliative Care Education (CAPCE) Utilization of Palliative Pain and Symptom Management (PPSMC) resources 4 in Central East region 2 in Scarborough North and South sub-regions 1 in Durham West and Durham North East sub-regions 1 in Northumberland County, Peterborough City/County and Haliburton County and City of Kawartha Lakes sub-regions Education needs assessment/gap analysis 19
