An Audit Checklist This checklist is designed to help staff, families and volunteers in assessing how dementia care services might improve in relation to meeting the needs of people with dementia with later stage experiences. It is not meant that this checklist is used to check up on services services may wish to audit themselves- the checklists purpose is to act as an aide memoir to enable discussion to occur between everybody involved. Dementia Care Matters would appreciate receiving feedback, in any way, on this document to help us to improve / revise it. Does the service have a specific philosophy about late stage dementia care? 2. Does the philosophy identify key concepts as below: i) High level of human connection and touch being needed. ii) Love, consoling care and warmth being demonstrated. iii) Provision of specific sensory stimulation and sensory calming approaches. iv) Emphasis on comfort needs being met. v) Assessments to reduce environmental stress. vi) Staff training to prevent multiple stressors. vii) Links to palliative care viii) Offering bodily consolation. 3. Does the service Undertake- i) Noise assessments in relation to each person. ii) Visual stress assessments in relation to each person. 4. Does the service use specifically designed physical checklists in relation to people with a dementia with later stage needs? 3
4 5. Does the service monitor whether it is achieving consistent carers with people with a dementia with later stage needs? 6. Does the service understand how to balance the need for inner retreat and quiet with periods of appropriate sensory stimulation? 7. Does the service have a policy in relation to pain control with people with a dementia in the later stages? 8. Does the service have a joint team approach with a palliative care service in relation to this client group? 9. Does the service monitor whether an individual with later stage dementia care needs receives a minimum of 3 sessions of 10 minutes, per session, i.e. total 30 mins per day of individual care time for one to one contact, excluding physical / personal care needs? 10. Does the service still refer to people s life histories, even at this stage, to search for clues and prompts to what the person may be experiencing? 11. Accepting of the staffing issues which services face, has the service used or is considering using trained volunteers in this area? 4
5 12. Does the service demonstrate knowledge of cocooning methods to provide inner comfort for people with a dementia at this stage, whilst ensuring this is not used as a restraint? 13. Does the service provide and make available, through its staff or outside resources, massage? 14. Accepting of the need to provide sensitive, individual person centred responses, has the service a range of resources, such as Access to religious and spiritual resources appropriate to the individual. A rocking chair. Dolls and other attachment object. Tactile items. Soft toys. Sensory fabrics to fiddle with i.e. wool, fur, velvet etc. Busy aprons. A range of memory kits such as young man/ woman/ baby/ nature etc. Hand Lotion. Soothers for mouth. Activity busy boards. 15. Does the service understand and know why the items in (14) may be beneficial? 16. Does the service provide training for staff, volunteers and families on how to connect with people with dementia in the later stages? 5
6 17. Does the service provide an environment with positive sensory smells to stimulate memories? 18. Does the service understand the importance of occupying people with a dementia s hands in the later stages? 19. Is the service music focused and knows how to provide rhythmic playing? 20. Does the service know how to encourage eating/ drinking without using peg/ tube feeding i.e. finger foods, use of simple instructions and modelling of eating actions? 21. Does the service know how to prevent bathing and other personal care from becoming a multiple stressor? 22. Has the service an action plan for the year on improving this specific services area? 6
