2 Mike Harnett Mike Harnett is VP of Human Factors for SIX Safety Systems, a consulting firm that specializes in fatigue management. Mike has a background in human factors and sleep neurobiology. Her background has led to a successful 25 year career consulting with various federal government agencies including NASA, as well as assisting airport authorities, rail, trucking, nuclear, manufacturing, mining, energy, maritime and first responder industries to implement fatigue management strategies. Mike actively serves on several technical committees to help establish national and international standards and was recently involved in a National Safety Council Blue Ribbon Panel addressing fatigue in the United States. Eyes Wide Shut: We Don t Have a Fatigue Problem Talk summary: Fatigue is a chameleon. It disguises itself as a rules violation, a lapse of attention, non-compliance, or more often, human error. As a result, organizations deal with the consequences of fatigue-related incidents on a regular basis without even knowing it. It is time to open our eyes to see how fatigue lurks not only in our workers, but hidden in the design and management of our work system. This requires a fundamental change in the way we currently plan work activities, evaluate errors, record incidents, perform investigations, conduct audits and collect worker input. It requires a plan that is rooted in science, driven by data, and integrated into our existing health and safety management system. Only then can we see the impact of fatigue on performance, and only then can we make informed decisions on the best way to identify, measure, and mitigate fatigue related risk Page 2

3 Dr. Myriam Juda Dr. Myriam Juda is a Vancouver-based research psychologist (Simon Fraser University) and consultant (Circadian Light Therapy), specialized in circadian rhythms. With expertise in the sleep-wake cycle, shift-work, and light and health, she has worked with some of the world s largest companies, including Volkswagen, Arcelor-Mittal, Siemens and Osram. In a collaboration between BC Hydro, Fraser Health, Provincial Health Services Authority, the University of British Columbia and Simon Fraser University, she is currently running one of the first clinical studies to explore the effects of cycled lighting changing in intensity and colour temperature over 24 hours on patients rest/activity cycle, sleep and psychological well-being. Work-Induced Circadian Misalignment: Social Jetlag and Its Implications to Fatigue and Health Talk summary: Our body has a circadian clock, which determines when we re awake or sleepy during the 24-hour day. When we work and sleep out of alignment with our circadian clock, such as in shift work, we experience symptoms similar to jetlag (e.g. fatigue, disturbed sleep). Although travelers adjust to a new time zone within a few days, the majority of shift workers never adjust to working nights. This is because shift workers are still being exposed to the natural light/dark cycle of the sun and thereby remain diurnally synchronized to their location. Genetic differences in chronotype (early birds versus night owls) play an important role in predicting a worker s circadian misalignment. When working nights, late types (night owls) are less misaligned and have better and longer sleep than early types (early birds). When working early shifts however, early birds are less misaligned and have better and longer sleep than late types. Light is the dominant environmental cue for synchronizing the circadian clock. Carefully scheduled exposure to light in the workplace can reduce the physiological, cognitive and health consequences associated with disruptions in our circadian clock. Page 3

4 Najib Ayas Associate Professor, Department of Medicine, Faculty of Medicine, UBC Dr. Najib Ayas graduated with distinction from the University of Alberta in He completed an Internal Medicine residency at the Mayo Clinic in 1996, and postgraduate training in Respiratory, Critical Care, and Sleep Medicine at Brigham and Women s Hospital/Harvard Medical School in He was on staff at Brigham and Women s Hospital for two years, and then joined the Sleep Disorders Program at the University of British Columbia in September He also has a Master of Public Health from the Harvard School of Public Health. His research focuses on the public health and safety consequences of sleep apnea and sleep deprivation. He has published over 150 papers, and his work has been published in a variety of medical journals including the American Journal of Respiratory and Critical Care Medicine, JAMA, NEJM, Chest, Sleep, Cardiovascular Nursing, Journal Spinal Cord Medicine, Sleep Medicine, Thorax, and the Archives of Internal Medicine. He is also an Executive of the Canadian Sleep Society, and an Associate Editor for the American Journal of Respiratory and Critical Care Medicine. The Impact of Healthcare Worker Fatigue on Patient and Occupational Safety Talk summary: There has been increasing interest in the impact of healthcare worker schedules on patient and occupational safety. Work schedules have a profound effect on providers sleep and performance, as well as on their safety and that of their patients. For example, nurses working extended shifts are at significantly increased risk of experiencing decreased vigilance on the job, suffering an occupational injury, or making a medical error. Some of the most extreme schedules are worked by physicians-in-training (resident physicians). The traditional 24+ hour on-call shifts are associated with increased risk of experiencing an occupational sharps injury or a motor vehicle crash on the drive home from work and of making a serious or even fatal medical error. As compared to when working 16-hours shifts, on-call residents have twice as many attentional failures when working overnight and commit 36% more serious medical errors. They also report making 300% more fatigue-related medical errors that lead to a patient s death. Conclusion: The weight of evidence strongly suggests that extended-duration work shifts significantly increase fatigue and impair performance and safety. Page 4

Investigating Human Fatigue Factors in Safety Incidents, One Big Risk Factor to Consider Talk summary: There is an often overlooked fatigue risk factor that is almost guaranteed to be taking its toll on your workforce even if you don t operate 24/7. Health, productivity and especially safety are all degraded by this fatigue risk factor. In his presentation, Clinton will explain this fatigue risk factor and a way to investigate it. 6 Jason Kumagai, MSc, CHFP, CCPE, PMP Jason Kumagai is the Principal Consultant at OPTIMAL FiT Inc. based in Calgary Alberta. He brings over 20 years of experience in Human Factors / Ergonomics across many industry sectors, including oil & gas, defense, rail, road transportation and healthcare. Jason has worked with companies to develop and implement Fatigue Risk Management Systems and has assisted with the development of Industry Fatigue Risk Management Guidelines. Jason was President for the Canadian College for the Certification of Professional Ergonomists, and held the position of President of the Prairie and Northern Region of the Association of Canadian Ergonomists. Waking-up to Fatigue Risk Management in the Workplace Practical Tools & Techniques Talk summary: This session will use case studies to describe the application of various tools and techniques that have been used to determine if fatigue is an issue and develop strategies to address the associated risks. Examples will span several different sectors including oil and gas, rail, aviation, mining, healthcare and commercial transportation. Tools include surveys, audits, fatigue calculators, biomathematical fatigue modelling, and actigraphy. This session will discuss some of the mechanisms for success such as leadership, policy, procedures, guidance documents, training, and monitoring. Page 6
