1 National Conference on Tackling Tobacco Use in Vulnerable Populations National SelfMade Health Network (SMHN) Dwana Dee Calhoun, MS-Director October 5-6, 2015 Bethesda, Maryland History One of a consortium of eight (8) national networks funded by the Centers for Disease Control (CDC) Office of Smoking and Health (OSH) in partnership with the Division of Cancer Prevention and Control (DCPC) to advance prevention and control efforts involving disparities. Disparities in Cancer-Lung, Colorectal, Breast and Cervical Disparities in Tobacco Use 1
2 Vision Envision a nation in which multi-generational vulnerable populations residing throughout rural, urban and frontier regions have equitable awareness and access to geographic and culturally-relevant information. Envision a nation in which underserved communities also possess equitable access to current, evidence-based resources and affordable services provided by a national, regional, local and statewide network of health, human, and community-based systems. As valued members of society, they routinely utilize these services leading to greater opportunities for more informed decisions about cancer-free living and tobacco-free environments. SMHN Priority Populations By Income: Low annual income at or near the Federal Poverty Level (FPL) according to family or household size with geographic consideration. By Level of Education: Fewer than 12 years of education, adults with a GED as their highest level of education or high school graduate as their highest level of education. By Health Insurance Status: Uninsured and underinsured Cigarette Smoking Rates Among Adults By Level of Education Highest Level of Education 12 or fewer years of education (no diploma) Cigarette Smoking Rate 24.2% GED Certificate 41.4% High School diploma 22% Associate s degree 17.8% Some college, no degree 20.9% Undergraduate college degree 9.1% Graduate degree 5.6% Reference: Current Cigarette Smoking Among Adults in the United States 2
3 SMHN Priority Populations By Geography: Populations residing in medically underserved areas (MUAs) which includes areas with a shortage of primary care providers (PCPs), high infant mortality, high poverty or a high elderly population. By Employment Status: Low income employees (including full-time and part-time employees, seasonal/contractual and minimum wage employees) Unemployed (non-employed) By Residential Status: Residents in low-income/affordable housing units and communities (including aging adults, pregnant/postpartum women and other adults) Rationale for Selection of SMHN Priority Populations (Part 1) Elevated risk of developing preventable or late-stage chronic conditions due to limited affordability or delayed access to quality oriented healthcare and resources. Experience greater difficulty navigating the healthcare system. Increased risk for utilization of emergency room (ER) visits and preventable hospital admissions. Rationale for Selection of SMHN Priority Populations (Part 2) 3
4 Rationale for Selection of SMHN Priority Populations (Part 3) Higher use of cigarette smoking among adults who live below the poverty level (29.2%) compared to national average (17.8%). Less use preventive healthcare such as: cancer screenings among vulnerable populations (including adults with lower health literacy levels). Due to limited or delayed utilization to preventive and early screening, and treatment services poses higher risk for developing multiple chronic conditions or co-morbid conditions. More likely to be newly insured for the first time as adults.
