2 WHAT IS A COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA)? Completed every 3 years, a key component of the Affordable Care Act Required of tax exempt hospitals to get an accurate picture of the health needs of each community Provides an overview of the health needs and priorities associated with our service area (Sauk County and surrounding communities) Guides our hospital in community benefit planning efforts and development of a strategic plan to address evaluated needs Also a way to improve health coordination of hospital community benefits with other efforts through partnerships with Sauk County Health Department
