Acknowledgments Thank You A special thank you to the school district superintendents, school principals, administrators, teachers, parents and students for participating in the 2015 Ottawa County Youth Assessment Survey (YAS). Planning and implementation of this study was coordinated by the YAS Committee, whose time and efforts were invaluable in making this project possible. Funding Provided by Allendale Community Foundation Community Foundation of the Holland/Zeeland Area Coopersville Area Community Foundation County of Ottawa Grand Haven Area Community Foundation Analysis Data analysis and report provided by Ottawa County Department of Public Health Epidemiologist Marcia Mansaray, M.Sc. and Grand Valley State University Associate Analyst Zachary Weber. 5 Introduction What is the YAS? A locally developed teen survey since Conducted every two years at grades 8, 10 and 12 in participating Ottawa County schools. Monitors how behaviors increase, decrease or stay the same over time. Measures how many teens are engaged in a wide variety of both risky and beneficial behaviors that affect their health and wellbeing. How did the YAS get its start? Prior to 2005, there wasn t a consistent or reliable way to find out about the needs and strengths of teens within the county. Several organizations recognized this gap and worked together to review existing surveys already considered valid and reliable. From that review, a survey tool was developed specifically for Ottawa County that could also be responsive to future needs of local teens. Why is the YAS important? Provides evidencebased information about what the biggest needs are (and aren t) among Ottawa County teens. Gives parents information they may use to guide their teens in becoming physically, emotionally and mentally healthy adults. Helps faith communities, local agencies and schools focus their efforts on the most pressing and prevalent issues teens say they face. Helps evaluate the effectiveness of interventions developed to address pressing and prevalent teen issues. Provides the option for school districts to have their own results to compare with Ottawa County s as a whole. Schools and parents may then make evidencebased decisions about how to address their student body s unique needs. Organizations and community members may request additional analysis that is suited to fit their needs. What is in the YAS report? Questions about: family and school experiences; sexting and sexual behaviors; mental health conditions; bullying, safety and violence; alcohol, tobacco and drug use; body image, nutrition and physical activity; and many more. New questions in 2015: bullied in neighborhood; use of electronic vapor products; using another ID to buy alcohol; recently used different forms of marijuana; have parents who talked about expectations when it comes to sex; and tested for a sexually transmitted disease in the past year. What issue areas affect males and females differently. Ages when teens may engage in risky behaviors for the first time Ottawa County Youth Assessment Sur vey 5

Participants Actual Representation The proportion of student respondents may vary from the true proportion of 8 th, 10 th and 12 th graders or any given school. As a result, using accepted statistical procedures, each student s data were assigned a poststratification weight to improve how well the results actually represented student responses from participating schools. This is done by comparing the actual number of respondents in 8 th, 10 th and 12 th grades per school to the reported headcount for those grade levels per school. In the tables to the right, observe how the numbers in the Percent column differ from the Weighted Percent column. 4,966 par ticipants 75 % White 11 % Hispanic 14 % Other Total 13 school systems participated Gender N % % Female % Male % Grade Level 8th % 10th % 12th % Race/Ethnicity Demographics of survey par t icipants White % Hispanic % Other % Weighted % 100.0% 50.3% 49.7% 34.1% 34.9% 31% 75.6% 10.1% 14.3% 3,170 participants answered sexual health questions 50 % male 50 % female 8 th grade 46 % 10 th grade 31 % 12 th grade 23 % Total 7 school systems participated Part icipants for sexual health quest ions N % % Gender Female % Male % Grade Level 8th % 10th % 12th % Race/Ethnicity White % Hispanic % Other % Weighted % 100.0% 49.6% 50.4% 32.7% 37.0% 31.3% 78.1% 8.6% 13.2% 6

Average Age of Onset Methodology The responses of 12 th grade students were analyzed to estimate the average age when they first tried one of the behaviors listed in the table below. For each of these behaviors, the responses were assigned a value. Then the mean age was calculated to give an estimate of the average age at which a student may try a behavior by the time they reached their last year of high school. 14 3/4 years old 15 3/4 years old Average age 12 th graders f irst engaged in behavior joined an organized gang smoked cigarettes used chewing tobacco used electronic vaping products drank alcohol used marijuana used inhalants to get high misused over the counter drugs misused prescription drugs had oral sex had sexual intercourse / /4 14 3/ /2 14 1/4 14 3/4 15 1/2 15 1/2 13 1/2 years old 15 years old 11 years old 14 3/4 years old 2015 Ottawa County Youth Assessment Sur vey 7

Interpreting the Results Data Presentation Survey questions are organized into issue areas and the number of students who responded to each question is shown in the Total N column. The Total N column for any particular question may be smaller than the 4,966 students who completed the survey. Reasons Total N column may be smaller than 4,966 1) Students were told they may leave the questions they did not want to answer blank. 2) Some questions were not shown to a student because of their answer to a previous question (skip logic). 3) Some responses may have been removed during data cleaning if the selected response was inconsistent with other responses provided by a student. To the right of the Total N column are four different categories of columns where the number and percentage of those who reported the behavior are presented. 4 dif ferent categories 1) Total Responses (starting with 2015 and going back to 2007) 2) 2015 by Gender 3) 2015 by Grade Level 4) 2015 by Race/Ethnicity 4) Not all schools included the 12 sexual health questions in their survey. 9 Data Cells In each of the data cells (Figure 1), the top number is the weighted percent of students reporting the behavior and the bottom number is the count of students who answered yes to the corresponding question. The results given for the Gender, Grade Level and Race/ Ethnicity categories show only 2015 data. Each data cell contains the results that correspond to those who reported the question out of the total respondents within each subgroup of the category. Example in Figure 2 In 2015, 30.7% of females (777 of all female respondents) versus 33.9% of males (856 of all male respondents) reported they ate five or more servings of fruits and vegetables on an average day. 10 Margin of Er ror Overall Percentage When using the 2015 YAS results, it is important to take the margin of error into account. For example, 72.4% of Ottawa County 8 th, 10 th and 12 th graders sampled are a healthy weight (Figure 1). Using the margin of error of ±1.4%, it can be concluded that the actual percent of Ottawa County 8 th, 10 th, and 12 th graders who are a healthy weight is most likely between 71% and 73.8%. Figure 1 71% 73.8% 72.4% 1.4% +1.4% 70% 75% Youth Assessment Survey Margins of Error Overall 1.4% Gender 2% Grade Level 2.5% Race/Ethnicity 1.7% White 3% Hispanic and Other Comparing Group Percentages Margin of error can also be used when comparing groups. The 2015 YAS reports 12.9% of females and 15.7% of males used an electronic vapor product in the past 30 days. One should not assume the prevalence of use is higher for males than females. When considering the ±2% Gender margin of error, it can be concluded the prevalence for females is likely between 10.9% and 14.9% and the prevalence for males is likely between 13.7% and 17.7%. Because these intervals overlap (Figure 2) one should NOT conclude the prevalence is higher for males than females. It is more accurate to conclude the prevalence of using an electronic vapor product in the past 30 days is approximately equal for males and females. When comparing two or more groups, if the intervals found by adding and subtracting the margin of error do not overlap, one MAY conclude the prevalence is higher for one group than the other. 10% Figure 2 2.0% +2.0% 12.9% 15% 2.0% +2.0% 15.7% 95% conf idence level 20% Female Male Sexual Behavior Section Margins of Error Overall 2% Gender 2.5% Grade Level 3% Race/Ethnicity 2% White 4% Hispanic and Other The level which states that a result is expected to fall within the margin of error 19 times out of 20 if the survey is repeated using the same procedures. These margins of error were found using the conservative 1 formula n where n is the number of respondents. Because a different number of students responded to the sexual behavior questions, separate margins of error were calculated to use when interpreting those responses. 10

To determine the symbol selection, linear trend analyses were done using logistic regression and were controlled for gender, grade in school and race/ethnicity. If the pvalue for the linear time variable is less than the significance level (0.05), then there is evidence of a linear change. If the estimated coefficient for the time variable is positive, then there is evidence of a linear increase. Similarly, if the coefficient is negative, then there is evidence of a linear decrease. This was done in accordance with the CDC s guidelines for conducting linear trend analysis. Results for the year 2005 were included in the analysis, but due to space they are not shown in this report. 48 This past year, much attention has been given to mental health (rightly so). Having the YAS data confirms that youth do in fact suffer from some form of depression. This tells us that we need to begin addressing issues of mental health much earlier in a child s life. Executive Director Patrick Cisler, Community SPOKE & Lakeshore Nonprofit Alliance 48

Risk & Protective Factors Community School Family Peer & Individual Risk and protective factors are scored using sets of survey questions called scales and are organized by domain (Community, School, Family and Peer & Individual). The scale construction followed guidelines provided by the University of Washington s Social Development Research Group. The 2015 YAS report contains scales for 7 risk factors (Community Disorganization; Perceived Availability of Drugs; Low School Commitment; Family Conflict; Parental Attitudes Favorable toward Alcohol, Tobacco and Other Drug Use; Poor Family Management; and Low Perceived Risk of Drug Use) and 1 protective factor (Family Opportunities for ProSocial Involvement). Each question in a scale had its responses ordered and valued from 1 to 4 or from 1 to 5, depending on the number of responses. The values of each student s responses were then summed for each scale and the mean scale value was calculated. If the student s mean was more than the expected scale mean, then the student s score for a scale was considered at risk. If the student s mean was less than or equal to the expected scale mean, then the student s score was considered not at risk. The Risk and Protective Factors tables report the percentage of students who scored at risk for each Risk Factor scale or not at risk for the single Protective Factor scale. Percentages were also reported among the subgroups for gender, grade level and race/ethnicity. Risk Factors are conditions that increase the likelihood of a young person becoming involved in drug use, delinquency, school problems or violence. LOW PERCENTAGES are desirable among risk factors. Protective Factors are conditions that buffer youth from exposure to risk by either reducing the impact of the risks or changing the way that a young person responds to risks. HIGH PERCENTAGES are desirable among risk factors Ottawa County Youth Assessment Sur vey 49

Methods School Participation & Survey Administration The YAS was designed as a selfadministered anonymous questionnaire for students in grades 8, 10 and 12 in Ottawa County schools. The YAS was a paper fillable form in 2005, 2007 and In 2011, the YAS Committee piloted an optional online questionnaire, in order to reduce both printing costs and student classroom time. Based on survey results and school feedback from 2011, the YAS Committee converted to a fully online survey system in All school systems with one or more of the eligible grade levels were formally invited to participate by the YAS Committee in the 2015 YAS. Thirteen school systems participated; representing public schools, nontraditional high schools, nonpublic schools and public charter schools. Working with each participating school, the YAS Committee assigned each school system a oneweek window to administer the online survey to one or more classrooms of students; either in a teachersupervised classroom or computer lab setting. Schools were asked to administer the survey in a way that ensured every student had an equal chance of taking the survey, such as in a required course for each respective grade level. From October through middecember 2015, a total of 4,966 students voluntarily completed the online questionnaire. Survey Questionnaire The core questions of the 2015 survey were adapted from the national Youth Risk Behavior Survey (YRBS), a national schoolbased survey conducted by the Centers for Disease Control and Prevention. Additional items in the questionnaire were selected from the validated Communities That Care survey instrument (developed by the University of Washington). In limited instances, questions were altered or developed by the YAS Committee for more appropriate application to Ottawa County students or in response to feedback from the community. The survey procedures were designed to protect students privacy by allowing anonymous and voluntary participation. Students were informed they could decline the online survey and they were also instructed to skip questions which they preferred not to answer. No student identifying information was collected in any part of the survey process. Appendix A contains the Frequency Tables, which consist of the survey questions and response options. Available online only at Ottawa County Youth Assessment Sur vey 55

Methods (continued) Preparing the Data SAS 9.4 statistical software was used to prepare and analyze the raw data from student responses YAS data was cleaned and edited for response ranges, height/weight plausibility and logical consistency in accordance with YRBS criteria contained in the 2013 YRBS Methodology Guide. Use caution when comparing the 2015 report with previous years. As new questions and behaviors are added, some questions do not remain comparable. In the 2015 report, past years are set to even though this value may have been reported in the past (e.g., I have never used any of the drugs in the YAS ). The addition of electronic vapor products changed the potential results for this question. Therefore, past survey results may not be comparable. Always consult the 2015 report for the most accurate comparisons and statistics for each survey year. Student weight status categories (table to the right) were assigned based on each student s selfreported height, weight, age and gender. Since all students were 20 years of age or younger, weight categories were determined based on a teen s percentile rank according to the Centers for Disease Control and Prevention s Pediatric Clinical Growth Charts for males and females. Weight Status Category Underweight Healthy Weight Overweight Obese Percentile Range less than the 5 th percentile 5 th to less than the 85 th percentile 85 th to less than the 95 th percentile 95 th percentile or more The information available through the YAS has been an amazing resource to view the needs in our community. The fact that it is specific to Ottawa County gives useful information we need to make informed decisions. 57 The Results New variables were constructed based on methods used in the 2013 YRBS Data User s Guide. From 206 original survey questions, 266 dichotomous yes/ no research measures, or variables, were created. For example, survey question 7 is: During the past 30 days, how many times did you ride in a car or other vehicle driven by someone who had been drinking alcohol? NO YES{ 0. 0 times 1. 1 time 2. 2 or 3 times 3. 4 or 5 times 4. 6 or more times From this question one research measure, or variable, is reported: I rode in a vehicle driven by someone who had been drinking alcohol during the past 30 days. If the student response was anywhere from 1 time to 6 or more times, then those responses are all recoded as a yes. If the student response was 0 times, then those responses are recoded as a no. The analysis for this report is primarily descriptive and involves the computation of percentages and counts for the yes responses to each variable. Among the 8 th, 10 th and 12 th grades in the participating schools, 34.1% of the students are 8 th graders. Due to differences in classroom size and other factors beyond the control of the YAS Committee, the final YAS dataset contained 45.7% of its responses from 8 th graders. To increase the representativeness of the results, student response data was weighted using poststratification weights based on the published number of students in each grade of each participating Ottawa County school. If only unweighted results were reported, the 2015 YAS report would be biased toward typical 8 th grade behavior. Weighting takes this into consideration. In this report, weighted percentages are reported for every variable for the most accurate picture. In past years, if the question did not exist or is no longer comparable due to changes in wording, the data cell contains. Some questions asked in past reports are no longer included in the 2015 YAS. To view 2005, 2007, 2009, 2011 or 2013 YAS reports, visit and select a report in the Ottawa County Youth Assessment Surveys section. Results for the year 2005 were included in the analysis, but due to space they are not shown in this report Ottawa County Youth Assessment Sur vey 57

Limitations Considerations Regarding the Data Several issues are critical to consider when interpreting the results of the Ottawa County YAS. Use caution when comparing results between subgroups (such as Hispanic, White or Other) when the number of responding students is small or the outcome or behavior is relatively rare. This is due to the fact that the amount of error in a measurement grows as the number of respondents gets smaller. Where a question has fewer than five respondents reporting the behavior, the results are suppressed and replaced with * to protect student identity and statistical validity. School personnel administered the survey to students during class time in required subjects in each target grade until they met or exceeded the number specified by the YAS Committee. Since schools volunteered to participate in the survey and classroom selection was not controlled by the YAS Committee, the students completing the survey cannot be said to have been randomly selected. This introduces the potential that sampling bias could affect the results. With any survey, the issue of response bias could be a factor in the results. This would occur when students respond more inline with what they believe the researchers want to see in comparison to their actual belief. Because there are sensitive issues throughout the YAS, response bias could have influenced students responses to be more favorable. It is the opinion of the YAS Committee that the results of this report are reasonably reliable. Since the number of students completing the survey is large (about 40% of all nonhomeschooled 8 th, 10 th and 12 th graders) and maintains a good geographic representation of the county, it is reasonable to assume the data constitutes a representative sample of Ottawa County 8 th, 10 th and 12 th graders. Students who were absent or received their education at home at the time the YAS was given are not represented in the results. The Ottawa County YAS sampled 8 th, 10 th and 12 th graders; whereas the equivalent questions from the national and state YRBS were administered to 9 th, 10 th, 11 th and 12 th graders. Also the YAS method for selecting a sample of students to take the survey is different than YRBS sampling methods. Therefore, YAS results cannot be directly compared to state and national YRBS results. When comparing YAS results between 2005, 2007, 2009, 2011, 2013 and 2015 survey years, please be aware the students surveyed were not from identical districts each year. With each YAS administration, every effort was made to encourage all districts to participate. The survey conducted in 2015 provides the largest sample of 8 th, 10 th and 12 th graders in the history of the YAS. It is possible the change of the questionnaire format from paper to online could introduce an unknown element that impacts how students answer the YAS questions. Keep this in mind when comparing 2015, 2013 and 2011 results to previous years. 58
