3 Burden of the Disease Thyroid disorders are common and large observational studies estimate that 10% population have undiagnosed thyroid problems. Thyroid disorders are prevalent and most common among all the endocrine diseases worldwide. In India, about 42 million people reportedly suffer from the thyroid disorders such as hypothyroidism, hyperthyroidism, hashimoto s thyroiditis and thyroid cancer. However, the reported burden may be an underestimation as almost one-third of the hypothyroid subjects remain undiagnosed and are unaware of the condition. Diagnostic delay in the thyroid disorders is common and related to the lack of awareness amongst primary care practitioners, who are a key part of the health workforce. In addition to this, there are several issues that arise when it comes to the management of the thyroid disorders such as lack of availability of doctors (very low doctor to population ratio), concentration of specialists in urban areas and lack of training in management of thyroid disorders at graduate level (MBBS). In view of inadequacy of the present medical education system and shortage of specially trained physicians in India, capacity building of primary care doctors is considered to be an effective short term intervention in management of chronic conditions. Keeping this in view, in 2014, Public Health Foundation of India (PHFI), Delhi launched the Certificate Course in Management of Thyroid Disorders (CCMTD) for Primary Care Physicians (PCPs) in collaboration with Chellaram Diabetes Institute (CDI), Pune with an educational grant support from GlaxoSmithKline Pharmaceuticals Limited (GSK). The primary objective is to enhance knowledge and skills of PCPs in the management of thyroid disorders. 4 Rationale of the Course u Thyroid disorders are the commonest and prevalent among the endocrine disorders, the need for development of a standardized and intensive training module for its effective management with a systematic and coordinated patient care approach has gained prime importance. u Many recent studies and advances in prevention and management of thyroid are yet to be translated into practice. u Till now there have been limited efforts to address this disease in a comprehensive manner by the medical fraternity. (Thus there is need to sensitize as well as build capacity among primary care physicians for prevention of thyroid disorders in patients). Journey of CCMTD so far The first cycle of CCMTD was launched as country wide initiative on 21st June 2015 and conducted in 25 centers. In March 2016, second cycle of the course was rolled out in 35 centers. Advancing to successful completion of CCMTD Cycle I and II; Cycle III is intended to be rolled out in 35 regional training centers all across the country. The third cycle will be launched from January 2017 onwards. Overview of CCMTD Certificate Course in Management of Thyroid Disorders (CCMTD) is a joint certificate program designed and delivered by Public Health Foundation of India (PHFI) in collaboration with Chellaram Diabetes Institute (CDI), Pune. The educational grant for the same has been provided by GlaxoSmithKline Pharmaceuticals Private Limited, India. CCMTD is an on-the-job training program with four study modules spanning over four consecutive months with once a month contact session. The program had been conceptualized to train Primary Care Physicians (PCPs) across the country with an objective to improve patient outcomes by enabling prevention, early diagnosis, evidence based treatment of thyroid disorders and establishing a robust referral linkage. Primary Objective To enhance knowledge, skills and core competencies of PCPs in the management of Thyroid disorders. Secondary Objectives 1. To develop/ update a standard teaching protocol and module for evidence based learning on thyroid disorders. 2. To build a network of PCPs in the field of thyroid disorders. 3. Update PCPs with the latest advancements in the field of thyroid disorders. 5 Stakeholders Partner Organizations Public Health Foundation of India (PHFI) National Experts Program Secretariat CCMTD Educational Grant Partner The Public Health Foundation of India (PHFI) is a public private initiative that has collaboratively evolved through consultations with multiple constituencies including Indian and International academia, state and central governments, multi & bi-lateral agencies and civil society groups. PHFI is a response to redress the limited institutional capacity in India for strengthening training, research & policy development in the area of public health. PHFI recognizes the fact that meeting the shortfall of health professionals is imperative to a sustained and holistic response to the public health concerns in the country which in turn requires health care to be addressed not only from the scientific perspective of what works, but also from the social perspective of, who needs it the most. For more information, please login Regional Faculty Academic Partner Chellaram Diabetes Institute (CDI) Observers Participants Chellaram Diabetes Institute (CDI) is a unit of the Chellaram Foundation founded by Mr Lal L Chellaram. CDI is a not-for-profit organization based in Pune with the principal objective of creating a lasting impact on Diabetes in India as well as globally. This vision is intended to be achieved through four pillars, viz. patient care, education, research and awareness. In addition, CDI operates an ambitious rural outreach program, which combines all the four pillars, to the doorstep of the underprivileged rural population. 10 Dr. Anil Bhansali Chandigarh Dr. Bhansali is presently the Professor and Head of Endocrinology and Metabolism at Postgraduate Institute of Medical Education & Research, Chandigarh. He was instrumental in setting up the newer assay technique for hormone estimation, state-of-art diabetic foot lab and facilities for clamp studies. He is the Ex-President of the Endocrine Society of India and the Founder President of the Punjab and Chandigarh Chapter of Research Society of Study of Diabetes in India (RSSDI). Dr. Bhansali has been the recipient of numerous awards and fellowships, prominent among these are the Prof. M.N. Sen ICMR award for Pituitary disorders, Prof. M. Vishwanathan award for stem cell therapy in T2DM and, Prof. Shurvir Singh award for research in diabetes, Dr. Deepak Banerjee Award, MNF Gold Medal award for best research papers and Dr. P.N. Shah Memorial Oration Award for Metabolic Bone Disease. He has been visiting professor to the University of Graz, Austria. Dr. Bhansali s work in the field of diabetes particularly on stem cell therapy in type 2 diabetes and newer modalities in the management of diabetic foot have been highly appreciated. Autologous stem cells therapy in T2DM has shown promising results. Newer development of off-loading systems, angioplasty and stem cells therapy in peripheral vascular disease has led to development of new centre for diabetic foot in the country. His work on newer test for establishing diagnosis of T2DM that is HbA1c has been well received, as he was the first to provide this data from Asia. He has published 300 articles in indexed, peer-reviewed International and National journals of repute in the field of Diabetes and Endocrinology. Sethi Hyderabad, Telangana Dr. Sethi is a Consultant Endocrinologist in CARE Hospital, Hyderabad. He started as a Civil Assistant Surgeon in 1989 and went on to become Assistant Professor of Endocrinology in Osmania Medical College, He held the position for about 9 years. He held the position of Professor of Endocrinology in Gandhi Medical College & Hospital for around 5 years. Dr. Sethi completed his MBBS from Osmania Medical College, Hyderabad in 1982, MD (Medicine) in 1986 and DM (Endocrinology) in 1988 both from PGIMER, Chandigarh. Prasanna Kumar Bangalore, Karnataka Having completed his training at the prestigious All India Institute of Medical Sciences, New Delhi, with a distinguished career spanning three decades. His area of expertise is Reproductive Endocrinology & Diabetes. Dr. Prasanna Kumar is one of India s foremost endocrinologists. Well known in the Endocrine and Diabetes community as an excellent researcher and teacher, he has headed both the Endocrine Society and the Research Society for the Study of Diabetes in India a rare honor. For the past 20 years, his philanthropic vision was largely responsible for the creation of the Bangalore Diabetes Hospital where he was the CEO and where each weekend a large number of children with Type I Diabetes come for comprehensive care and free insulin supplies. He has been the principal investigator in more than 100 global clinical trials and has been responsible in several early pioneering work in translational research. dr.kmpk@gmail.com Dr. Krishna G Seshadri Chennai, Tamil Nadu Prof. Seshadri currently is the Professor of Internal Medicine & Endocrinology and Head of the department of Endocrinology & Diabetes at Sri Ramachandra University and its medical center. He is also senior consultant at Fortis Malar Hospital. He has graduated from Madras Medical College and pursued his Post graduation in the United States as a Resident in Internal Medicine (Illinois Masonic Medical Centre, Chicago) and proceeded to complete his Fellowship in Endocrinology at Cook county Hospital, Chicago. He has obtained his American Board certification in Internal Medicine, Endocrinology, Diabetes and Metabolism. In 1996, he was appointed as Lecturer in the University Of Illinois College Of Medicine. Prof. Seshadri has several indexed publications including original work reviews, written several book chapters and is an editor of year book on diabetes published by nature Macmillan. He holds editorial positions in several journals including Thyroid research in clinical practice and Indian Journal of Endocrinology and Metabolism. He has assumed editorship of a new quarterly on endocrine reviews by nature publishing. He is currently Executive Committee member of the Endocrine Society of India and secretary of the Endocrine Society Tamil Nadu. Dr Seshadri is also Secretary of the Endocrine Society of India. Prof. Seshadri is a recipient of the PN Shah Oration award by the Endocrine Society, He has recently been conferred the Sam GP Moses oration by the Research Society for the Study of Diabetes in India, He has conducted several pivotal multi centric trials in diabetes. His special interest includes translational research in molecular markers in thyroid cancer, inflammation and diabetes and Vitamin D and glucose uptake and has published work on Vitamin D and GLUT4 expression. A Harvard Macy Scholar, Dr Seshadri has helped in developing a competency based integrated medical education curricula and helped in framing the new guidelines for the MCI. He was co-opted to the working group to develop the new undergraduate curriculum for the MCI in 2011 and is a member of the postgraduate curriculum group for Endocrinology. He currently is the member of the graduate committee for medical education constituted by the medical council of India. 12 Dr. Manash P Baruah Guwahati, Assam Dr. Baruah is currently a Director of Excel Care Hospitals group and associated as a Consulatnt Endocrinologist with Excel Centre, a multi-specialty clinic in Guwahati, Assam. His research interest lies in Clinical Diabetology and Clinical Thyroidology. He completed his DM in Endocrinology from the reputed AIIMS (New Delhi). Dr. Baruah is an Associate Editor of Indian Journal of Endocrinology and Metabolism (ESI/ MedKnow), and Thyroid Research and Practice (ITS/MedKnow). 13 Dr. Nikhil Tandon New Delhi Dr. Tandon is currently Professor and Head, Department of Endocrinology & Metabolism at AIIMS, New Delhi, and Head, Capacity Building, Centre for Chronic Disease Control. He is an alumnus of the AIIMS, from where he also completed his MD (Internal Medicine). Subsequent to that he did his Ph.D. from University of Cambridge, UK in Dr. Tandon, an endocrinologist by training, is an accomplished clinician researcher, with research contributions spanning from investigating immunological mechanisms in autoimmune thyroid disease to studying the pathogenesis of chronic disease through epidemiological studies. His ongoing research includes studying the early life influence on adult chronic disease by being an investigator in the New Delhi birth cohort, translation research in the delivery of diabetes care and collaborative research in the area of genetics of type 2 diabetes. She has been associated with Public Health Foundation of India (PHFI) since 2013 and had worked in Certificate Course in Evidence Based Diabetes Management (CCEBDM) project of training PCPs in diabetes management. Presently she is working as Program Associate in CCMTD project. In the past, she was involved in National Certification Committee for Polio Eradication Project and had prepared the National Document for Certification of Poliomyelitis Eradication in INCLEN Trust International, New Delhi. Prior joining INCLEN she worked in World of Women (WOW) Corporation, where she did requirement gathering, data collection, training, prepared the software requirement specification document and did implementation of patient portal and physician portal.
