2 What is Acute Rehabilitation? Rehabilitation is a team of healthcare professionals working together to help people regain function or skills lost due to injury or disability. The Acute Rehabilitation at Baptist Health Madisonville is committed to providing the necessary rehabilitative care to return each patient to the most productive and fulfilling life possible. 3 Our interdisciplinary team of physicians, nurses, therapists, case managers and counselors all work together to provide a full range of acute rehabilitation services. A comprehensive plan of care is custom-tailored to each patient s specific needs. Families are encouraged to become actively involved in the healing process through caregiver education and counseling. Our greatest asset is our people. We all work together toward a common goal: to provide quality, comprehensive care to meet patients needs. We use various activities to help meet our patients needs, including the Nintendo Wii. 5 What types of injuries and illnesses are treated? l CVA stroke l Spinal Cord Injury l Congenital Deformity l Amputation l Major Multiple Trauma l Hip Fracture l Brain Injury l Arthritis l Neurological Disorders (multiple sclerosis, muscular dystrophy, Parkinson s, etc.) l Burns How does someone qualify for Acute Rehabilitation? The primary care physician will make a referral for a rehab evaluation. Once the referral is received, our admission coordinator will evaluate the patient and review their medical history, previous level of function, assess motivation status, determine rehab potential, and establish a payment source. The admission coordinator will present the information to the Rehabilitation team who will determine whether or not the Acute Rehabilitation is the most appropriate treatment environment. 6 How do you determine when to end the treatments? The rehabilitation team will determine the discharge date based upon the following criteria: l Completion of program and/or treatment goals met l Change in medical condition requiring more acute medical care l Patient s refusal or inability to participate in the recommended plan of care If you have other questions about our program, or if you believe Baptist Health Madisonville s acute rehab program can help you, please give us a call at or visit us online at BaptistHealthMadisonville.com.
