4 Background The council's membership includes 27 key community leaders focusing on health and social service delivery. The council fosters collaboration and cooperation.

5 PROBLEM STATEMENT Midland is aging. By 2025, individuals over 60 will be almost 30% of the population, compared with 22% in % of the adult population does no physical activity 33% of the adult population is overweight and 36% are obese In the past eight years, Midland has treated more people for opiate addiction than alcoholism In 2016, 798 students in grades 6-12 reported attempting suicide (The Legacy Center, 2016)

7 COMPLICATING FACTORS Transportation and poverty are complicating factors across each of the four focus areas Individuals with lower incomes were more likely to report their health as fair or poor Transportation remained a barrier to accessing needed health services and to utilizing recreational facilities

8 Priority Area: Later Life Quality Objective 1: Increase the number of individuals completing Advance Care Plans (ACP) in order to enhance quality of life Objective 2: Reduce the incidence of falls among older adults

9 LATER LIFE QUALITY PROGRESS 2016 ACHIEVEMENTS Falls prevention workshops to over 300 individuals Implemented Remembering When to prevent falls and kitchen fires, providing assessments to 106 older adults Public attention to Advance Care Planning through community presentations, Spiritual Care Conference, and presentation of Being Mortal documentary 2017 ACTIONS Promote the use of the Steadi Tool Kit falls screening protocol at primary care physician offices and encourage referral of at-risk individuals to one of the falls prevention programs. Increase the number of trained advance care planning facilitators by at least 10 in 2017

10 Priority Area: Substance Use Disorder Objective 1: Increase the number of people seeking services for Substance Use Disorder (SUD) Objective 2: Reduce the number of drug overdoses and meet the needs of those struggling with opiate dependency

12 Priority Area: Healthy Weight Objective 1: Promote a health weight initiative focused on increased activity and improved nutrition to multi-sector based community partners and engage their participation Objective 2: Develop a community wide healthy weight campaign composed of a healthy weight icon and corresponding message to distribute to community partners and to disseminate on multiple media sites and venues where community members live, learn, work, and play beginning January 2016 Objective 3: Increase awareness of and engage participation in evidence-based healthy weight behaviors by leveraging current efforts

13 HEALTHY WEIGHT PROGRESS 2016 ACCOMPLISHMENTS: 20 participants in the Exercise Prescription Program 1832 middle school students participated in extracurricular exercise programs before and after school breast feeding education or support contacts Over 500 direct contacts for nutrition education 154 participants in the Diabetes Prevention Program 39,000 senior activity and nutrition contacts

16 Our Community In addition to youth mental health concerns, national statistics indicate 12% of older adults are depressed and account for 20% of suicides each year Half of all Americans will meet the criteria for a diagnosable mental health condition at some point of our lives MidMichigan Medical Center indicated that from , 288 individuals sought assistance through the Emergency Department for suicide attempts. The Midland City Police had 154 calls due to thoughts of suicide, suicide attempts or deaths from suicide from Shortage of mental health care workers nationally is reflected locally

19 Objective #1: Mental health portal powered by 211 Improve information and referral for mental health services Currently working on Hope Portal: The primary purpose is to be the reliable entry point for those who are seeking information and resources related to mental health needs.

20 Objective #2: Suicide Prevention Ensure individuals who are at risk for suicide are properly screened for suicide at every health encounter and that an effective process for on-going intervention is in place. Develop a standard of care for screening for suicide including a policy specific to suicide care, screening questions and referral processes. Develop a training system on suicide risk factors, best practices for suicide prevention and assessment processes. Enhance the process for referrals and after care support.

21 Objective #3: Jail Diversion People with Mental Illness will be diverted out of the criminal justice system when appropriate Increase the percentage of people with mental illness and intellectual/ developmental disabilities in jail being referred to the right services and supports Decrease the percentage of those with mental illness or intellectual/developmental disabilities who recommit a crime through community treatment Community partners work together on this issue

22 Objective #4: Mental Health Services in the Schools Increase mental health services in Midland County Schools Train school professionals to respond effectively to youth with mental health problems Promote use of classroom mental health website Implement Youth Intervention Program Promote OK2Say Program Use MindUp Curriculum (mindfulness training) Start middle school suspension program

23 Objective #5: Public Awareness/Communication Raise awareness, educate our community about mental health issues, and keep residents up to date on health and suicide services available in our community. Increased awareness in community Reduced number of suicides/attempts Less stigma associated with mental health issues

24 Objective #6: Research Advisory Committee Pilot: Mental Health Research Advisory Committee for Midland for 2017 Monthly meetings Identify and/or apply for other research grants to support improving mental health in our community Provide feedback for mental health programs, research or media campaigns developed by the Mental Health Gap working group or targeted for Midland County
