Learning Objectives Review the problems o small ield dosimetry and the solutions that have been identiied Learn about the IAEA-AAPM recommendations and data or small ield dosimetry

Overview The problems in small-ield dosimetry The IAEA dosimetry ormalism Conclusions

What constitutes small-ield conditions? Beam-related small-ield conditions the existence o lateral charged particle disequilibrium partial geometrical shielding o the primary photon source as seen rom the point o measurement Detector-related small-ield condition detector size compared to ield size

Lateral charged particle loss broad photon ield narrow photon ield volume volume A small ield can be deined as a ield with size smaller than the lateral range o charged particles is a measure o the degree o charged particle equilibrium or transient equilibrium

Lateral charged particle loss Concept o r LCPE MC calculations, Seuntjens (2013)

Detector size relative to ield size Small ield conditions exist when one o the edges o the sensitive volume o a detector is less then a lateral charged particle equilibrium range (r LCPE ) away rom the edge o the ield r LCPE (in cm) = TPR 20, (Li et al Med Phys 22, ) Slide courtesy: H. Palmans

Detector-related small ield condition Meltsner et al., Med Phys 36:339 (2009) Exradin A16 outer diameter Exradin A16 inner diameter Based on criterion 1, one could claim that the GammaKnie 18 or 14 mm diameter ields are not small (quasi point source + electron equilibrium length about 6 mm).

Detector dependence o output actor From Sanchez-Doblado et al Phys Med 23:58-66

Detector issues in small ield dosimetry Energy dependence o the response Perturbation eects Central electrode Wall eects Fact that cavity is dierent rom water, luence perturbation Volume averaging These eects depend somewhat on the beam spot size

Detector issues in small ield dosimetry Dosimetry protocol values (e.g., TG-51) o these actors are applicable usually only in TCPE and only or the conditions: 10 x 10 cm 2 ; z re = 10 cm; SSD or SAD 100 cm 15

Stopping power ratio water to air 0.5% eect Very small eects! Andreo&Brahme PMB 8:839 (1986) Eklund and Ahnesjö, Phys Med Biol 53:4231 (2008)

Role o dierent perturbation actors PP31006 and PP31016 chambers Crop et al., Phys Med Biol 54:2951 (2009)

Magnitude o correction actors on and o-axis 8 mm x 8 mm ield, 10 cm depth (0.6 mm, 2 mm spot sizes) Relatively small eects! Very large eects! Very large eects! Crop et al., Phys Med Biol 54:2951 (2009)

Summary o issues leading to dosimetric uncertainties in small ields Beam dependent issues Beam ocal spot size Lateral disequilibrium How do we measure beam quality in practice? Detector eects There is no ideal detector Volume averaging and luence perturbation eects Corrections depend on beam spot size

What are the single set o two largest contributors to correction actors and their uncertainties or commercial air-illed ionization chambers in small photon ields? 1% 1. The stopping power ratio and the central electrode eect 5% 75% 17% 3% 2. The stopping power ratio and the chamber wall eect 3. The luence perturbation eect and the volume averaging eect 4. The stopping power ratio and the volume averaging eect 5. The ionization chamber wall eect and the stem eect

Correct answer: 3 The luence perturbation eect and the volume averaging eect Discussion: The ield size dependence o stopping power ratios is 0.5% or less. For most ionization chambers the ield size dependence o wall corrections is limited to a ew percent. The volume averaging and luence perturbation corrections are potentially very large (on the order o 10-30% or more depending on the situation) Reerence: Crop et al (2009) Phys Med Biol Bouchard et al (2009) Med Phys 36 (10),

Which two competing eects lead to ield size dependent correction actors o unshielded diode detectors? 1. Intrinsic energy dependence o Si in photon beams and volume averaging 2. Intrinsic energy dependence o Si in photon beams and perturbation eects 3. Polarity eect and recombination 4. Polarity eect and electrometer calibration 5. Recombination eect and diode doping 78% 14% 3% 5% 0%

Correct answer: 2 Intrinsic energy dependence o Si in photon beams and electron luence perturbation eects Discussion: Volume averaging is usually small in diodes because o the small size o the sensitive volume. Diodes are not polarized by an external bias, so there is no polarity eect. Recombination eects and diode doping are not relevant in this context. 52 For what purpose is the measurement o the beam quality speciier required? 1. To speciy the correction actors to be applied to the output ratios measured in small ields 2. To speciy small ield output actors 3. To speciy the beam quality correction actor in the ield 4. To ensure the beam is o adequate quality 5. To speciy the absorbed dose calibration coeicient or a small ield 58% 22% 13% 6% 1%

Correct answer: 3 To speciy the k,re beam quality correction actor in the ield Discussion: In general, no beam quality measurement is perormed in small ields, only in ields. Reerences: Palmans 2012 Med Phys 39: 5513
