2 Item Action Introductions, apologies, minutes Mr (Chair) welcomed all present and accepted apologies. The minutes from last meeting on 10 th February were approved. SCAN Issues- Latest Developments Peter Burns a representative form the Ben Walton Trust who introduced his project on Oral Cancer to the group at the February meeting has since confirmed that patients with cancer of the Oropharynx should be included in the study. The study is very short of numbers as few patients are willing to participate. Speech and Language Update There is currently a full compliment of SLT to support the H&N group with Kirsty McLachlan and Nadine Hare both returning form maternity leave in March. Waiting Times reported that the lack of clinics over the Christmas and New Year period in 2011 resulted in several breaches in waiting times. This should be noted and plans made to avoid a similar situation for 2012 where Christmas day falls on a Tuesday (H&N MDM). There has been no annual leave cover for the H&N Cancer Tracker, Janice Larkman which results in a back log of patients. A member of the admin staff is to be trained to work along side Janice and be available to step in when required. The majority of patients are referred by GPs using the Urgent or Urgent with Suspicion of Cancer criteria. Scottish referral guidelines can be found on It was noted that there is a capacity issue for radiotherapy planning with only two slots a week available for IMRT planning. This can potentially lead to a delay in treatment although it is anticipated that there will be an increase to 4 slots by the end of Service management Clare Gorman was unable to attend the meeting as planned. We await an update on service management issues. Neck Lump Clinic As a result of a Lean in Lothian project the Neck Lump clinic in East Lothian has been successfully running under the guidance of Richard Adamson and Andy Evans since 23/03/12. This is designed as a door into the service for patients with neck lumps where they will be assessed and discharged if found to have benign conditions. To date there are 87 patients on the neck lump database. commented that a similar clinic would be of benefit in West Lothian. M&M Meeting An M&M meeting, chaired by Andy Evans, initially planned for April 2012 has been rescheduled for August 31 st in the Breast Seminar Room WGH 2pm-4pm. Clare Gorman Andy Evans 2
3 Survival Analysis reported that as part of the H&N National meeting planned for October 2012 ISD will provide a survival analysis of 2006 patients. ISD will provide a template for completion by Valerie Findlay (Lothian, Borders and D&G) and Maggie McHardy (Fife) by 11th July. It has yet to be decided on what fields or cohort the data will be analysed. A meeting took place on 29 th March between and Dr Janet Ironside, Dr Elizabeth Junor and Cat Graham(Lead Statistician, Wellcome Trust Research Facility) to discuss the possibility of an independent survival analysis. Cat Graham is expected to supply a cost for her service before the analysis can proceed. Shirley Anne Savage has offered to carry out a 10 year survival analysis on H&N patients in SCAN and present her findings at the National Network meeting in October. Shirley Anne Savage SCAN Website Sheena Mackenzie gave the group a tour of the new updated SCAN website and in particular those areas relating to H&N cancer. Transforming Care After Treatment (TCAT) The details of this new national programme, which is being supported by Macmillan, have not been finalised but areas of interest include survivorship and models of follow up. Kate Macdonald and Val Doherty are on the national group. Expect some questions about your follow up protocols and processes! Sheena Mackenzie Chemotherapy Electronic Prescribing and Administration System (CEPAS) Scheduling at the final site, Dumfries, is scheduled to be implemented this month. Prescribing is due to be implemented on all sites by August. Chemotherapy CEL Due to be published in August. Radiotherapy Capacity The scoping project undertaken by WOSCAN and SCAN recommended that WOSCAN should develop a business case for a West of Scotland satellite radiotherapy centre at Monklands Hospital. The Scottish Government has agreed that work on radiotherapy capacity in SE Scotland will be undertaken in conjunction with the new Radiotherapy Programme Board. Audit Annual Comparative Report The 2011 Annual Comparative Report will aim to report on the same clinical effectiveness measures as for 2010 and will include an update on Actions Points listed. National Meeting A National H&N Cancer meeting hosted by WOSCAN is planned for 26 th October ISD will present the analysis of 2011 data from across the SCAN region as well as a survival analysis of 2006 data. Members of the group are invited to register and contribute topics for discussion to the meeting. As yet an agenda has not been finalised by WOSCAN although we expect an update following an advisory board meeting on 12 th June. Guy Vernham/Richard Adamson 3
4 QPI- Development The National Cancer Quality Steering Group (NCQSG) has opted to progress the development of Head and Neck Cancer Quality Performance Indicators (QPIs) as part of the ongoing national quality work programme. They are now looking to determine membership of the Head and Neck Cancer QPI Development Group to enable work to be progressed timeously. They would like to identify, via regional networks, key opinion leaders involved in the management of Head and Neck Cancer who will be invited to participate in this work, either as part of the core QPI development group or a wider clinical reference group. The core group (approx ) will comprise a range of clinical experts drawn from different disciplines across the Networks, lay representatives, Healthcare Improvement Scotland and NHS Management. The membership of this group is key to ensuring system wide engagement of clinicians and therefore the success of the project. As a first step key opinion leaders involved in the management of Head and Neck Cancer from within the network should be identified against each of the categories in the table provided by Iona Scott (circulated to the group). From the pool of individuals identified by this process a core QPI development group will be formed.. Those nominated but not involved in this core group will form a wider reference group that we will consult on specific elements of QPI development and initial drafts. Head and Neck Cancer QPI Development is due to commence in September 2012 (first meeting), with preparatory work by Healthcare Improvement Scotland already underway. Nominations should be forwarded to by Friday 22nd June, which will allow for sufficient notice of the first meeting to be given to those nominated for the group. Theatre Capacity for Max Fax Patients Carolyn Kear and Jim Morrison highlighted the lack of theatre capacity for Max fax patients. Mr Morrison was able to identify cases where patients had to be reassessed because of an excessive wait for a theatre slot thus compromising their treatment. agreed to write to Sandra Mair as Chair of SCAN highlighting the problem. Dumfries & Galloway Service Structure Patients diagnosed at D&G (SCAN) are the reviewed by either ECC MDM or Southern General MDM (WOSCAN) depending on their diagnosis. Historically this means that maxillofacial patients are reviewed by Glasgow MDM and then treated either in Glasgow or in Dumfries. D&G ENT patients are generally reviewed by the ECC MDM and either treated locally or in Edinburgh. This cross board pathway makes it difficult to capture all the D&G patients and ensure equity of care. Mr Orabi, the most recent ENT surgeon in D&G expressed his concern over current practice. Any Other Business Valerie Findlay will be leaving the H&N group to take up a full time position with the Haematology group on July 16 th. thanked Val for her valuable contribution to the Group and wished her well for the future.
