5 Semi-Annual Perfrmance Rebates: Participating Members will qualify fr a semi-annual cntract perfrmance rebate ( Perfrmance Rebate ) f 3% n all GSK Vaccine Prducts if required aggregate Share Requirements are btained during each semi-annual perid listed belw. Evaluatin and Pricing Perids: The evaluatin perids fr the Semi-Annual Perfrmance Rebate are as fllws: Evaluatin Perid Rebate Perid 07/01/16 thrugh 12/31/16 07/01/16 thrugh 12/31/16 01/01/17 thrugh 06/30/17 01/01/17 thrugh 06/30/17 If a Participating Member is added during a current Rebate Perid, member will be eligible t earn Perfrmance Rebates frm their effective membership date. Participating Members added during an Evaluatin Perid will be included in the market share calculatin at the beginning f the next Evaluatin Perid and will nt be included in the market share calculatin fr the then current Evaluatin perid. If a Participating Member attains the required Share Requirements, member will earn a 3% Perfrmance Rebate n all Cntract Net Sales f GSK Vaccines purchased during the Rebate Perid. Rebates will be paid apprximately sixty (60) days after the end f the Rebate Perid. Ttal payment must be equal t r greater than $100 r Perfrmance Rebate will nt be paid. Cntract Pricing: Cntract Pricing fr each pricing perid may be earned by Participating Members n GSK Vaccines listed belw, if during each crrespnding Evaluatin Perid, participating members, as a grup, achieve and maintain the required aggregate market share perfrmance level. If the participating member des nt achieve the required aggregate market share perfrmance level as required and utlined belw, the Participating Member may be mved t the GSK individual Vaccine Pricing Agreement ( ivpa ) and will n lnger be able t receive Cntract Pricing. In the event that GSK is unable t supply any f the vaccines identified belw fr an extended perid f time, then GSK may adjust Share Requirements fr applicable (Bucket) fr affected evaluatin perid. [5]
