3 SUMMARY General objectives The general objective of the project was to improve the epidemiological information on invasive meningococcal and Haemophilus influenzae disease within the European Union and the laboratory capacity to accurately characterise the isolates of H. influenzae and Neisseria meningitidis. In addition, the project was also aiming at evaluating the impact of vaccination with conjugate vaccines on the epidemiology of H. influenzae and N. meningitidis, and compare the impact of vaccination with conjugate vaccines produced by different manufacturers and according to different schedules. Finally, EU-IBIS was also intended to form a focus for wider collaboration with non European Union countries and candidate European Union countries. Strategic relevance and contribution to the public health programme The development of web-based system for rapid exchange of information on meningococcal infection had an international importance. EU-IBIS was intended to develop a web-based notice board for rapid dissemination of information across the Community Network (epidemiological surveillance structures) of Decision 2119/98/EC and the EU-IBIS network and hence EU, allowing appropriate action to be taken with contacts of travel-associated cases. Methods and means A separate disease surveillance network has been established for each of H. influenzae and N. meningitidis, and these run independently. Standardised case definitions laid down by Community legislation were used. Where surveillance is performed using other definitions, datasets were re-coded to provide comparable data for all participating countries. The descriptive epidemiology was analysed using standard statistical packages on the minimum data set provided for each infection (including age, sex, date of onset, method of confirmation, site of identification, grouping, typing and subtyping results as appropriate). Analysis of age-specific incidence rates, seasonality, temporal trends and diversity of H. influenzae and N.meningitidis infections were examined. In countries with vaccination programmes, coverage data was also requested and comparison of rates of infection in both vaccinated and unvaccinated cohorts was interpreted in conjunction with coverage, schedule and vaccine used, implementation and method of introduction. Where a need was seen, sentinel reference laboratories were asked to report cases of an emergent strain of H. influenzae or N. meningitidis, and this information was sent to the Community Network (epidemiological surveillance structures) of Decision 2119/98 EC and shared on the Eurosurveillance bulletin A Web enabled database was developed and an alert page was warning of any trends in participant countries that may have implications for other member states or for countries that may access the Page 3/12

Expected outcomes period EU-IBIS / H. influenzae and Meningococci network had to provide annual reports to network participants and the Community Network (Decision 2119/98/EC) on the epidemiological data analyses, reports to network participants on EQAS analyses, presentation of surveillance network results at EU-IBIS network meetings and relevant scientific conferences. EU-IBIS was also intended to publish its results in peer review journals, in Eurosurveillance and on Internet. In addition, for EU-IBIS/ Meningococci network, the project developed a Web-enabled database in order to allow the rapid transfer of data on imported and emergent strains of N. meningitides. EU-IBIS was also inteneded to report on progress towards harmonisation of policy on control of N. meningitidis and on the actions needed to achieve it. Page 4/12
