3 SUMMARY Mission and vision of the operating grant holder Measures of mortality are among the most important indicators for public health. All European Union member states collect all-cause mortality data in order to facilitate planning, monitoring annual trends of diseases and evaluating public health interventions. However, there is little tradition of conducting timely monitoring of mortality. This lack of timely collection, analysis and interpretation is a limitation when it comes to the assessment of the health impact of events that may lead to sudden changes in mortality such as major epidemics, extreme temperatures as well as deliberate or accidental release of biological or chemical agents. In the DG Sanco supported project EuroMOMO we developed a coordinated approach to real-time mortality monitoring across Europe. This system is applied among countries that are able to meet the minimal requirements for mortality monitoring. Currently, the weekly published EuroMOMO mortality bulletin at is based on data submitted from 18 countries or regions of countries. The EuroMOMO network demonstrated how a timely, standardised and coordinated approach to mortality monitoring increases the European capacity to assess the impact of events with a potential impact on public health, including the 2009/10 influenza A(H1N1) pandemic. Without EuroMOMO, Europe would have been less well prepared for the pandemic In addition, EuroMOMO was pivotal to facilitate country-specific in-depth analyses of influenza-associated mortality in general or the impact of the pandemic more specifically. Our mission is to reinforce the EU`s preparedness to respond to potential risk of all hazards by a continued operation of the EuroMOMO network. The vision is further to extend the collaboration by adding new countries to the network. By conducting regular network activites, partners will learn and exchange best practice on how to apply data from real-time mortality monitoring for risk assessment, in particular as regards cross-border threats and the management of the public health response to emergencies. Strategic objectives and specific activities The general aim of the EuroMOMO network is to operate coordinated real-time mortality monitoring and analysis using the EuroMOMO system across as many European countries as possible, in order to improve the capacity to assess risks associated with major health threats, including pandemic influenza and other infectious or non-infectious public health events with a severe impact(i.e. an allhazards approach). EuroMOMO system is a European consensus system developed, tested and implemented in 2009 by the original EuroMOMO project ( , Co-funded by DG Sanco). Using a standardized method the system estimates weekly all-cause mortality (observed mortality minus expected mortality) in participating countries which is comparable across populations and subpopulations in European countries Page 3/9
4 Since autumn 2009 weekly European mortality bulletins are published on the networks euromomo.eu and EuroMOMO partners have regularly submitted weekly mortality outputs to the network hub at Statens Serum Institut. The number of participating European countries or regions of countries has increased from 10 in 2009 to 18 in The specific objectives of ongoing operational activities at the EuroMOMO hub are: To produce quality checked weekly EuroMOMO reports of: o weekly crude all-cause mortality for each participating country and standard age group o weekly expected all-cause mortality for each participating country and standard age group o delays in mortality reporting at country level o sustained excess mortality patterns for each studied country and age group o comparisons of European mortality patterns with influenza- and climate-related patterns. To produce an end of season summary of the 2013/2014 influenza season To provide timely excess mortality data to ECDC, the commission and WHO To strive to expand mortality monitoring in Europe Specific objective for work year 2014: To review and if necessary revise the EuroMOMO output based on recommendations from EuroMOMO network partners and EuroMOMO Advisory Board members To statistically review and, if necessary, to revise the EuroMOMO algorithm based on recommendations from EuroMOMO network partners and EuroMOMO Advisory Board members. To implement the revised algorithm in partner countries before week /2015 (i.e. the start of the influenza season) To train network partners in the implementation and use of the revised algorithm Main activities: Preparations and publication of weekly quality checked European mortality reports Maintenance of the EuroMOMO system Technical support to EuroMOMO partners, including advice and training in the use of the EuroMOMO system and the analysis of outputs Maintenance of the EuroMOMO website Review and revision of the EuroMOMO system Organising and conducting a EuroMOMO network meeting Organising and conducting communication team and advisory board meetings Country visits to countries interested to join EuroMOMO Monitoring of the progress of operation, ensuring that objectives and deliverables are met Dissemination of EuroMOMO outputs and information to the relevant stakeholders, incl DG Sanco, ECDC and WHO, and to attend relevant scientific conferences and meetings, prepare peer-reviewed publications Expected outcomes Page 4/9
5 To meet the operational objectives described in section we use the EuroMOMO algorithm and procedures that have been applied as a common tool for producing weekly indicators, including: Observed number of all-cause deaths, expected number of deaths (baseline), number of deaths corrected for delay in data transmission, deviations from the baseline: Difference between observed and expected number of death, expressed in numbers and as standard deviation scores (z-scores) for instant comparison. The indicators are reported by total population and by age group (=65 years), reflecting the same age groups that are used by the European Influenza Surveillance Network. Partners run the common algorithm on their mortality data to compute the agreed indicators and sent the weekly results to the network hub, where they are stored in a central database. At the hub all received results will be quality checked. For the countries where the statistical software Stata is not available, the algorithm may be run for the country by the hub and results then sent back to the partner institute. The hub is responsible for maintenance of the algorithm as well as for technical support and guidance for the participating countries and to those intending to participate. The hub analyses and pools data from participating countries every week and produces a estimation of the crude number of deaths and expected baseline for all partner institutes participating in that specific week, adjusted for heterogeneities between participating counties. At the hub mortality outputs are compared with prevailing influenza- and climaterelated patterns. When there are excess mortality signals the hub invites partners to carry out and report in depth analysis of their national data and facilitates communication with risk assessment at ECDC. The hub prepares the weekly mortality bulletin, a draft version is sent to the partners for review and comments. The hub maintains a website where weekly European mortality reports are published ( The mortality bulletin is updated every week on Friday afternoon. The results from the participating countries and pooled analysis are accessible to the participating institutes and the European international public health counterparts. In accordance with the current data agreement with partner countries, only pooled results are released for public viewing - however, this policy is under review at present. The EuroMOMO bulletin text is sent to ECDC and WHO Europe for inclusion in their weekly Influenza surveillance bulletins. In spring the hub will produce and publish a partner reviewed end-of-season summary of European mortality patterns during the 2013/2014 influenza season. Further hub activities include Annual network meeting, communication team and advisory board meetings. The network hub offers advice and training in the implementation and use of the EuroMOMO algorithm, on the analysis and assessment of mortality outputs as well as facilitate risk communication between member states and relevant European agencies, such as ECDC and WHO Europe. A main objective is to expand the number of mortality monitoring partners in EU, by inviting non-partner countries to network meeting, country visits, advocacy through relevant channels (e.g.ecdc Advisory Forum) visibility through scientific publication and presentation at conferences. Countries from all EU will be invited to the EuroMOMO annual meeting. The review and revision of the EuroMOMO system will include the EuroMOMO algorithm with the delay adjustment and the European output. Basis for the revision will be recommendations of both the internal and external EuroMOMO network stakeholders. They will be derived from networks, coordination Page 5/9
