The schedule refers to full working days in lab Monday thru Friday. Pick-up and delivery days are not considered working days. Pick-up and delivery service is available throughout metro Atlanta for a $4.00 fee per case. Out of town cases should be sent to us by either Priority Mail and UPS Ground (at no charge) or United Parcel Service (UPS) airpack (Second Day Service - Blue - $7.95, Next Day Service - Red - $9.95). All out of town cases are returned via United Parcel Service (UPS) for a $6.00 per case fee. Policies INFECTION CONTROL - OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION (OSHA) It has been determined that many tasks within the dental laboratory are classified as Category I or Category II hazards as defined by OSHA. We are committed to a proactive role in providing our employees, dentists and their patients with protection from the dangers of cross-contamination. A charge of $1.75 is added to each case to help defray the costs of our comprehensive compliance program. PAYMENT TERMS All work done by Lab is due in full upon receipt of each monthly statement. Lab offers a two percent (2%) discount for payment in full by check within ten (10) days of statement date. Unpaid balances over thirty (30) days from statement will be charged an additional 1.5% interest per month (18% annually). Customer agrees to provide a valid credit card on account with Lab at all times and any unpaid balances after 61 days will be automatically be charged to Customer s credit card. If Customer declines to provide a valid credit card on account, or if the credit card on file is not valid and payment is rejected, all accounts will be classified C.O.D. and Customer agrees to pay all balances on C.O.D. basis plus outstanding balances, if any. PRODUCT REMAKE POLICY All New Image products carry a 100% satisfaction guarantee for the appropriate term of each product. This covers the full replacement cost of all materials and workmanship of the prosthetic produced according to the dentist s written prescription. When a removable product fits the model produced from the dentists impression and does not fit intraorally, New Image shall not be responsible for the full cost of refabrication. In this case, if all models and product are returned to New Image, the charge for product remake will be 50% of the original price. Assist you in keeping your restorative and removable appliance patients on a regular dental maintenance program. To keep the Warranty valid, the crown and bridge patient must adhere to a semi-annual dental maintenance program (cleaning and exam). The full or partial denture patient must have the prosthesis relined as recommended by the dentist, in addition to the 6-month recall. 3. Increase the traffic or patient load for the dentist using this service as a tool for his recall program. This program is available on crown and bridge cases. It is self-explanatory to the patient as details are on the back of the Warranty.* WHAT IS COVERED 1. Repair or replacement of appliance. WHAT IS NOT COVERED 1. Cash refund for prosthesis. 2. Full or partial dentures must be relined as recommended to the attending dentist by New Image Dental Laboratory, Inc. 5.
