We are committed to a proactive role in providing our employees, dentists and their patients with protection from the dangers of cross-contamination. A charge of $1.75 is added to each case to help defray the costs of our comprehensive compliance program. PAYMENT TERMS All work done by Lab is due in full upon receipt of each monthly statement. Unpaid balances over thirty (30) days from statement will be charged an additional 1.5% interest per month (18% annually). Customer agrees to provide a valid credit card on account with Lab at all times and any unpaid balances after 61 days will be automatically be charged to Customer s credit card. If Customer declines to provide a valid credit card on account, or if the credit card on file is not valid and payment is rejected, all accounts will be classified C.O.D. and Customer agrees to pay all balances on C.O.D. basis plus outstanding balances, if any. Credit Cards: Visa, Mastercard or American Express are accepted by New Image Dental Laboratory. C.O.D.: $10.50 Shipping Charge, $35.00 Charge on all returned checks. PRODUCT REMAKE POLICY All New Image products carry a 100% satisfaction guarantee for the appropriate term of each product. This covers the full replacement cost of all materials and workmanship of the prosthetic produced according to the dentist s written prescription. When a removable product fits the model produced from the dentists impression and does not fit intraorally, New Image shall not be responsible for the full cost of refabrication. In this case, if all models and product are returned to New Image, the charge for product remake will be 50% of the original price. If models are not returned to the laboratory, the cost of remake will be 100% of the original price. IMPLANT COMPONENTS Componentry can be ordered by the laboratory for your convenience. However, be advised any rush shipping and handling fees will be added to the final cost of the components when invoiced. Prosthesis must be inserted by a licensed practicing dentist. 2. Patient must adhere to a semi-annual dental maintenance (cleaning and exam) program in the office of a licensed practicing dentist. 3. The maintenance schedule on this Certificate must be documented by the attending dentist each visit to validate this Warranty. 4. Full or partial dentures must be relined as recommended to the attending dentist by New Image Dental Laboratory, Inc. 5. Within the limitations of this Warranty, the prosthesis, written work authorization and the Warranty Card must be shipped together to New Image Dental Laboratory, Inc.
