1 DeCare Dental Encompass Network Participating Dentist Directory Serving Securian in Maine DeCare Dental and Participating Dentists are independent contractors. DeCare Dental and Participating Dentists do NOT have a relationship of employer and employee nor of principal and agent. No such relationship other than that of independent parties contracting with each other solely for the purpose of Participating Dentists' provision of dental care to subscribers may be deemed to exist or be construed to exist between DeCare Dental and Participating Dentists. A Participating Dentist is solely responsible for the dental care provided to any patient, and DeCare Dental does not control the detail, manner, or methods by which a Participating Dentist provides care. Securian Dental is underwritten by Securian Life Insurance Company, 400 Robert Street North, St. Paul MN 55101, and administered by DeCare Dental Health International, LLC. Securian Dental is offered under policy form series or a state variation thereof.

3 Your Dental Network The more you understand about good dental health, preventive care and the role your dentist can play in the overall health of your teeth, gums and mouth, the greater the chances are that your teeth will last you a lifetime. Your goal should be to get the right care at the right time and place for the right price. You are well on your way when you choose one of the dentists listed in this directory. Advantages of using a participating dentist (Encompass) participating dentists have signed an agreement to accept our allowable charge as payment in full for covered dental care. (If your benefit contract has deductible and coinsurance provisions, the dentist will bill you for these amounts.) A network dentist will not balance bill more than Encompass allowable charge. Participating dentists usually file the claim directly with us, relieving you of any paperwork. Encompass participating dentists will receive payments directly from your dental plan. If you use a non-participating dentist, you may be balance billed for the difference between their charge and the Encompass scheduled allowance. Check your Certificate of Coverage or call one of our customer service representatives if you have a question. Making an appointment Selecting a participating dentist is easy. You do not need to notify us. Simply schedule an appointment with the participating dentist of your choice, and he or she will take care of the rest. To select a Encompass participating dentist near you, consult this directory, which has been arranged alphabetically by city. Because the list is continually growing, it s a good idea to check our web site (securiandental.com) or call us at to get updated information if you do not find a listing for the dentist or area you are looking for. Credentialing The following dentists have met the criteria for participation in the Encompass by being duly licensed by the State of Maine Board of Dental Examiners, maintaining malpractice and liability insurance, holding a current Federal Drug Enforcement Administration certificate (if applicable), and signing a Participating Dentist Agreement DeCare Dental Encompass serving Securian dental plan members. *Before scheduling your appointment, please check with the dentist s office to confirm they are participating, or if you are a new patient whether they are accepting new patients.
