3 Overview REINVENTING ORAL HEALTH: A Midwest Collaborative Initiative When people align around shared political, social, economic or environmental values, and take collective action, thinking and behavior that compromises the lives of millions of people around the world can truly change. Simon Mainwaring OVERVIEW Through the DentaQuest Foundation Oral Health 2020 effort and the Midwest Collaborative Initiative, Oral Health Kansas, Michigan Oral Health Coalition and Wisconsin Oral Health Coalition have done just that as they endeavor to reinvent oral health. Through funding by the Foundation, the Michigan Oral Health Coalition strives to align national, state and local partners into a formal network. Success was achieved by developing a community network model that created local oral health coalitions throughout the state of Michigan that worked to improve the oral health of children and adults through new ideas, activism and leadership. In 2015 to further the DentaQuest Foundation s initial investment, a partnership was created between Michigan and the Kansas and Wisconsin state oral health coalitions to create a tri-state movement of innovation, advocacy and leadership to eradicate dental disease by This partnership formally connects the local stakeholders to the state and national stakeholders ultimately making them a network for change within each state, as well as the region. Kansas, Michigan and Wisconsin are ideal partners. As the collective voice of oral health in their respective states, each has a broad circle of influence among traditional and non-traditional stakeholders and has facilitated consensus-based support around State Oral Health Plans, advocacy and policy work. Combined, the three state oral health coalitions advocate for 18.6 million Americans in a politically conservative environment. Adding to this challenge, 116 counties are HRSA-designated dental health professional shortage areas among the three states. In 2016, nineteen local oral health coalitions tackled these difficulties to advocate for specific population groups with unique oral health issues in their communities. The connection they have to other coalitions within the tri-state network continues to strengthen their efforts. Each of the three state coalitions include a diverse leadership and membership which represent education, providers, government, community, third party payers, policy, higher/professional education and the general public who are eager to link together by sharing their ideas and expertise. One definition of oral health maintains that oral health contributes positively to physical, mental and social well-being and the enjoyment of life s possibilities, by allowing the individual to speak, eat and socialize unhindered by pain, discomfort or embarrassment. A core principle of the Initiative is that everyone should be able to attain a state of oral health that will enable them to enjoy a healthy and full life. On the following pages, we share our accomplishments that have been helping us move toward this principle and the current environment as it relates to the bold, shared goals to be achieved by Work continues in the state and regional network! The work we are doing to build local coalitions enables us to focus on the oral health needs of young children, seniors, recent immigrants, minorities, people living in poverty, and those with disabilities. The community oral health coalitions supported by the Midwest Collaborative Initiative address the Oral Health 2020 vision and Oral Health Kansas values. Each coalition designs goals and strategies that best reflect the needs of its community. As Oral Health Kansas works with each community s coalition, we are able to identify disparities along racial, ethnic, cultural, and socio-economic lines in accessing good oral health. 6 2016 KANSAS Reinventing Oral Health Butler County BUTLER COUNTY 66, County Health Ranking 21 st of 105 8, % No KanCare For many years, a coalition in Butler County has addressed the health needs of the area s youngest residents. Through the Midwest Collaborative Initiative, the coalition added an oral health focus in 2015 by incorporating dental screenings in early childhood developmental screenings. During the school year, dental hygienists screened 200-plus children residing in Butler County. Although data is incomplete, a preliminary analysis indicates more than 200 children will be screened in the school year. Butler County Oral Health Coalition also began to include fluoride varnish as a part of the services the children receive during their developmental screening and children are referred for follow-up professional dental care when additional care is indicated. The Coalition provides each child who is screened with a tooth-brushing kit and a two-minute timer with the Coalition s logo. In 2017, Coalition members hope to encourage other early childhood screening programs in Kansas to adopt a dental screening process. 7 2016 KANSAS Reinventing Oral Health Finney County FINNEY COUNTY In the early 2000s, an oral health coalition in Finney County came together to fight for community water fluoridation. After the coalition s success, the members disbanded. Finney County Oral Health Coalition re-formed in 2015 through the Midwest Collaborative Initiative with the goal of educating families enrolled in private and public dental insurance, many of whom are immigrants working in the agricultural industry, about how to use their dental benefits. In 2016, Finney County Oral Health Coalition members began raising community awareness of the Coalition, its activities, and the importance of oral health by giving community presentations and distributing tooth-brushing kits and oral health literature at information booths. 37, County Health Ranking 63 rd of 105 9,136 18% No KanCare Alicia Dianda adianda@genesisfh.org Encouraging families enrolled in private and public dental insurance to use their dental benefits. 8 2016 KANSAS Reinventing Oral Health Johnson County In 2015, community members of Johnson County, the largest county in Kansas, formed an oral health coalition aimed at improving the oral health of the area s older adults. Johnson County Oral Health Coalition s first project was to collaborate with Meals on Wheels to deliver dental care kits to over one thousand seniors and educate them about the importance of healthy daily oral health habits. The Coalition also developed paper placements featuring oral health tips that were placed at congregate meal sites. JOHNSON COUNTY 580, County Health Ranking 1 st of , % No KanCare In partnership with University of Missouri-Kansas City School of Dentistry, Coalition members conducted a research project in 2016 in a HUD-subsidized housing complex that is home to 100 low-income seniors. The research included oral health assessments and surveys about barriers to dental care, oral health literacy, and daily oral health habits. Each senior who participated received an individualized home care plan, including tooth-healthy food and snack choices. Data from this research project is being analyzed by the principal investigators and will be available in In order to better understand the barriers seniors face in maintaining oral health, Johnson County Oral Health Coalition conducted a 2016 review of Kansas regulations for senior adult care facilities. Coalition members discovered that the state s regulations regarding access to dental services are not as stringent as many other states. Johnson County Oral Health Coalition plans to meet with state-level stakeholders in 2017 to discuss the findings and consider possible regulatory improvements. 9 2016 KANSAS Reinventing Oral Health Sedgwick County SEDGWICK COUNTY 511, County Health Ranking 69 th of , % No KanCare Stakeholders in Wichita formed Wichita-Sedgwick County Oral Health Coalition in Since then, the Coalition has been active in convening stakeholders to analyze dental needs in the state s largest city and search for solutions to address un-met needs. The Coalition s major project over the past decade has been to host an annual Give Kids A Smile event each year serving approximately 300 local children who have received free dental services. The Wichita-Sedgwick county Oral Health Coalition has been successful in attracting federal grants to research the oral health outcomes of children who take part in Give Kids A Smile and it plans to once again host an event in One of the Coalition s leaders is currently conducting a federal research project on how good dental care for long-term care residents can prevent hospital readmissions. Specific medical conditions, such as diabetes and respiratory disease, are being studied through this project. The Midwest Collaborative Initiative helps connect this long-standing coalition with other oral health coalitions across Kansas and with the statewide efforts to improve oral health for underserved populations. 10 2016 KANSAS Reinventing Oral Health Shawnee County SHAWNEE COUNTY 178, County Health Ranking 51 st of , % Yes KanCare Shawnee County Oral Health Coalition was established in The following year, it successfully applied for grants from United Way of Greater Topeka and the American Dental Association s Samuel D. Harris Fund to launch a Brush, Book, Bed, a program developed by the American Academy of Pediatrics to help families establish bedtime routines that include reading and brushing teeth. Since the fall of 2016, the program has provided dental-related books, selected from the Maryland Dental Action Coalition s recommended book list to families of approximately 200 children aged birth to three years. Families will also receive copies of popular non-dental children s books. In 2016, Shawnee County Oral Health Coalition also convened 15 community organizations to co-host a series of legislative candidate forums. The Greater Topeka Chamber of Commerce and Topeka- Capitol Journal were among the partners that hosted the forums, which were well-attended by candidates and members of the public. The Mission of the Michigan Oral Health Coalition (MOHC) is to improve oral health in Michigan by focusing on prevention, health promotion, oral health data, access and the link between oral health and overall health. The MOHC s overarching goal is to increase access to care and to improve the oral health of Michigan s adult residents and its children. Some of the strategies implemented are derived from the statewide oral health plan that was developed in partnership with the state of Michigan and Community Stakeholders with support from the DentaQuest Foundation through its Oral Health 2020 initiative. The MOHC has focused on the developing and sustaining community networks that will align resources and data to devise approaches that will prevent oral disease, while also developing models that are replicable and can be used throughout the state of Michigan to improve overall oral health. This year the MOHC has focused on training and development of five local coalition s throughout the state of Michigan. The Holland Free Clinic, Kent County Oral Health Coalition, Saginaw Health Plan, Tri-County Dental Health and the Macomb County Oral Health Coalition, are building upon the foundation laid in 2015 to become the catalyst for social change in their communities. They are also charged with the task of devising the most effective approaches to meet the unique health needs for the communities that they serve. The following pages highlight the achievements and plans for the coming year. Community Grantee Profiles 11

12 Holland Free Health Clinic 2016 MICHIGAN Reinventing Oral Health OTTAWA COUNTY ALLEGAN COUNTY OTTAWA: 279,955 ALLEGAN: 114, County Health Ranking OTTAWA: 1 st of 83 ALLEGAN: 12 th of 83 OTTAWA: 22,921 ALLEGAN: 14,993 OTTAWA: 11% ALLEGAN: 13% OTTAWA: No ALLEGAN: Yes OTTAWA: Healthy Kids Dental Fee-for-Service, Adult ALLEGAN: Healthy Kids Dental Fee-for Service, Adult Holland Free Health Clinic (HFHC) is a localized, volunteer-driven initiative which engages health providers and other agencies in a collaborative effort to address unmet health needs. HFHC acts as a local health safety net for the homeless, disabled, working poor and the unemployed. To be eligible, patients must have an income at or below 200% of the federal poverty level, are uninsured or have Medicaid and face barriers to care. Through service, education, and empowerment, HFHC s services help patients understand the importance of a healthy lifestyle and empower them to take control of their health. HFHC s services are in place to improve patients quality of life and the overall health of the community. The HFHC dental clinic is one of only two free programs in Ottawa/ Allegan Counties. Available services include preventive, restorative, oral surgery, and prosthodontic care. Treatment is provided by five dentists, five hygienists, and six dental assistants who volunteer their time at the on-site dental clinic. Patients may also receive care from one of the twenty local dentists participating in the HFHC referral network. Funding from the Michigan Oral Health Coalition s 2016 Community Mini-Grant Program has enabled the Holland Free Health Clinic to form the Lakeshore Dental Access Coalition. LDAC is a network of community partners and dental professionals who strive to pursue access to oral health care for residents of Allegan and Ottawa Counties. Tara L. 13 2016 MICHIGAN Reinventing Oral Health Kent County Oral Health Coalition 636,639 KENT COUNTY 2016 County Health Ranking 16 th of 83 97,074 12% Yes Healthy Kids Dental Fee-for-Service, Adult Throughout 2015, the Kent County Oral Health Coalition (KCOHC) task forces completed their work in five areas: Perinatal/ Early Childhood Health, Interprofessional Engagement with Primary Care Providers, Broad Community Messaging, provided Recommendations for the Dental Residency program in Kent County, and Senior Oral Health. The task forces goals were set by the coalition in the Community Plan, additionally a steering committee was formed and advised the 2016 strategic planning phase. KCOHC was selected by the DentaQuest Foundation as one of the Oral Health 2020 Grassroots Engagement Initiative grantees. The coalition has worked with Health Net of West Michigan and the Kent County Health Department on a recently completed Oral Health Landscape Assessment, which includes qualitative and quantitative grassroots data on the oral health of our community. The information gathered and analyzed in 2016 will serve to update the Kent County Oral Health Exam; a community needs assessment that focuses exclusively on oral health access and the status of families living in Kent County. The support of the Michigan Oral Health Coalition (MOHC) has allowed our community the ability to align measures on a local level along with other communities across the state so that we impact bigger picture, longer term goals. The recent release of the State Oral Health Plan is an example of collaborative planning and collaborative leadership, investing in the process as individual communities and workgroups to develop a shared vision of oral health for all. KCOHC is working to establish the next set of goals and plans for oral health in our community, and we look forward to developing grassroots leadership and participation through Kaitlyn Deacon kdeacon@healthnetwm.org Assessing needs of oral health access. 14 2016 MICHIGAN Reinventing Oral Health Macomb County Health Department MACOMB COUNTY 860, County Health Ranking 58 th of , % Yes Healthy Kids Dental Fee-for-Service, Adult The Macomb County Health Department identified the need in 2014, to garner support for improving the overall oral health from community advocate groups, with the goal of improving oral health in Macomb County. The coalition s desire was to have a collective voice to achieve their goals which became a reality in 2015; with the formation of the Macomb County Oral Health Coalition. The Coalition s membership is representative of more than a dozen partner agencies that work collaboratively to ensure that all persons residing in Macomb County have access to quality, affordable oral health services integrated into their overall health. The Macomb County Oral Health Coalition partnered with the Michigan Dental Association/Michigan Dental Association Foundation and hosted the 2016 Mission of Mercy. The event was held in Southeastern Michigan on the Campus of Macomb Community College in Warren, Michigan. Coalition members assisted with the coordination of this two day Oral Health event which treated approximately 1,200 residents of Southeastern Michigan at no cost. The coalition has set forth several specific priorities including that of promoting oral health education in the community with special emphasis on early childhood environments. Addressing the health literacy in Macomb County is another area of focus as the coalition develops prevention messages targeting the community. Additionally, this unique collaboration has also promoted expanded access to Healthy Kids Dental in Macomb County to residents as well as dental providers. The Macomb County Coalitions alliance with a dozen local partners as well as the Oral Health Network through the Michigan Oral Health Coalition has facilitated access to ongoing training necessary to move forward their advocacy goals; and to increase access to dental care by expanding service delivery sites. The collaborative is also addressing the inadequate number of oral health service providers for the counties special populations; children with special health needs, the homeless population, mental health and also communities of color. 15 Saginaw Health Plan 2016 MICHIGAN Reinventing Oral Health Saginaw Health Plan (SHP) is a community supported non-profit program designed to provide health care coverage to the uninsured residents of Saginaw County. It has formed a growing network of local health care providers and public and private entities working collectively to address local unmet oral health needs and to help ensure that residents have access to care. SAGINAW COUNTY 196, County Health Ranking 76 th of 83 42,156 12% Yes Healthy Kids Dental Fee-for-Service, Adult In the past, SHP has provided over $100,000 to help members pay for emergency oral health treatment. In 2015, the Health Plan added a dental benefit through Delta Dental for its Plan B members. Services include oral exams and cleanings twice per calendar year, as well as annual bitewing X-rays. Emergency palliative treatment, brush biopsy, minor restorative services, oral surgery services and periodontal maintenance are also covered benefits under the plan. Additionally, as an incentive for individuals to purchase insurance on the ACA Marketplace, the Health Plan will provide the same dental benefit for individuals with income up to 250% of the Federal Poverty Level (FPL) who have proof of premium payment and do not have dental coverage provided by their Marketplace plan. The Michigan Oral Health Coalition grant funding is being used to refine and improve Saginaw County s oral health system of care for residents, including low-income uninsured and underinsured. A Dental Resource Guide is being developed for Saginaw, Bay and Midland Counties (Great Lakes Bay Region) as residents frequently cross county borders to receive care. The Guide is organized by County and identifies providers for Adults (Seniors, Veterans and Special Needs) and Children (Special Needs). The Guide indicates whether the dental provider accepts Medicaid, Healthy Michigan Plan, MiChild, Healthy Kids, commercial insurance, payment plans and/or provides discounts. The Guide includes information regarding how to apply for healthcare coverage, defines Medicaid coverage, outlines dental coverage available for veterans, summarizes resources available to help pay for dental services and includes tips on how to care for one s teeth from infancy through adulthood. Through use of this Guide, individuals will become more knowledgeable and have access to a variety of available dental resources and providers. 16 Tri-County Dental Health 2016 MICHIGAN Reinventing Oral Health Tri-County Dental Health, a program of Jewish Vocational Service (JVS), is committed to improving access to oral health care services. It connects Macomb, Oakland and Wayne County residents who are low-income, uninsured, have disabilities, and/or rely on Medicaid, with dental care providers. WAYNE COUNTY 1,088, County Health Ranking 83 rd of ,714 16% Yes Healthy Kids Dental Fee-for-Service, Adult Tri-County Dental Health formed the Oral Health Coalition for Adults with Disabilities (OHCAD) with partners from the dental and mental health communities. Its mission is to increase access to appropriate and compassionate dental care for adults with disabilities. Their current focus is on people with intellectual and developmental disabilities. The coalition intends to create systems change through greater understanding of the needs of the population and how to work with the challenges they present. The OHCAD continues its strategic planning process with the development of an action plan to address identified areas of need. This year s priority is education and advocacy. Coalition members plan to open a dialog with members of the Michigan legislature and the Lt. Governor to focus more attention on the need for training dental students to address the oral health care needs of adults with disabilities. They also will develop continuing education classes for dental professionals focusing on the needs of people with disabilities. Additionally, there are plans to partner with local dental hygiene schools and Macomb Oakland Regional Center (MORC) to design caregiver and provider training, specifically addressing the various requirements of patients with special needs. Members represent dental professional organizations, community mental health agencies and dental educational programs. The state has a robust school-based program, effective partnerships, and an active state oral health coalition, Wisconsin Oral Health Coalition (WOHC). WOHC is comprised of more than 170 individuals, organizations, and agencies committed to overcoming oral health access issues and working to improve oral health for all Wisconsin residents. In 2013, WOHC brought together a group of key stakeholders, with additional input from other representatives of the public health and dental community, to develop the state oral health plan, Wisconsin s Roadmap to Improving Oral Health (Roadmap). Local and state coalitions utilize the Roadmap s strategic areas and goals to guide their day-to-day work. While Wisconsin has made significant progress in improving the oral health of Wisconsin residents, oral disease throughout the life course continues to be a key health concern for the state: In Wisconsin s Healthy Smiles/Healthy Growth Survey, 53 percent of Wisconsin third-graders had caries experience. 1 In the Basic Screening Survey (BSS), 13 percent of Wisconsin adults had complete tooth loss, 15 percent had untreated tooth decay, and 16 percent needed dental care. 2 In Wisconsin s Healthy Smiles Survey, 42 percent of nursing home residents had untreated decay. 3 In spite of these concerns, Wisconsin is also making significant strides in increasing access: Wisconsin adults continue to have extensive dental benefits through Medicaid. In response, Church Health Services, in collaboration with Care 4 U Primary Care Clinic and the Green Lake, Columbia and Marquette County Public Health Department, created a collaborative initiative, Amish Children s Dental Coalition. Amish Children s Dental Coalition was selected as a Midwest Collaborative Initiative community mini grant recipient in The Coalition is working on developing an Amish Community Dental Screening Program at each of the 15 Amish community schools. Once the screening is complete, a licensed dentist will develop a treatment plan and refer children in need of additional oral care to Church Health Services dental clinic in Beaver Dam. In 2016, Amish Children s Dental Coalition increased its proactive efforts focused on oral health promotion and prevention. It provided outreach activities to the Amish community to increase oral health awareness, including featuring an article on the benefits of fluoride in a monthly Amish newsletter. In addition, the Coalition increased its presence within the Amish community by attending and presenting at the Amish health fair attended by more than 200 Amish community members, discussing the importance of oral health care, fluoride, regular dental visits, and early prevention. The Coalition hopes to expand upon its proactive outreach in Stacy Nehmer stacyn@churchclinic.org Healthy equity by focusing on oral health services to Amish children. Community Grantee Profiles 18

19 2016 WISCONSIN Reinventing Oral Health Door County Oral Health Coalition In 2011, the Door County Public Health Department held a Community Health Improvement Process (CHIPS) meeting where participants were asked to rank 12 health focus groups. 20 2016 WISCONSIN Reinventing Oral Health Eau Claire Healthy Communities Oral Health Promotion Action Team Eau Claire Healthy Communities is a local coalition comprised of community partners working together to promote the health and well-being of Eau Claire County residents. The coalition was formed in 1996 to address findings of the local community health assessment. The Oral Health Promotion Action Team is part of Eau Claire Healthy Communities and an outgrowth of the Health Access Subcommittee of Healthy Communities. EAU CLAIRE COUNTY 101, County Health Ranking 20 th of 72 25,073 10% Yes BadgerCare Plus Eau Claire Healthy Communities Oral Health Action Team was selected as a Midwest Collaborative Initiative community mini grant recipient in Over the past two years, Eau Claire Healthy Communities addressed the importance of the age one dental visit. In 2016, the coalition recorded a webinar, as part of the coalition s Dental Home by Age 1 Campaign, to educate medical providers and community members on the importance of the age one dental visit. As of August 2016, more than 90 individuals from various organizations have viewed this webinar. In addition, the coalition partnered with local pediatric dentists in developing a news article/ media interview on the age one dental visit, which was aired on the local news station, WEAU 13, on January 17, In 2017, Eau Claire Healthy Communities hopes to expand its focus to perinatal oral health and continue to increase awareness on the importance of infant oral health. 21 2016 WISCONSIN Reinventing Oral Health Lincoln County Oral Health Coalition Oral health in Lincoln County became a priority health focus through a community health needs assessment process that started in January Community partners were invited to review Lincoln LINCOLN COUNTY 28, County Health Ranking 51 st of % Yes BadgerCare Plus County data related to Healthiest Wisconsin Upon completion of the data review, members recommended oral health as one of the top three health priorities for the county. The Lincoln County Oral Health Coalition Action Plan was initiated in fall of 2012 to mobilize stakeholders to collaboratively address oral health issues that impact Lincoln County residents. Lincoln County Oral Health Coalition was selected as a Midwest Collaborative Initiative community mini grant recipient in Its goal was to improve public perception of the importance of oral health by developing a dental home campaign targeted at the adult population. The objective is to reduce the 55 percent of adults who, according to the County Health Rankings, have not had a recent dental visit. Towards this end, Lincoln County Oral Health Coalition attended a series of social marketing trainings in June and July It also secured additional funding to develop a billboard and educational materials. Most recently, the Coalition contracted with a local graphic designer and began developing materials. In 2017, Lincoln County Oral Coalition hopes to complete its dental home campaign and begin monitoring utilization of oral health services attributed to increased oral health awareness. To date, the Coalition has convened key stakeholders at an oral health luncheon, followed by a general health coalition meeting, to discuss the opioid epidemic. In 2017, Northern Wisconsin Health Network plans to host two oral health coalition dinner events with primary care/ addiction treatment providers, mental health providers, and dental providers to determine and facilitate best practices for collaboration, referrals, and information dissemination. This proved to be a successful partnership and prompted key stakeholders to form a local coalition. In 2016, Coalition members focused on increasing participation in Wisconsin Seal-A-Smile, a statewide school-based sealant program, created protocols to follow up on urgent health needs for Seal-A-Smile participants, and reviewed fluoride varnish policies. The Coalition also expanded its incentives program for students to increase the return rate of treatment consent forms. In 2017, the Oconto-Marinette Oral Health Coalition hopes to partner with Head Start to address program participants oral health needs. 24 2016 WISCONSIN Reinventing Oral Health Oral Health Coalition Of Dane County Oral Health Coalition of Dane County (OHCDC) was formed in 2008 and currently has a membership of 25 organizations, plus three unaffiliated individuals. There are four subcommittees: Education, Advocacy, Data, and Membership. OHCDC and assisting organizations have been instrumental in collecting data and raising awareness of oral health status and access issues in Dane County. 516,284 DANE COUNTY 2016 County Health Ranking 11 th of 72 64,174 9% No BadgerCare Plus OHCDC was selected as a Midwest Collaborative Initiative community mini grant recipient in Coalition members are looking to discuss and revise OHCDC s infrastructure and organizational process. In 2016, OHCDC hired an external facilitator to guide its strategic planning and made significant inroads into its three identified objectives. The Coalition developed a media and communication plan related to the Max Pohle Clinic closure, implemented and promoted an emergency department (ED) and urgent care center (UCC) non-traumatic dental pain protocol, implemented and promoted a Community Dental Care Coordinator (CDCC) for Dane County, and established a system for the ongoing collection of program outcome data, including number of calls and referrals. In 2017, OHCDC hopes to refine and monitor the impact of the ED/UCC diversion protocol and care coordination system. 25 2016 WISCONSIN Reinventing Oral Health St. Croix County Oral Health Coalition St. Croix County Oral Health Coalition began in 2010 as part of St. Croix County s Community Health Improvement Plan. The group consists of concerned citizens and local health professionals interested in improving dental access to the county. In 2016, St. Croix County Oral Health Coalition became a Midwest Collaborative Initiative community mini grant recipient. ST. CROIX COUNTY 86, County Health Ranking 3 rd of 72 9,036 7% No BadgerCare Plus In 2016, the Coalition partnered with schools and area hospitals to support a backpack program, which provides food and health related items for school-aged children from underprivileged homes. Specifically, the Coalition contributed oral health education newsletters, dental supplies and water bottles, encouraging residents to drink fluoridated tap water. In addition, the Coalition presented project updates to Healthier Together Pierce and St. Croix County Coalition, as a way to raise awareness on the importance of oral health. 26 2016 WISCONSIN Reinventing Oral Health Waukesha County Dental Coalition WAUKESHA COUNTY 395, County Health Ranking 6 th of 72 31,157 6% No BadgerCare Plus Waukesha County Dental Coalition (WCDC) is a prime example of how community members across all sectors work together toward common and measurable goals. WCDC is interested in improving oral health and access to dental care in Waukesha County for uninsured and racial/ethnic minority populations. With the leadership and expertise of the dental community and dedicated Coalition members, the group established a dental clinic in May 2008: Waukesha County Community Dental Clinic (WCCDC). Unfortunately, after the dental clinic was established, the Coalition became inactive. In 2016, WCCDC decided to re-establish its coalition. Through the financial assistance of the Midwest Collaborative Initiative community mini grant, WCCDC brought together former partners to address the serious gap in access to affordable dental care for low-income adults, uninsured, and underinsured adults who live in Waukesha County. In 2017, the reconstituted coalition hopes to evaluate the need for an expansion of dental care in Waukesha County and identify a viable delivery model to meet the needs of this population.
