2 Donate a Smile A Campaign to Secure Health Programs for the Underserved Case Statement Holland Free Health Clinic (HFHC) provides a local safety net for those with medical needs and no place to turn for help. Founded in 2002, HFHC has assisted the local homeless, disabled, working poor, and unemployed with dental, optical, counseling, hearing, medication, diabetic support and spiritual care. Since the clinic s inception, it has provided over $14,000,000 in life changing services. The impact to each patient is immeasurable. Through Christ s mission, we are able to serve our patients with compassion and provide hope for the future. Through program and service expansion, Holland Free Health Clinic has made tremendous impact on improving access to healthcare, but the growth has not come without costs. In order to continue this work HFHC has launched this critical campaign, Donate a Smile, to help secure services for our patients now and in the future. Your participation of time, talent and financial support will ensure the mission to express the healing ministry of Jesus by facilitating access to health care for those in need continues. We need and appreciate your investment to provide hope, health and bring smiles to our patients. Campaign Objectives Promote awareness of the HFHC mission, services and impact on the community Raise financial support from current and prospective donors. This critical effort will ensure HFHC continues to be an integral part of the local health care system, filling gaps in basic services for people who would otherwise go without. We have initiated this campaign to secure the future of HFHC services for those in need. 5 We need YOU! HFHC is a localized, volunteer-driven organization which engages health providers, other agencies and ministry partners in a collaborative effort to address unmet health related needs. We invite you to consider your personal involvement of time and expertise as a volunteer. Programs and services are strategically planned to correlate with the identified healthcare gaps specific to the greater Ottawa and Allegan counties. Our care goes beyond treatment and places great emphasis on prevention and education. Programs are designed to help each patient understand the importance of a healthy lifestyle and empower them to take control of their health. Our work is vital and the need has never been greater. HFHC has established strong collaborative relationships with local agencies, health providers and learning institutions. One notable Community Partner is Hope College. HFHC provides student volunteers and interns firsthand experience working with the underserved and improving access to quality health services. During the 2016 year, the 40 students providing more than 3,000 hours of time were instrumental in strengthening our volunteer base and allowing us to expand our programs. Guided by Christ s mission, we serve our patients with compassion and provide hope for the future. To continue this work, we need your participation in the Donate a Smile campaign. Your contribution of time, talent and financial support ensures the continuation of our mission to express the healing ministry of Jesus by facilitating access to health care for those in need. 6 Get Involved at Donate your TIME Volunteers washing feet at the Pamper & Prayer foot clinic, greeting visitors at our front desk, those serving on planning committees and other roles are vital to the success of our programs and lives of our patients. HFHC services would not exist without the time and commitment of volunteers. Please consider joining our team today. Support volunteers donated over 4,640 hours in 2016 Donate your TALENT Local providers licensed in dentistry, eye care, podiatry, nursing, counseling and other specialties volunteer their talent providing HFHC services to those with no place else to turn. Others share their professional expertise in marketing, graphic design, accounting, strategic planning and fundraising. What skills do you have to offer the HFHC team?

HFHC has a history of success doing big things on a small budget. We provide impactful free health services with minimal operational costs. 8 Larry s Story For years I was homeless, struggled with periods of depression and I suffered from severe mouth pain. Many of my teeth were missing and I knew the others were rotten and needed to be pulled. The poor condition of my mouth made it hard to eat, I had limited food choices and was self-conscious to go out in public. One day at the Community Kitchen lunch, a HFHC volunteer told him about the dental and counseling services and signed him up for an appointment. He has been a patient for over a year, had his decayed teeth pulled, received a set of dentures and individualized support for his depression. Larry shared: Receiving dentures and counseling from HFHC changed my life. They helped restore my dignity, confidence and health. I never dreamed that I could access those services or that somebody cared about me when I was hitting rock bottom. The Holland Free Health Clinic has been a huge blessing to me in getting my life on track and giving me my smile back. Through them, I ve discovered my worth again.
