4 Overview REINVENTING ORAL HEALTH: A Midwest Collaborative Initiative When people align around shared political, social, economic or environmental values, and take collective action, thinking and behavior that compromises the lives of millions of people around the world can truly change. Simon Mainwaring OVERVIEW Through the DentaQuest Foundation Oral Health 2020 effort and the Midwest Collaborative Initiative, Oral Health Kansas, Michigan Oral Health Coalition and Wisconsin Oral Health Coalition have done just that as they endeavor to reinvent oral health. Through funding by the Foundation, the Michigan Oral Health Coalition strives to align national, state and local partners into a formal network. Success was achieved by developing a community network model that created local oral health coalitions throughout the state of Michigan that worked to improve the oral health of children and adults through new ideas, activism and leadership. In 2015 to further the DentaQuest Foundation s initial investment, a partnership was created between Michigan and the Kansas and Wisconsin state oral health coalitions to create a tri-state movement of innovation, advocacy and leadership to eradicate dental disease by This partnership formally connects the local stakeholders to the state and national stakeholders ultimately making them a network for change within each state, as well as the region. Kansas, Michigan and Wisconsin are ideal partners. As the collective voice of oral health in their respective states, each has a broad circle of influence among traditional and non-traditional stakeholders and has facilitated consensus-based support around State Oral Health Plans, advocacy and policy work. Combined, the three state oral health coalitions advocate for 18.6 million Americans in a politically conservative environment. Adding to this challenge, 116 counties are HRSA-designated dental health professional shortage areas among the three states. In 2016, nineteen local oral health coalitions tackled these difficulties to advocate for specific population groups with unique oral health issues in their communities. The connection they have to other coalitions within the tri-state network continues to strengthen their efforts. Each of the three state coalitions include a diverse leadership and membership which represent education, providers, government, community, third party payers, policy, higher/professional education and the general public who are eager to link together by sharing their ideas and expertise. One definition of oral health maintains that oral health contributes positively to physical, mental and social well-being and the enjoyment of life s possibilities, by allowing the individual to speak, eat and socialize unhindered by pain, discomfort or embarrassment. A core principle of the Initiative is that everyone should be able to attain a state of oral health that will enable them to enjoy a healthy and full life. On the following pages, we share our accomplishments that have been helping us move toward this principle and the current environment as it relates to the bold, shared goals to be achieved by Work continues in the state and regional network! The work we are doing to build local coalitions enables us to focus on the oral health needs of young children, seniors, recent immigrants, minorities, people living in poverty, and those with disabilities. The community oral health coalitions supported by the Midwest Collaborative Initiative address the Oral Health 2020 vision and Oral Health Kansas values. Each coalition designs goals and strategies that best reflect the needs of its community. As Oral Health Kansas works with each community s coalition, we are able to identify disparities along racial, ethnic, cultural, and socio-economic lines in accessing good oral health. The Mission of the Michigan Oral Health Coalition (MOHC) is to improve oral health in Michigan by focusing on prevention, health promotion, oral health data, access and the link between oral health and overall health. The MOHC s overarching goal is to increase access to care and to improve the oral health of Michigan s adult residents and its children. Some of the strategies implemented are derived from the statewide oral health plan that was developed in partnership with the state of Michigan and Community Stakeholders with support from the DentaQuest Foundation through its Oral Health 2020 initiative. The MOHC has focused on the developing and sustaining community networks that will align resources and data to devise approaches that will prevent oral disease, while also developing models that are replicable and can be used throughout the state of Michigan to improve overall oral health. This year the MOHC has focused on training and development of five local coalition s throughout the state of Michigan. The Holland Free Clinic, Kent County Oral Health Coalition, Saginaw Health Plan, Tri-County Dental Health and the Macomb County Oral Health Coalition, are building upon the foundation laid in 2015 to become the catalyst for social change in their communities. They are also charged with the task of devising the most effective approaches to meet the unique health needs for the communities that they serve. The state has a robust school-based program, effective partnerships, and an active state oral health coalition, Wisconsin Oral Health Coalition (WOHC). WOHC is comprised of more than 170 individuals, organizations, and agencies committed to overcoming oral health access issues and working to improve oral health for all Wisconsin residents. In 2013, WOHC brought together a group of key stakeholders, with additional input from other representatives of the public health and dental community, to develop the state oral health plan, Wisconsin s Roadmap to Improving Oral Health (Roadmap). Local and state coalitions utilize the Roadmap s strategic areas and goals to guide their day-to-day work. While Wisconsin has made significant progress in improving the oral health of Wisconsin residents, oral disease throughout the life course continues to be a key health concern for the state: In Wisconsin s Healthy Smiles/Healthy Growth Survey, 53 percent of Wisconsin third-graders had caries experience. 1 In the Basic Screening Survey (BSS), 13 percent of Wisconsin adults had complete tooth loss, 15 percent had untreated tooth decay, and 16 percent needed dental care. 2 In Wisconsin s Healthy Smiles Survey, 42 percent of nursing home residents had untreated decay. 3 In spite of these concerns, Wisconsin is also making significant strides in increasing access: Wisconsin adults continue to have extensive dental benefits through Medicaid. In early July 2015, Governor Scott Walker signed into law a dental Medicaid pilot program to test market-based rates in improving access to all dental care services provided to Medicaid-enrolled children and enrolled adults seeking emergency dental care in Brown, Marathon, Polk, and Racine counties Healthy Smiles/Healthy Growth Wisconsin s Third Grade Children The Oral Health of Wisconsin s Adults Wisconsin Healthy Smiles Survey The Oral Health of Wisconsin s Older Adults Community Grantee Profiles 17
