1 Diabetes Palestine Association Narrative Annual Report 2016 Diabetes Palestine Association is a not-for-profit association established and registered in Palestine in the year Association is located in and performs its functions in partnership with the Diabetes Care Center at the Augusta Victoria Hospital (AVH). Functionally, Association works in West Bank, Gaza and East. Diabetes Palestine serves as a member of the National Committee for Non-Communicable Diseases chaired by the Ministry of Health (MOH). Since it established, Diabetes Palestine (DP) has been working in close partnership with Diabetes Care Center at Augusta Victoria Hospital to improve diabetes care and to help in adopting best practices in diabetes management and prevention of complications. DP created a new and unique model of diabetes care and prevention in the community known as Diabetes Comprehensive Care Model (DCCM). The model has been successfully implemented in Diabetes Care Center at AVH since 2003 and has gradually disseminated to major providers in the Ministry of Health and United Nations Relief and Works Agency (UNRWA) clinics. The model empowers diabetes patients and encourages them to take an active role in the care process and lifestyle modifications. Recently, DP started to implement the model in Gaza in partnership with Union of Health Work Committees (UHWC). A three-year project supported by DanChurchAid (DCA) and the World Diabetes Foundation (WDF) aims to create a model clinic in Gaza to be a powerhouse for diabetes care model DCCM implementation and dissemination. Association provides its services by a variety of trained and specialized healthcare professionals, including Endocrinologists, Diabetologists, family physicians, nurses, nurse educators, community nurses, foot care nurses, dietitians, social workers, national/international consultants and community leaders.
2 2016 Activities include: 1. Diabetes management The Diabetes Palestine Association in partnership with Diabetes Care Center at Augusta Victoria Hospital had 2,336 registered diabetes patients managed and followed-up regularly by the team. During patient s visit, medical laboratory tests including HbA1c and MAU, and lipid profile performed individual, and group counseling, nutrition counseling, patient s feet and eyes screened and patients evaluated by a diabetologist. 2. National Diabetes Symposium: Diabetes Palestine Association conducted its 7th National Diabetes Symposium on 28th of May 2016 in Ramallah. The main theme of the Symposium was "Diabetes and Pregnancy." International consultants' team from the EASD led by Prof. Alberto de Leiva Spain also included Prof. Bartolomé Bonet -Spain, Prof. Angela Napoli-Italy, and foot care consultant Bent Nielsen Denmark. Palestinian Ministry of Health, UNRWA and the Palestinian Society of Obstetricians and Gynecologist in addition to local NGOs and Palestinian Universities participated, over 250 healthcare professionals attended the Symposium. Presentations of international and local protocols for the screening and the management of gestational Diabetes and pregnancy planning for diabetic mothers were discussed. 3. Community program: Mobile Diabetes Clinic visits rural and urban Palestinian communities around 60 communities were visited in total on a daily basis and worked in close cooperation and partnership with Ministry of Health. The main objectives of the mobile clinic visits are:
3 Raise awareness of the people and patients about diabetes; improve access to diabetes care; build the capacity and skills of the front-line local professionals through on-job training, provide care for diabetes patients and refer cases in need for advanced management to the Diabetes Care Center in and/or to local diabetes clinics and centers. 4. Awareness and Screening Campaigns: Many campaigns were conducted during the year 2016 in West Bank area and Gaza. Gaza campaigns were implemented through our Partners in the Health Work Committees. Campaigns run the following activities: Screening for high-risk groups, radio and TV messages, HbA1c measurement, foot and eye screening for diabetes patients, distribution of glucose measuring machines and glucostics, and preparation of healthy food. Awareness campaigns also involve arranging visits to patients and their relatives to Augusta Victoria Hospital in in a step to challenge all the measures of occupation to isolate the city and to restrict the entrance to.

Participation in Al-Bawasel camp in Qatar: Two children and one staff nurse participated in Al Bawasel camp in Qatar for one week, they met with their peers and shared experiences with many patients from MENA countries. 6. Mobile Diabetes Clinic participated in the Right to Movement Marathon which held in Bethlehem area in April The participation of the Mobile Clinic was essential to advocate for the right of health to Palestinian People and to raise awareness about diabetes. 7. International Trainers: Foot care specialists from Denmark visited our center in and the partner clinic in Gaza for two weeks to continue the training of our teams on foot care management and insole making. Jorn Nerup, our consultant from Denmark, had visited the Diabetes Care Center in during April to continue the training of our teams on clinical audit and clinical research. 8. Diabetes Eye Screening: Digital non-mydriatic fundus screening camera was introduced and used in the center and the mobile clinic to screen all diabetes patients managed by our teams. Grading done by our team in the community and patients in need for laser referred immediately to MOH clinic.

Renovation and opening of the Diabetes Care Center at Augusta Victoria Hospital Diabetes Palestine president Mr.Nabil Zghier funded the renovation of the Diabetes Care Center at Augusta Victoria Hospital. The renovation completed, and the opening ceremony was done early in June The center built in a modern way to accommodate all activities of the DCCM and to provide a patient-friendly environment. 10. Diabetes Palestine Association installed wireless Insulin pumps for the treatment of type 1 diabetes children as a way to motivate patients to adhere to diabetes management plan, follow-up and to improve diabetes control. The diabetes team follow-up the patients and provides all necessary supplies as needed. We will conduct a study of the results of these patients to assess the effectiveness. We will conduct a survey of the results of these patients evaluate the effectiveness of the diabetes control. 11. Diabetes Palestine provides insulin pens to patients with type 1 diabetes (170 patients) and fewer number to patients with type 2 diabetes (50 patients). Insulin pens are not supported by the government health insurance or by UNRWA.

Diabetes Palestine participated in the IDF- MENA regional council meetings that were held in Pakistan from August 2016; and in Sudan from December Diabetes Palestine participated in the Associations Village of the EASD last in Munich-Germany on September. 14. Diabetes Palestine in participation with Follow the Women Group Association from Denmark started the implementation of building a playground in Yatta area- South Hebron to improve health lifestyle for diabetes children and women.
