2 PROGRAM OVERVIEW The Medtronic-Bayer CGM Therapy Expert Program is designed to update healthcare professionals on the most current scientific and technical information regarding CGM pump therapy, blood glucose meters and other related products. The program is intended for healthcare professionals who have some experience with insulin pump therapy, but are interested in advancing their clinical and practical knowledge.

3 WHO SHOULD ATTEND? Healthcare professionals who: Have some experience with patients using insulin pumps Are interested in expanding their knowledge about patient selection and protocols for therapy optimization Want hands-on practice and experience programming a Medtronic insulin pump and Bayer Contour NextLink meter Would like to participate in a deep-dive session on calculating insulin pump starting doses, adjusting and fine-tuning pump settings, and utilizing CareLink reports for evaluating finger-stick data to improve glycemic control WHAT S LEARNED After attending this program, attendees will have an increased knowledge of: Studies and clinical evidence that demonstrate improved glycemic control for patients who use insulin pump therapy How to program and adjust pump settings, insert infusion sets, and deliver boluses Factors that contribute to successful management of insulin pump therapy and enhance patient outcomes Resources and support systems that are available for healthcare professionals and patients Methods and best practices for integrating insulin pump therapy into a medical practice Clinical relevance and importance of blood glucose meter accuracy for the pump patient

4 WHAT TO EXPECT The Medtronic-Bayer CGM Therapy Expert Program provides healthcare professionals with practical training on initiating and finetuning insulin pump therapy using a case-based approach to patient care. The keynote speaker will be a healthcare provider with expertise in managing patients on insulin pump therapy. Other faculty will include a team of Medtronic clinical experts who will lead discussions and facilitate practice exercises with hands-on training and case study presentations. The objectives and content of each program is adjusted and tailored based on the attending clinicians CVs and answers provided on the pre-program survey. Although programs may vary, an outline of a typical agenda can be seen on the next page. The day s agenda is ambitious; however, there is plenty of time for questions and discussion. 6 FREQUENTLY ASKED QUESTIONS How do I register for the CGM Therapy Expert Program? Your Medtronic Sales Representative will provide you with registration information. How will I know that I m registered and that all details have been taken care of? You will receive a program confirmation once your registration is complete. Are there any requirements for attendance? You will need to submit an up-to-date CV to attend the program. In addition, you may be asked to sign a Non-Disclosure Agreement (NDA). What does a typical travel schedule look like? Your Medtronic representative will provide you with details on making travel arrangements. An itinerary will be ed to you containing information about your hotel accommodations and travel. Most clinicians find it best to arrive at their hotel the evening before the program.

7 FREQUENTLY ASKED QUESTIONS continued Will meals be provided? The following meals are included as part of the Pump Therapy Expert Program: Breakfast: Served outside the meeting area 30 minutes prior to start Lunch: A modest lunch will be provided the day of the program Dinner: An optional company sponsored dinner may be held. Please speak with your Medtronic representative for details. Additional meals will be reimbursed according to the AdvaMed Code. (A copy of the Medtronic Clinician Expense Policy will be provided with your program materials.) Is there a cost to attend? No. Are CME credits available? This course does not offer Continuing Medical Education (CME) credits. Will I be compensated for my time? No. This is an educational program and there is no compensation. However, all travel and meal expenses incurred in relation to this program will be covered. How should I dress for the program? Dress code for the day is business casual. Please dress comfortably as you will be sitting most of the day. We suggest that you bring a light sweater or wrap as hotel conference rooms can be cool. Ladies, consider wearing a two piece outfit (such as a top and pants or skirt) this allows for easier access to the abdomen when it is time to insert the infusion set.
