EY.16 Welsh language support for families in Cardiff Project to develop stimulating Welsh-medium activities (e.g. Miri Meithrin and leisure activities) and raise awareness Menter Caerdydd All Families First services to meet Welsh Language Standards but this particular service will no longer receive Families First funding. Work is also undertaken with those young people not attending school to assist with re entering the school system Housing advice and assessment for year olds along with family mediation and welfare rights advice. Mediation Service will also work with 14 / 15 year olds and their families. Project works in close partnership with Children s Services 14+ team and CCC s youth Housing Gateway Working restoratively with families to address areas of conflict within or outside the home. Working with year olds who are not in education, employment or training. Aimed at the most disengaged young people who are far away from reach their potential. Provide an integrated response to vulnerable families where there are children between the ages of 9 and 14 years with unmet needs, through a flexible, bespoke family focused programme Support for young people and parents. Delivery of evidence based parenting programmes in the West of Cardiff Youth Service Prince s Trust Prince s Trust Service to continue under Prince s Trust Service to continue under Prince s Trust Partnership (Fast Track) (CYE12), referrals will cease 01/06/18 - alternative support for families to continue via Early Help Family Support Workers, mobilising from September 2018.

5 C&YE.14 Support service for young families Supporting young parent s transition into parenthood across Cardiff on an individual and group basis. Specialist intervention, advice and support including therapeutic parenting programmes and issued based courses HEALTHY LIFESTYLES PACKAGE HL.1 FitFun A 10 week programme for families wanting to become healthier together. It is aimed at children above a healthy weight aged 8-11 years. Board (Dietetics team) Referrals ceased. Cardiff & Vale UHB dietetics support for the Families First programme will continue but this particular project will end. HL.2 Healthy Lifestyles project for 8-13 year olds A play, physical activity and food project - after school as a 10-week course and as a play scheme in the school holidays. Play and Leisure Service Cardiff & Vale UHB dietetics support for the Families First programme will continue but this particular project will end. HL.3 Summer Play and respite scheme A two-week play and respite scheme for children and young people with multiple and profound physical and learning disabilities. Ty Gwyn Special School Provision to be included in pilot of Informal Respite arrangements with the Disability Futures programme. HL.4 Healthy Lifestyles project for year olds Weight-management project for young people aged 13-25, including Get Cooking courses, accredited nutrition training for professionals, support for parents and carers. Play and Leisure Service Referrals ceased. Cardiff & Vale UHB dietetics support for the Families First programme will continue but this particular project will end. HL.5 Sexual Health Outreach and C-Card scheme Sex and relationships education and sexual advice/support for vulnerable young people, including the delivery of a condom distribution scheme (including training for professionals to be part of the C-card scheme). SRE session participation workshops to young people attending High Schools in Cardiff around issues of CSE and risk taking behaviours. Cardiff YMCA Provision will continue via new Healthy Relationships Service HL.8 Strengthening Families Programme Highly structured evidence-based course (accompanied by DVD and manual) delivered over 7 weeks for parents and children (aged 10-14) to attend together the course helps Children s Services Delivery to be incorporated into new Parenting Service still accepting referrals

6 families to build healthy relationships and address problem behaviours CADARN PACKAGE EMH.1 Transitions A one stop shop for young people aged and their families to provide support in managing the transition from Child and Adolescent Mental Health Services [CAMHS] to adult services. Referrals ceased support for young people in transition to be part of core delivery. EMH.2 Bounce Back Project works with schools and community settings to provide a structured, time-limited psycho social and emotional intervention for year 10 and 11 pupils at risk of becoming NEET. Referrals ceased support for young people s emotional wellbeing will continue through Family Wellbeing and Youth Support Services. EMH.3 Asylum seeker and refugee service EMH.4 2Talk2 EMH.5 Gofal i Chi EMH.6 School exclusion family support project EMH.7 Bereavement Service EMH.8 Next Steps Family intervention for families that have asylum seeker or refugee status where emotional and mental health issues are having an impact on family life. 1:1 counselling service for young people who are out of school and accessing Careers Wales programmes to support them into education, employment or training. Individual and group support for young carers (aged 5-10 and 11-18) to enable them to better understand and manage their caring role in both practical and emotional terms. Supports families emotional wellbeing and promotes engagement with the local authority, schools and relevant agencies. Therapeutic service for families who have experienced a significant impact on their wellbeing as a result of bereavement. Family-led intervention to enhance the resilience of families leaving the Integrated Family Support Service or with substance misuse issues. Referrals ceased but support will continue through new Family Wellbeing Service to be mobilised from 1 st April. 7 EMH.9 Families Learning Together EMH.10 Emotional and mental health and wellbeing for families accessing integrated children s centre Enhances families achievement and engagement with education and learning through a family course and support into the social and emotional aspects of learning. Support for the emotional and mental health and wellbeing of families accessing day care at the integrated children s centre all families are screened and additional interventions provided for those who need it. (Teulu service) Ely and Caerau Children s Centre Referrals will cease at the start of June - support will continue through Parenting and other FF services N/A EMH.11 Transition support Primary to secondary school transition intervention for vulnerable young people and their families. May 2018 to allow time to work with pupils before 24th July. New Right2 arrangements for supporting young people in schools via Youth Support Service. EMH.12 I Can Do It! Programme promotes resilience and builds positive relationships between a young person, peers and their family targeted on those pupils who are experiencing anxiety and difficulty in their relationships. Referrals ceased. New Right2 arrangements for supporting young people in schools via Youth Support Service. EMH.13 Pyramid Clubs Clubs improve learning and wellbeing of quiet, withdrawn and less confident children aged 7-14, as well as reducing the likelihood of developing poor mental health. March New Right2 arrangements for supporting young people in schools via Youth Support Service. EMH.14 Cardiff Against Bullying Preventative, reparative and reactive antibullying programmes (part of implementation of Cardiff s anti-bullying strategy). July Referrals for advice resources can be accepted up until the end of the project. Referrals for 1:1 May 2018 to allow time to work with pupils. New Right2 arrangements for supporting young people in schools via Youth Support Service. DISABILITY FOCUS PACKAGE DF.1 Disability TAF Key Working Provides a key worker to work alongside the family and with other practitioners to identify and remove barriers and broker introductions to new services.

8 DF.2b Hope 2 Shine: Support for BME families DF.2b 1:1 support for emerging disabilities A peer support matching service providing 1:1 and group work for families with a disabled child from BME communities 1:1 parenting support for parents where the child has emerging disabilities DF.3 Parenting and independent living skills DF.4 Inclusive Youth Provision Support for disabled young people and their families via specialist parenting programmes, drop-in sessions, key worker support, mentoring and advocacy. Transition and thematic workshops along with daytime and weekend service placements and social activities. Cathays Inclusive Youth Provision DF.5 Welfare rights and advocacy An advocacy service which promotes welfare benefits, direct payments and awarenessraising. Diverse Cymru Referrals ceased for work over 2 months to complete. Short term cases i.e. first stage work, will be accepted up to 16 th March Welfare Benefits support to continue as part of advice services offered via Hubs. DF.6 Information and advice Provides specialist information and advice for families and signposts them to relevant services. Parents Federation Referrals ceased. Information and advice for families to be provided via Disability Index and Support4Families. DF.7 Support for transition through education Provides support for transition through education within schools aged 6-14 and then after school age (year 10+). SNAP Cymru No new referrals directly from Disability TAF after Wednesday 28 th February Key working role will continue within Disability TAF. Local Authority SNAP service to continue.
