Avenidas Rose Kleiner Center will provide lowincome, vulnerable seniors with person-centered, "hands-on" care combined with intensive care coordination to allow them to remain in their homes and connected to their communities. BAWSI will engage students in fun, high-energy physical activities alongside female athlete coaches to serve as role models for young girls in underserved schools. The Centre for Living with Dying program will provide emotional support to adults and children facing life-threatening illness, the trauma of losing a loved one, or the traumatic experience of a critical and/or unexpected situation. Campbell Union School District will expand the Harvest of the Month program, which provides a tasting kit of fresh California grown produce to each elementary school classroom. CDM will integrate a new addition of the Food Shed cafe and food-related educational programs by offering fresh and healthy foods to children and families, showcasing healthy alternatives to food and beverages, and connecting the foods to existing exhibits. CAFF will work with a consortium of school food service directors committed to procuring and promoting locally-grown fruits and vegetables to students. The agency will also develop wellness policies to be adopted by each partnering school district. CHAC will teach social skills and emotional intelligence trainings to third graders and provides psychoeducational groups in low-income schools to increase emotional resilience and reduce the likelihood of engaging in high-risk behaviors. Community Health Partnership will expand on successful health improvement initiatives by building and sustaining the common elements of a high functioning Patient Centered Medical Home. The Safe School Ambassadors (SSA) program creates caring school climates and reduces bullying by engaging and empowering student bystanders to stand up and speak up when they witness bullying, harassment and cyber bullying. Discovery Counseling Center will provide a twotiered intervention program for students and their parents to prepare them with the social and emotional skills necessary for healthy relationships, drug resistance, and harm-reduction. EDRC will provide group and individual support to individuals on the path to recovery from eating disorders, promote awareness about eating disorders, and connect individuals and families to resources. The Teen Families and Schools Together (FAST) Program is a collaborative prevention and parent involvement program designed to build and strengthen relationships within families, schools, and communities.

Got Choices will reduce risky, aggressive and delinquent behaviors, increase protective factors and support positive, informed decision making among teen and tween girls. The Seeking Safety program will provide criticallyneeded mental health assessment and treatment for homeless individuals at IVSN s homeless shelters. MayView Community Health Center will support and strengthen care coordination by training staff members as a health coaches and care coordinators to work directly with patients. PARTI will implement the High Impact Program - a program aimed to promote leadership and life skills, service learning projects, and college and career training for youth in San Jose. This parenting skills and advocacy program is designed to help parents promote and support their children s healthy development. RotaCare Bay Area, Inc. Enhance Access to Health Care to Vulnerable Populations in the South Bay with Free Health Care RotaCare will provide free clinic services to underserved individuals, integrate medical review/quality management, and promote staff leadership collaboration and training. San Jose Downtown Foundation Santa Clara County Public Health Department Almaden Boulevard Loop Exercise (ABLE) Project Healthy Teen Relationships Campaign SJDA will create the Almaden Boulevard Loop Exercise (ABLE) project through its StreetLife Plan to promote free unlimited access to functional, easyto-use, beautifully designed fitness equipment for people of all ages and abilities. The SCC Public Health Department will continue to implement a comprehensive teen dating violence prevention campaign through youth mobilization, social marketing strategies, and policy development. School Health Clinics Patient Navigator Second Harvest Food Bank of Santa Clara and San Mateo Counties Expanding CalFresh Access to Low-income People School Health Clinics will engage patient navigators to reduce barriers that impede patients from obtaining both primary and specialty medical care and community resources. Second Harvest Food Bank will identify low-income individuals and families who are currently not receiving benefits and implement strategies to increase enrollment in Santa Clara County. Sunday Friends Sunnyvale Community Services THINK Together Vision To Learn Cultivating Prevention in Very Low- Income Families Nutrition Access and Advocacy Healthy Living Program Vision To Learn: Santa Clara County Expanded Operation Sunday Friends will engage very low-income children and their parents in building developmental assets that lead to productive, violence-free futures. SCS will increase access to healthy foods, and expand education and advocacy for nutrition and health for low-income families and seniors in Sunnyvale. THINK Together will provide opportunities for underserved students to develop healthy eating habits and increase physical activity. Vision To Learn will provide free eye exams and glasses via their mobile optometric clinic to students in San Jose.

3 YMCA/Project Cornerstone 30 Total Grantees Creating School and Community Coalitions to Reduce Project Cornerstone will reduce violence by empowering all members of the school community to reduce bullying and violence, promote achievement, increase understanding of developmental assets, and help all students feel valued and respected.
