3 How you have engaged with us: HEALTHWATCH As a statutory partner and critical friend of the CCG, Healthwatch is represented on the CCG governing body and seek strategic commissioning responses to the findings of their Enter and View exercises and public surveys HEALTH FORUMS Local service providers, patient groups and other community stakeholders met three times a year in nine geographical patches of the county to exchange information and discuss local health issues PATIENT PARTICIPATION GROUPS (PPGs) The chairs of Somerset s 72 PPGs met the CCG every quarter to scrutinise service provision and commissioning, cascade information back to patients in their own practices and share good practice WEEKLY PUBLIC ENGAGEMENT BULLETIN Over 300 community stakeholders, both individuals and organisations, were updated regularly on engagement opportunities and new developments PATIENT EXPERIENCES Learning points from individual patient and carer stories have been used by clinical commissioners, committees and programme boards to identify areas for improvement and to measure the quality of commissioned services DEVELOPMENT DAY Over 40 participants, supported by existing volunteers, PPG chairs and lay users, attended a development day in October 2016 to find out more about the different ways in which they could get involved LAY MEMBERS Patient scrutiny is embedded in CCG governance. A nonexecutive director and PPG representative bring public scutiny to the CCG governing body. Lay users also sit on the CCG s Clinical Operational Group and Joint Committee SOMERSET ENGAGEMENT ADVISORY GROUP (SEAG) Voluntary & community organisations, Healthwatch, patient and carer groups and service providers scrutinise, challenge and support our commissioning activities, and have taken on a formal role as part of our STP

5 What difference did your feedback make? What you told us: People are worried about what the Somerset Sustainability and Transformation Plan (STP) means for future services People told us that the Continuing Healthcare processes were complicated and confusing Healthwatch made recommendations for improvements on mental health wards, based on patient feedback Patients and carers posted comments online via NHS Choices about their good and bad experiences of services Patients, carers and voluntary organisations raised concerns about prescribing policy changes Young people with diabetes told us their experiences of transitioning to adult services Healthwatch fed back the results of their GP practice survey Stroke survivors challenged commissioners on future funding of rehabilitation services Individual patients and carers volunteered to share their experiences of health services What we did: We briefed patient groups and voluntary organisations and placed two patient representatives on the STP Models of Care Reference Group The CHC team worked with a delegation of the Somerset Engagement Advisory Group to update their public-facing materials Improvements are being considered by service providers and commissioners This feedback is used as a quality measure at quarterly quality reviews with service providers This feedback helped the CCG Governing Body assess and mitigate the impact of the changes The results of this survey were considered by the Diabetes Clinical Programme Board to inform future commissioning The survey results were used to develop the Somerset Primary Care Plan The CCG and the County Council worked together to ensure a smooth transition to other services providing similar support Four patient stories were presented to the CCG Governing to identify learning points

7 Our priorities and challenges for the year ahead: In , our key priority will be to involve you in the development of the Sustainability and Transformation Plan (STP). There will be some key challenges ahead. How can we raise awareness and understanding of the STP? We will work with our community stakeholders voluntary organisations, Healthwatch, PPGs, patient leaders to cascade messages and updates to the public How can we reach people who do not usually engage with us? We will look for ways to engage the working population, marginalised or hidden groups, families, carers, minority ethnic communities, younger people, LGBT people and people who don t use services Sustainability and Transformation Plan We will work with health and social care colleagues across Somerset to involve patients, carers and the wider community in the development of the STP How can we promote equity and equality through the STP? With your help, we will assess the impact of service changes on different people How can we ensure public scrutiny? We will work with STP design groups and workstream leads to ensure that patient, carer, Healthwatch and voluntary sector representatives are involved when developing future services and new models of care How can we make best use of existing resources? We will re-frame some of our existing engagement conduits, such as health forums, to ensure that people in different parts of the county have the opportunity to feed back and influence the STP
