4 DMHA Mental Health and Addiction Forensic Treatment Fund HEA Funds specifically to support services for those without insurance coverage who are involved in the criminal justice system (see eligibility checklist)
5 Forensic Treatment Fund Two Funding Priorities: Pre-Incarceration Diversion Services & Post-Incarceration Re-Entry Services
6 Recovery Works Eligibility Criteria Is the individual a resident of Indiana? Is the individual at least 18 years of age? Taxable income does not exceed 200% of the Federal Poverty Level. (Taxable income includes participant, spouse, and any dependents. I.E. How much would the participant claim on their taxes? Has the participant entered the Criminal Justice System with a felony charge or prior felony conviction?
7 Recovery Works Reimbursement Voucher Reimbursement for the service is not available to an individual if the services being provided are covered under: A policy of accident and sickness insurance A health maintenance organization contract The Medicaid program* The federal Medicare program or any other federal assistance program
