2 Dear Friends, PacificSource is an independent, not-for-profit health plan that has been serving communities in the Northwest for more than 80 years. We care about the communities we serve and view each one as a part of our own backyard. We live, work and play in these communities and our members are our friends and neighbors. Founded in 1992, the PacificSource Foundation for Health Improvement is an expression of our commitment to our communities. Its threefold mission is to promote better health, enable better care, and to lower healthcare costs within the regions we serve. The Foundation s grants and partnerships focus on 1) improving access to healthcare for vulnerable populations and 2) promoting health excellence via innovative care and community health & wellness programs. This report offers a glimpse of the many ways that the Foundation and our company were able to make real and lasting differences in the lives of our neighbors in We are honored and proud to help so many local healthcare and wellness initiatives grow and flourish. 4 Tending to the places that we call home. Sustainable investments are a guiding principle for PacificSource and our PacificSource Foundation for Health Improvement. Together we are committed to supporting homegrown programs that aim to ensure access to high-quality healthcare. Our best tools include connecting with community partners to improve health. We do this by providing financial support and also by sharing our skills and resources to promote community created solutions. We sponsor diverse organizations working to improve the lives of vulnerable populations, test promising healthcare innovations, and seek to improve the overall health and well-being of the community.
5 Primary grant and philanthropy programs PacificSource Foundation for Health Improvement Our foundation is focused on serving vulnerable populations while enabling innovative community health and wellness programs through grants as well as offering our expertise to community partners. Access to Healthcare $207,700 Health Excellence $827,500 PacificSource Community Support and Volunteerism PacificSource and its employees actively support and engage in community programs. These include company grants and sponsorships, deep relationships with academic and scholastic partners, employee-fueled charitable campaigns, volunteerism and financial support. PacificSource Healthy Life University Partnerships: $275,000 University of Oregon Oregon State University Healthy Life Challenges: $7,000 Healthy Communities: $85,000 Charities of Choice: $57,000 Company-Wide Charitable Campaigns: $230,000 In the past six years, PacificSource has invested nearly $12 million across our local communities.
6 Our Access to Healthcare grants bear fruit (and vegetables). We believe that a true test of a healthy society is how it provides for its children and most vulnerable members. We work to ensure that everyone in our communities obtains the healthcare services they need, regardless of their ability to pay. The PacificSource Foundation s Access to Healthcare grants emphasize support for children and youth. We also fund programs that serve vulnerable adults, working to eliminate health disparities and striving to achieve health equity for every member of the community. Our partner organizations deliver a wide range of medical, dental and behavioral healthcare services that provide access to culturally appropriate and comprehensive primary care (physical, mental, behavioral and oral). These grants focus on helping to ensure that the most vulnerable members of our communities are able to grow and thrive. Boys & Girls Club of Corvallis Regular dental care too often goes unaddressed in low-income families. The Boys & Girls Club of Corvallis, Oregon has taken it upon itself to eliminate dental emergencies among low-income children and adults in Benton and Linn Counties due to inadequate dental care. Through cooperation with county services, the support of volunteer professionals like the Johnson Dental Clinic, and financial assistance from the PacificSource Foundation and others, the Club continues to expand the availability of oral hygiene and dental services in the area. Dr. Kenneth Johnson, founder of
7 the Johnson Clinic, speaks for all volunteers when he says, You reach out to people to help them, not for recognition, but for the satisfaction in your tummy and in your heart. Volunteers in Medicine Clinic of the Cascades Last year the PacificSource Foundation funded care for 85 patients of this Central Oregon clinic serving low-income, uninsured adults not eligible for services under the Affordable Care Act. Those patients represented 1,113 medical problems and 1,391 visits to the clinic. The small staff is augmented by volunteer medical professionals and others who provide primary and specialty care, prescription medications, mental health and patient education programs. Thanks to the Foundation s support of the dedicated clinic staff and volunteers, the patients were able to stay active as family breadwinners and use their hard-earned wages for basic necessities such as rent, food and utilities instead of daunting medical bills. Canyon County Community Clinic Operating entirely via donations, grants and volunteers, this western Idaho clinic provides all its services free of charge to the area s underinsured and underserved. In cooperation with community partners, the clinic has developed a comprehensive program for tending to low-income chronically ill patients. It includes immediate stabilization followed by a 5-week curriculum focused on meal planning, food preparation, exercise, lifestyle choices, and connecting to accountability partners. With a pilot launch targeting diabetic patients, the clinic is using its PacificSource Foundation funds to offset the cost of specialist referrals and educational materials Oregon Grant Recipients Oregon Boys & Girls Club of Corvallis Center for Community Counseling Medical Teams International South Lane Mental Health Volunteers in Medicine Clinic Eugene Volunteers in Medicine Clinic of the Cascades Looking Glass Youth Family Services Central Point School Community Outreach Inc. Eugene School-based Health Center Idaho Children s Home Society of Idaho Snake River Community Clinic Canyon County Community In 2015 the Foundation funded $207,700 in Healthcare Access grants aimed at serving venerable populations.
8 Sowing Innovation and Excellence. Big ideas can grow from a tiny seed. The PacificSource Foundation s two Health Excellence programs are born out of our commitment to help local communities plant seeds of innovation that can positively impact community health and wellness. We partner with and financially support diverse organizations to promote creative thinking that reinvents and refines the delivery and quality of healthcare. From tobacco cessation and nutrition programs to promoting care coordination and chronic disease management, our Community-Based Strategic grants seek out and support local organizations involved in healthcare initiatives that help people through the power of prevention and by being able to get the care they need, when they need it. Our Community Health Excellence (CHE) grants support clinicians and hospital systems in testing and adopting models of care that have the potential to significantly benefit patients. Over the years these grants have funded some 70 such initiatives, improving service to patients in areas such as infection control, best practices in acute and preventive care, chronic disease management, and the integration of care. Kootenai Health This Coeur d ALene, Idaho health system repeatedly earns regional and national recognition for its outstanding health services and for giving back to its community. A PacificSource CHE grant is now helping Kootenai eliminate the gap in care between hospital discharge and ongoing monitoring in sub-acute settings. This problem can be particularly prevalent in rural areas and among elderly and low-income patients, In 2015 the Foundation funded $639,000 in CHE grants and $188,500 in Community-based Strategic grants. leading to setbacks and high hospital readmission rates. Today, a new Nurse Navigator connects with all discharged patients, coordinates outpatient care and helps prevent any issues that might make readmission necessary. Cornerstone Community Housing This Lane County, Oregon nonprofit is not only committed to building quality, affordable housing for people living on limited incomes, but to providing
9 services that promote good health and personal growth. With the help of a PacificSource strategic grant, the Cornerstone Healthy Homes program connects affordable housing residents with services such as health and wellness coaching, medication management and dental care from on-site community health workers. Cornerstone programs like Active Kids, Teen Chef, Get Motivated and Community Gardens are now in place to make the habits of good nutrition and a healthy lifestyle fun, rewarding, and easy to get involved with. Billings Clinic Foundation In Yellowstone County, Montana Billings Clinic has joined forces with Riverstone Health and St. Vincent Healthcare to create Healthy by Design a community care model envisioned to provide for patients with complex issues. Our strategic grant is supporting the development of this vital health service, whose pilot launch is scheduled for the fall of Preliminary steps include a gap analysis to prioritize basic needs and health services, identification of community services key to supporting targeted patients, and strategic planning framed by a collective impact model. 10 At PacificSource, we all work to nurture a vigorous community. Our employees at PacificSource bring the sunshine to our community efforts and are personally engaged in the service of improving our communities health. They bring creativity, passion and many hours of personal time and commitment of resources to brightening the lives of their neighbors. PacificSource and its employees are helping to cultivate initiatives like these: Healthy Life reflects our drive to inspire positive health behaviors that promote health and well-being. We have created fun and effective ways to encourage healthier diets, regular exercise and healthful behaviors at schools, sporting events, and workplaces. Working directly with community partners, PacificSource employees contributed their time and resources in 2015 toward wellness programs promoting physical activity, healthy eating, tobacco cessation and stress management. Healthy Communities is our employee-driven community giving program that supports nonprofit organizations improving access to care or encouraging healthy lifestyles, and/or sponsoring safe, healthy, educational and fun family-oriented activities. In 2015, our employees selected key local organizations that benefited from more than $140,000 in PacificSource financial assistance and 573 hours of volunteer time. Charities of Choice. Employees in each regional office select one community organization to support in an especially significant way. PacificSource provides funding and the employees find creative ways to leverage it while also getting personally involved as volunteers. It s a cornerstone of our commitment to a culture of deep community engagement.
11 PacificSource Healthy Life Schools Challenge This program aims to improve the health of K-12 teachers and staff, while serving as positive role models for students. Participants compete in a month-long friendly competition that encourages healthy behaviors, empowers staff to be role models for students, improves overall health, and increases an appreciation of wellness. We have seen that this popular program can also reduce medical costs and decrease absenteeism. Enthused one recent participant: Loving this program! I m actually rather amazed at what a difference it s making. Women and Children s Alliance In Boise, PacificSource employees gathered presents and decorated a Christmas tree for a family of five they found via their Charity of Choice, the Women and Children s Alliance. The family included a mother with a hearing impaired 8-year-old whose wish list included a hearing aid, and three young boys hoping for toys and sports apparel. Employees collected some $1,500 in gifts and an additional $460 in gift cards. Their reward was a note of heartfelt thanks from the family: This is light at the end of a long, dark road, to a happier life. Helena, Montana Food Share In Helena, Montana, every 1 in 7 residents struggles with food shortages. Medical emergencies, disabilities, job layoffs and other issues create a situation where Helena Food Share, the area s principal emergency food provider, is serving some 1,500 households a month. In support of the organization s annual CanStructure event as its Charity of Choice, the PacificSource Montana office raised $890 to pay for 1,700 pounds of canned goods, which were donated after the event. This included over 600 meal in a can items serving a high need in the community. Our 2015 Charities of Choice: Idaho Falls ID Children with Disabilities Disability Services, $2,000 Boise ID Women and Children s Alliance Women s Services, $10,000 Helena MT Helena Food Share Hunger Relief, $7,500 Southern OR The Asante Foundation Health Services, $7,500 Central OR Partners in Care/Hospice House Health Services, $10,000 Portland OR School House Supplies Youth Education, $10,000 Willamette Valley CASA of Lane County Youth Services, $10,000 Corporate Campaigns Through employee pledges, fund-raising and PacificSource contributions, in 2015 our company donated over $100,000 to the United Way Campaign and $22,500 to the March of Dimes.
