2 Welcome North East London NHS Foundation Trust area North East London NHS Foundation Trust (NELFT) provides community and mental health services for people living in the London boroughs of Waltham Forest, Redbridge, Havering and Barking and Dagenham and community health services in south west Essex across Brentwood, Basildon and Thurrock. With an annual budget of 314 million in 2012/2013, we provide care and treatment for a population of almost 1.5 million and employ around 5,500 staff. Find out more about NELFT and our services visit: Telephone: This has been a momentous year for the NHS and a challenging year both for NELFT and for our local health economy. The changes to NHS commissioning and the issues raised by the Francis Report together with the continuing pressure on public finances have been felt locally as well as nationally. For us it has meant that, after several years of progressive growth, the last financial year has given us the opportunity to consolidate and embed our community health services within NELFT, to expand and improve the quality of our mental health services and to reflect on what we have achieved in a relatively short space of time in becoming an established provider of both mental and physical health services. During the year, we prepared for the launch of the clinical commissioning groups (CCG s) in April With the national focus on integrating services and on delivering care closer to home, NELFT has spent the year laying the foundations for some large scale changes to the way we operate. In October 2013 we will begin moving to borough led structures for the delivery of services that mirror commissioning structures by re-aligning our service to our boroughs in a more integrated way. Merging the leadership of community and mental health services will support us to deliver more holistic care to our patients and service users and help us to further develop relationships with our health and social care partners. Recognising that there is a mental health component in many long term conditions and addressing the physical health needs of our mental health service users has long been a focus of our Council of Governors and we are grateful to all our governors for the commitment and encouragement they have shown over the year in helping us to progress this agenda. 3 A listening organisation As NELFT continues to grow, we are keen to ensure that the people who use our services, and their relatives and carers, have the opportunity to contribute their experiences, views and ideas to influence service improvements and developments. We communicate regularly with our 10,200 public members who represent our community across north east London and south west Essex through sharing news via our members magazine or getting involved in community events. We have a team of active and involved governors who influence the decisions being made in the boardroom and a range of people, from all walks of life, who volunteer their time, take part in steering groups and play an active role in sharing their thoughts and experiences of NELFT services. Add this to our 5,500 staff and we feel confident that we are hearing a good balance of views about what is happening on the frontline and what makes a difference to our local communities. Patients and service users get involved Over the past year, over 80 patients, services users and carers have taken part in activities ranging from taking part in steering groups for NELFT projects, supporting teams with training activities and developing ideas for new services. Increasingly our staff are involving patient representatives on interview panels, reinforcing our aim to put the patient perspective at the centre of our services. Patient surveys As well as listening to views on the big decisions, we also carry out a wide range of patient and service user surveys that capture individual experiences. Patient surveys are a vital source of feedback to us on areas in which our clinical services are doing well and areas that need to be improved. Encouraging innovation from staff In a similar format to the BBC television programme, Dragons Den, the NELFT Innovation Cave gives staff the opportunity to pitch their ideas for service improvements to a team of directors and clinicians. The project has been incredibly popular as it gives staff access to funding to make improvements in their area of work. Approved projects have included a patient education programme for our coronary heart disease service, an inpatient medicines management system and a training programme on how to launch and run a community radio station for the Waltham Forest community drug and alcohol service. Valuing our volunteers For the NELFT staff awards 2013, we added a new category, volunteer of the year, which recognises the invaluable contribution of patients, service users, carers and other members of the public who volunteer their time and energy to help services develop and improve. There were over 30 nominations for groups and individuals and the judging panel had the difficult decision of choosing just three winners for their outstanding contribution to NELFT services. The winners were: L!sten, who are a young people s service user participation group made up of ex-service users. Volunteers working with the south west Essex community mums and dads service to support local families. The Friends of Goodmayes who, through their fundraising and volunteering activities, have provided a number of facilities, including funding for a new calming room and a raised bed garden for patients in Sunflowers Court. 4 Improvements and achievements children and young people NELFT s children and young people s services have had a remarkable year. Here are some of the achievements of NELFT services. NELFT is Baby Friendly in south west Essex Staff at children and young people s services in Essex achieved Stage 1 accreditation, in UNICEF s Baby Friendly Initiative, an internationally recognised standard which reflects a commitment to breastfeeding promotion and practices, in February Jill Williamson, who is leading the initiative to encourage more mothers across south west Essex to breastfeed, said: We are really committed to increasing our breastfeeding rates and want to ensure that our staff have the knowledge and skills to help mums get it right and stick with breastfeeding. This is why we joined the Baby Friendly scheme. Service users train clinicians Current and past mental health service users experiences were the focal point of training for staff at the child and adolescent mental health service (CAMHS) in Barking and Dagenham in February The training centred on clinicians learning more about the service from a young person s perspective. Ex-service users and young people still receiving care shared their personal experiences with the clinicians; what worked well, how they felt at different stages of their treatment and how other young people in a similar position to them might feel when coming into the services. Together, they explored ways to fine-tune the interactions between staff and service users and looked at how to make services more accessible. Merrisha Gordon, CAMHS service manager said: Our clinicians have reported how beneficial they found the training. We are very privileged to have such committed young people working with us to improve services. Pictured left to right: Stacey Soloman and Dr Amanda Jones Perinatal service achievements Our perinatal parent infant mental health service (PPIMHS), based at Goodmayes, is a specialist psychological service that works with women with mental health problems during pregnancy, as well as parents and children up to the age of three. Over the year the service had a number of achievements which recognised the passion, skill and dedication of the teams involved. In November 2012, Dr Protti, consultant perinatal psychiatrist, was elected to become a member of the executive committee of the perinatal section at the Royal College of Psychiatrists. The committee represents the interests of perinatal psychiatry nationally as well as fostering research in the area. Dr Protti said: I am delighted to be on this committee and representing my discipline and NELFT. Dr Amanda Jones, consultant perinatal psychotherapist, took part in a BBC3 documentary, aired in March The programme was about teenage mothers and post natal depression. Singer, Stacey Solomon was featured talking to Dr Jones about her experiences of post natal depression after the birth of her first son. Carol White, service director for child and adolescent mental health and psychological services, said: It is fantastic that this service is gaining national recognition, highlighting the dedication and talent of staff. This type of recognition will continue to promote and enhance the work that takes place within the NELFT perinatal mental health service. 5 Improvements and achievements adults and older people During the last year there have been new and innovative projects and service developments that will improve care for patients and service users receiving community and mental health services. New initiatives to improve care for people with dementia NELFT took an important step in its care of older people by implementing the national butterfly scheme. The scheme, which was launched across NELFT services in March 2013 by Geraldine Rodgers, nurse consultant for adults and older people, trains staff on how to improve their interactions with individuals who have been diagnosed with a dementia related memory impairment. This is enhanced by the use of a visual device that can be used to quickly identify if they have a memory impairment. Staff, including receptionists, porters and clinicians, who do not have formal training or experience in how to deal with patients with memory impairments, developed their skills to understand more about the subtle yet effective techniques they can use to improve the care of patients with memory related issues. As well as the training, a simple sticker of a butterfly is placed on the patient file of anyone diagnosed with memory impairment. As a result, anyone seeing the file is able to quickly and discreetly identify a patient who may need additional support so that Pictured: The butterfly symbol is clearly displayed on patient files they can receive more effective and appropriate care, reducing their stress levels and increasing their safety and wellbeing. The scheme also trained champions who are based in a number of NELFT inpatient departments and community hospitals. Butterfly champions provide on-going support and guidance to staff around the scheme. Delivering care closer to home Delivering care closer to home was, and continues to be, a key theme. In particular there were a number of service developments to avoid unnecessary admissions to hospital, or long-term residential care, and to reduce the length of stay in acute and community wards. In Waltham Forest, we worked with Barts Healthcare NHS Trust, GP partners and residential care home providers to launch a rapid response service in winter This means that people who have a long term condition and require urgent care can access a member of the rapid response team who can meet their needs, reducing the need to attend accident and emergency departments. With a similar aim, NELFT community and mental health teams working in Havering, Barking and Dagenham have teamed up with Barking, Havering and Redbridge University Hospitals NHS Trust to launch a new community treatment team (CTT). The CTT is led by a consultant geriatrician with a team of occupational therapists and nursing staff who assess a patient s needs, deliver, or organise, the urgent care required, and refer on to one or more specialist teams, including specialist long term condition services, falls clinics, social care and adult psychiatry teams. Our intermediate care service in south west Essex and social care services at Thurrock Council are working together to provide intermediate care to adults and older people by providing a range of integrated services, including integrated community teams, that promote faster recovery and prevent unnecessary hospital admissions. 6 Valuing staff NELFT recognises the importance of valuing and developing staff. There is evidence of a strong relationship between healthy and happy staff and a positive patient experience. Staff awards 2013 NELFT recognises its healthcare professionals and support staff that have gone the extra mile to care for patients and service users through its annual staff awards. John Brouder, NELFT chief executive, said: We know there are many people working for the trust who go above and beyond to provide an excellent service to patients and service users and help make continued improvements to the care we provide. We wanted to make sure their work is recognised by presenting them with a NELFT award More than 150 nominations were received for staff across the trust including clinicians, health care assistants, dental specialists, inpatient teams, community nurses and support staff. The finalists for each of the categories were invited to attend an evening event in Billericay, where winners were presented with their trophies in front of an audience. One of the three winners for the outstanding contribution award included Jack Closs, a health care assistant who works at Sunflowers Court mental health facility in Goodmayes. Other winners were recognised for establishing new services, for example Charlotte Curl, senior dentist at Hainault Health Centre, was recognised for developing a new service that supports patients with a phobia of dental care. NHS Heroes NELFT took part in the NHS Heroes The scheme was launched nationally in the summer of 2012 to recognise the NHS staff, from senior doctors to ancillary staff and managers, who go the extra mile to improve life for people in their working environment. Nominations were invited from patients, service users, carers and work colleagues. NELFT held a special gathering to celebrate our 22 individual and team NHS heroes in November. Pictured: Marisol Lehan using the health check kiosk Supporting staff health and wellbeing Over the past year, we have continued to work with staff in a number of ways to ensure that they are supported and feel that they are part of an organisation that values, nurtures and develops its workforce. One of the ways we do this is through our regular improving working lives groups that provide a forum for staff to feedback to senior management. Through these, staff can contribute their ideas, experiences and feedback. These forums are made up of different disciplines and work areas and provide a useful platform for 360 degree feedback. In November 2012, the group that meets in south west Essex, called Staff Plus, achieved the Staying Healthy at Work accreditation award which is part of the NHS East of England programme to improve the physical, mental and social wellbeing of employees. Annie Cooper, assessor and programme lead for Staying Healthy at Work, said: The staff have put a lot of work into this achievement. This level of commitment and support to staff health and wellbeing is linked to high performing NHS organisations. A number of activities have been set up to support staff as a result of feedback at the forums. These range from a walking group which meets every Tuesday lunch time to walk around the Goodmayes Hospital site, weight loss support groups, recognition schemes and the availability of a health check kiosk which assesses blood pressure and heart rate. 7 NELFT success The trust had a number of success stories during 2012/13 Pictured left to right: Dan Childs, BBC producer, Rachel Bruno and Dr Bill Travers, consultant psychiatrist Mental health services recognised in television documentary The mental health inpatient unit at Goodmayes became the centre of attention during a BBC3 documentary presented by Rachel Bruno who is heavyweight boxing champion Frank Bruno s daughter. The progamme, called My Dad and Me, focused on care for people with bipolar disorder and the treatment adults with severe mental health problems receive from NELFT s inpatient unit, Sunflowers Court. Frank Bruno credited the facility with giving him his life back following a breakdown in his mental health ten years ago. The programme was filmed at the beginning of 2013 and has been broadcasted a number of times during the summer. Dementia care NELFT celebrated the achievements of Maureen Kanabar, an Admiral nurse who won the Dementia Carer Award at the Great London Care Awards Admiral nurses provide specialist care to people with dementia and memory problems. Maureen was nominated by a carer of a patient with dementia. Maureen has been a nurse since 1979 and has specialised in dementia nursing for more than ten years before winning the award that recognises and rewards high quality nursing leadership in dementia care. Pictured: Maureen Kanabar, award winner at the Great British Care Awards 2012 Research and Development Research and development is fundamental to NELFT with an essential role in developing and evaluating new approaches to provide more effective, higher quality, better value care. Underlining our commitment, NELFT was named as one of the top NHS trusts in the country for increasing opportunities for patients to take part in clinical research studies in mental health by the National Institute for Health Research s Clinical Research Network (the research delivery arm of the NHS). During the year, NELFT involved 1,707 participants in research portfolio studies in comparison to 764 participants in the previous year. We have more than 38 active portfolio studies, which is indicative of high quality clinical research funded through competitive grants and linked to the National Institute for Health Research Clinical Research Network. We have also seen an increase in the number of non-portfolio studies. Over the year there were also 35 local studies which provide us with authentic information to support service improvements. These home grown research projects looked at various aspects of improving community and mental health including an investigation into ways to prevent anxiety disorders in young children and research into predicting responses to physiotherapy among patients with shoulder pain. A growing reputation NELFT s reputation as a leading organisation in research and development has led to high profile and international attention. Highlights include NELFT s director of research and development, Professor Martin Orrell, attending 10 Downing Street to discuss dementia care and visits from Japan s Ministry of Health and an Australian care home organisation for 30,000 older people in South Australia. 8 Celebrating the Olympic Games In August 2012, NELFT joined the rest of the world in celebrating the Olympic and Paralympic Games NELFT provides services across Barking and Dagenham and Waltham Forest which were two of the six official Olympic boroughs. NELFT were also proudly represented by a number of staff who became Olympic gamesmakers. Staff were involved in the opening and closing ceremonies, and as part of the track-side press team. Pictured left: paralympian Sophie Warner at the John Tallack Centre, Thurrock Community Hospital Inspirational presentations and visits The second NELFT allied health professionals (AHP) conference took place in October 2012 and was opened with a moving presentation from Paralympic athlete Martine Wright. Martine, who lost both legs in the 7/7 bombings at Aldgate, spoke of how, with the help of AHPs, she has become a Paralympic athlete and is member of the GB sitting volleyball team. Paralympian Sophia Warner inspired and motivated staff and young patients at the John Tallack Centre, Thurrock Community Hospital in February Sophia, a Commonwealth gold medal sprinter and London 2012 Team GB Paralympian, toured the therapy clinic where patients are fitted with specially designed garments to aid their mobility.
