2 Our AIM Together every person in the Algoma School District Community will improve one or more dimensions of their well-being, by September 1,
3 5 6 Over 100 people have been educated on the nutritional principles of eating more fruits and vegetables, cutting out processed foods, eating healthy proteins and fats, and avoiding sugary beverages. In a pre- and post-survey of participants in the program, 100% of the individuals reported improvements in at least one nutritional principle. Over 100 school-age volunteers showed up to clean the city s Crescent Beach, documenting collections of trash, and increasing awareness of what waste can do to our ecology. 3
4 7 274 Active Community Members at the Algoma Community Wellness Center What began as an initiative at a high school basketball game, was scaled up to teaching Hands Only CPR to well over 800 individuals at local businesses, NFL Packer game, and within the Hispanic community. This led to the team earning the American Health Association Community Impact Award, and being recognized by the Green Bay Packers with funding and creation of a public service announcement aimed at fostering awareness of learning CPR articles of clothing have been taken from our Clothing Closets at the Algoma Public Schools Community Fab Labs Students teach community members how to use workshop tools. Tools are accessible to those who do not have them at home. Sixty-seven (67) community members have been trained so far. 4
5 9 Community members raised over $2,600 spreading awareness of Breast Cancer in our community. They realized that showing support and awareness was an important first step, and that educating and finding ways for our loved ones to get screened was another priority. As a result, they shifted their thinking (a Live Algoma Way of Being ) to use the money in a different way, which resulted in purchasing mammograms for people who don t have the insurance or financial means to get screened. All people matter and early detection helps save lives. Working with city officials on the comprehensive city survey to connect it to our work. 10 Adding a new salad bar resulted in a 450% increase in weekly average vegetable consumption, and contributed to an increase in baseline indicators from 35/100 to 51/100 on the Smarter Lunchroom assessment in our first year. In 2016, a new ice rink was built to take advantage of our cold weather climate and encourage locals to get out and move in winter. Now, we are working on a walking/biking trail! 5
6 11 93% Utilization of our New Community Nurse Activator at the Algoma Community Wellness Center. Physical Therapists bring their patients to the Algoma Community Wellness Center for their last appointment. The PTs show them how to independently continue their care and they sign up for a membership at a discounted rate. To date, 24 people have completed this program. 12 This year the Live Algoma Coalition is working closely with the University of Wisconsin-Green Bay Social Work class of 2017 to gain a greater understanding of the Live Algoma community and individual needs as well as current participant progress. We are striving to increase Live Algoma pledges and follow-up with those who are already participating in the Live Algoma initiative. 6
7 13 The Shanty Days 5K Run/Walk raised enough money to donate a wheeled racing stroller to the Algoma Community Wellness Center. Using the Switch Thinking Framework, which helps people to make hard choices more easily, three of the largest employers have implemented healthy snacks initiatives, and the local grocery stores have created recipes highlighting nutrient dense foods that are on sale. 14 Fourteen high school students involved in Youth Leadership recognize the need to continue the momentum and building of generative leaders. As a result, they developed a Blueprint for youth engagement. The purpose of the Blueprint is to believe in and empower kids to ignite their passion to make a difference in their community. The nine step Blueprint includes: Dream Big Start Small Create a Shared Vision for Tomorrow Build Coalitions Unity of Message Moving Pillars of Power Leave No One Who Wants to be Involved with Us Behind Develop Your Strategy and Tactics Build Momentum Finish What You Started 7
8 15 Creation of Pathfinder Academy An innovative space aimed at engaging, empowering, connecting and preparing individuals. The most significant accomplishment of developing youth leadership is the creation of Pathfinder Academy. In the summer of 2016, the School District of Algoma purchased a building on the main street business district to physically integrate and nurture this process of student learning in the heart of downtown. This academy provides an interactive environment where students are self-directed and linked to the community. This hands on approach to learning offers a progressive way of achieving knowledge and dreams. At this time we have approximately 15 students participating at Pathfinder Academy, and we expect that number to grow as its reputation builds. 16 Students who learned the PDSA process inspired our teachers to learn it too! 33 District Staff now using PDSAs! 8
9 17 Live Algoma also asked community residents to make an individual commitment by taking the Health and Wellbeing Pledge. The pledge has been completed by 700 individuals to date, almost 25% of the city s population and has been instructive in identifying a range of activities to improve health across multiple dimensions. Commitment to change in writing and sharing with others is known to be an effective strategy for increasing readiness to change. Brief intervention is an evidenced-based strategy that is used to initiate change for an unhealthy or risky behavior. While frequently focused on alcohol use or smoking, this prevention approach can also be used to foster other health practices. With the limited community resources available, the pledge is a cost-effective means of starting the conversation about health and wellness of individuals and empowering members to recognize areas where they can control their own health.. 18 Shared our work with 9
10 19 Activation Team Leads pulled data from all of our existing resources and grouped the data within the seven dimensions of our wellbeing. 20 Team members reviewed the data grouped by dimensions and chose data points that would pertain to their area of focus.. 10
11 Failing Forward what we have learned 21 The turning points in our journey were deciding to ask the people what they want. The collaborations (both likely and unlikely) that we built were youth voice. This is hard work! To make an impact we have to improve the systems. BRIGHT SPOTS who have taught us a few things Learning from the Oklahoma City Public Health Department, we have built off of their Community Health Worker model to develop a Community Nurse Activator role in the Algoma Community. The service is Free to community members and has shown high utilization. Local providers view this role as an extension of the primary care team and a link to community resources and assets. The Quinn Community Center, in partnership with Proviso Partners for Health in Chicago, has taught us that we don t have to have the answers for everything. In fact, we don t have to even have a space perfect in order for youth to be empowered. As a result, we started our Pathfinder Academy knowing that we didn t have to be 100% ready and that the youth would show us the way
