2 Doorway is funded by the Victorian Government to assist 50 homeless people with serious mental illness to establish homes in the private rental market, and build lives in the community. It is a 3 year demonstration project

5 Utilising private rental in the community 46% of people with a mental illness are homeless or in inadequate housing Private rental as a solution Based on Housing First model as developed in the Pathways to Housing program in New York 5
6 Engaging Real Estate agents 3 year arrangement Market rent paid on time Landlord insurance paid Access to flexible support from Housing and Recovery Workers who visit the tenant regularly in their home The project covers the cost of furnishing the property for tenants, ensuring that tenants can quickly establish their new homes Surety Fund available REIV supported 6
7 Engaging clinical treatment Clinical treatment is essential most people with a severe and enduring mental illness Clinical team commits to supporting the person to be successful in their tenancy Clinical team manages referrals & eligibility Doorway co-locates with clinical services 7
8 Supporting Recovery Housing and Recovery Worker (including peer positions) Initially weekly support visits Mentors and offers support around tenancy related issues Liaises with real estate agent Intentionally supports the participant to take a lead Supports the development of natural supports Takes a lead in enabling the integrated team 8
9 Engaging family and friends Participant encouraged to invite family and friends to work with them to create a home 9
10 Integrated Support team Creation of a single supportive team to assist the participant to meet their goals Meet on at least a 3 monthly basis more if needed Core team and flexible components Directed by the participant Flexible components include employment specialists, family peer support, cultural/spiritual support 10
11 Choice Support participants to articulate goals and preferences Empower the participant to have choice and control over the services they receive Help participants to choose their own housing and home environment Co-design wrap around services with participants 11

14 Sustainability Assist participants to develop skills in choosing and managing tenancies Support participants to develop good neighbor skills Participants own the lease Provide tailored employment assistance Aim for financial independence 14
15 69 people have accessed the program so far. 52 have commenced leases. It takes an average of 4 applications to get a lease.

17 Positive Mental Health Outcomes After 18 months: Time in bed-based clinical mental health services per client per year has decreased from 43.4 days to 8.1 days Biggest decreases occurring across Community Care Unit (24.2 to 2.6 days) and acute in-patient (11.6 to 3.7) services A shift to planned services, rather than crisis driven Six participants have progressed to the point of being able to be discharged from their Area Mental Health Service 17
18 Health Outcomes more engaged in the development and management of their support plans and with their primary health providers easier for clients to attend appointments with clinicians, and for clinicians to stay in touch with or seek out their clients if need be ED presentations across all clients in the 12 months pre and post-housing decreased from 117 to 44 total number of hospital admissions across all doorway clients decreased from 24 to 5 pre and post-housing 18
19 Tenancy Outcomes Most clients are successfully managing their tenancies Most clients reported feeling more independent, having greater levels of self-respect and pride and finding greater meaning in their lives Six clients have been offered and accepted their second 12 month lease The number of tenancy related incidents has been relatively low with five lease breaks by clients (excluding those leaving the program), six breach of duty notices and no evictions Most clients are able to manage their rents with only seven people falling into rental arrears on one occasion and four people on more than one occasion. Doorway Workers have negotiated re-payment plans with participants in all instances of rental arrears 19
