2 HEMI-EPIPHYSIODESIS The minimally invasive treatment to guide growth
3 Congenital abnormalities, but also infections and injury, can cause the growth of a child s thigh and lower leg to become unbalanced. This misalignment frequently leads to joint malalignment (varus or valgus malalignment). The classic indications are knock knees and bow legs. Limbs misaligned in this way can be corrected with a minimally invasive hemi-epiphysiodesis on the convex side, that is on the inner side in the case of knock knees and on the outer side for bow legs. The implant restricts (desis) thgrowth in the growth zone (physis), allowing the growth to be guided as required. An angular deformity can corrected in this way by guiding the growth. As the bones are not severed, the possible neurovascular complications of this method are significantly less than in osteotomy. It is also possible to keep the instability of fragments and the time-consuming healing phase to a minimum.
4 ADVANTAGES of treatment with a hemi-epiphysiodesis plate The preset bending point in the centre of the implant allows the surgeon to shape the hemi-epiphysiodesis plate to the bone in a controlled fashion. The two small terminal spikes allow secure positioning without risk of displacement once the position on the periosteum has been established, and this also ensures the secure placement of the marking wires for the screws to be introduced. The spikes also exert an additional stabilising effect on the seating of the plate and the guidance of growth. To ensure that the epiphysiodesis plate is accurately aligned and the screws are introduced as required, the surgeon has the option of using two K-wire holes and a central guide hole. Using angle-stable screws allows growth to be restrained. For guided growth, standard screws are used divergently. The slim plate design and completely sunken screw heads minimise the long accustomisation periods and particularly the formation of painful bursa sacs over the screw heads. The low-contact contouring of the underside of the hemi-epiphysiodesis plate is particularly gentle on the periosteum and enables the metal to be removed easily later.

VaWiKo is derived from the German designation Variabel winkelstabile Kombination, which translates into English as Variable Angle-Stable Combination. This system combines conventional plating technologies with angle-stable locking in variable angles. In Germany, in the EU states, Switzerland and in the USA TiOB is a Registered Trade Mark.
