8 St. Charles Hospital offers an innovative form of rehabilitation with Downtown St. Charles. This rehabilitation module is the only one of its kind on Long Island and provides patients the ability to experience the physical and cognitive challenges they may face in the real world following a life-altering injury or illness. Downtown St. Charles occupies 2,358 square feet on the fourth floor of St. Charles Hospital s inpatient rehabilitation unit. This unique rehabilitation environment simulates a typical community and is complete with a real life grocery store, café, a city street that includes curbs, a sidewalk, a stop light, a crosswalk, a real, full-sized car, a fully functioning three-room apartment and more. Downtown is a visually appealing, therapeutic environment for patients recovering from stroke, traumatic brain injury, orthopedic surgery, spinal cord injury or other life-changing injuries or illnesses. Patients learn to maneuver across streets, up curbs, in and out of a real car, around a fully functioning apartment, cook a meal, do their laundry, shop in a market, and use an ATM machine. They work on important life skills while still under the guidance and supervision of skilled therapists. This type of rehabilitation boosts self-confidence and helps patients return to the most productive and independent life possible.
9 Downtown St. Charles was an instant hit......with patients and therapists since the official opening in Many patients at St. Charles Rehabilitation have been away from home for weeks and even months. Downtown St. Charles looks and feels like a home and downtown street in any community. It provides a Town Square for visits with family and friends and a drop-down movie screen for group movie nights. Not long ago, a teenage boy was admitted to St. Charles Rehabilitation following a car accident. His mother commented that she had not seen her son smile since the accident - until the day he was brought into Downtown St. Charles. She stated that she was thrilled that his rehabilitation was going so well and that many of his motor skills were returning. She went on to say that it was the return of his beautiful smile that meant more to her than anything else. This innovative rehabilitation environment was three years in the making and surpassed all my expectations. We take for granted the ability to get out of bed in the morning, dress ourselves, and navigate around our home and community. 10 Our Car We ve relocated a full-size automobile to the fourth floor to provide patients with the ability to experience safe car transfers, preparing them for discharge. In the car, patients practice adjusting to moving around a confined space and objects such as a dashboard, steering wheel, and foot pedals. Patients also learn safe transferring of mobility equipment into the car trunk. Our Apartment Our fully equipped kitchen includes a working stove/oven, dishwasher, sink, refrigerator, washer, dryer and cabinets. Food can be prepared and cooked, dishes cleaned, clothes washed and dried and groceries put away. The bedroom features a twin bed that allows patients to practice safe transfers while the bathroom features a sink, toilet and bathtub. Our Market Our simulated grocery area is stocked with realistic produce and stocked shelves. The checkout counter features a cash register, bagging area, and check-writing platform. A hand-held shopping basket is available to assist with transporting groceries. The market is complete with a coffee bar. Mobility Tracks Varied floor surfaces such as carpet, tile, linoleum, and cement provide walking and wheelchair practice. A traffic light, programmed to the exact time it takes to cross a city street, allows patients the opportunity to practice safely crossing a street in a timely manner while still under the watchful eye of their therapist. An ATM machine and mailbox are featured along with curb cuts that model community settings. Our Town Square The Town Square is an attractive gathering spot decorated with a Tuscan theme with small tables and chairs where patients and their loved ones can visit over a cup of coffee. Computers with access to the internet are available for patients. A kiosk is available to patients and families featuring community and accessibility resources.

Treatment in our program begins with a thorough assessment of your needs. Both you and your family are key members of the rehabilitation team and you play an important role in the creation of your individualized treatment plan. Weekly team conferences and family meetings help guide and modify your treatment program. St. Charles Hospital employs the most technologically advanced rehabilitation approaches and equipment available today, while upholding a deeply felt commitment to each and every patient. Compassionate care for children Our inpatient program, including Long Island s only pediatric inpatient traumatic brain injury unit, is unsurpassed anywhere in the region. Children with brain injury, multiple trauma, cerebral palsy, and other injuries and disorders receive state-of-the-art care in our CARF accredited Pediatric Rehabilitation Program. All members of the child s treatment team meet weekly. Key members of the rehabilitation team are the child and parents. Both play an important role in the creation of the child s individualized treatment plan. Parents and our pediatric patients are involved with decision making every step of the way.

It was nationally recognized for its effective treatment of patients with infantile paralysis and for its role in helping the medical community turn the corner on this dreaded disease. Your rehabilitation team will work with you and your family to assist you in returning to an active and productive life. As a hospital-based rehabilitation program, your medical needs can be effectively managed at St. Charles. 16 Our commitment to teamwork gives everyone a place at the table. Each week, the rehabilitation team physiatrists, nurses, physical therapists, occupational therapists, recreational therapists, speech/language pathologists, psychologists, social workers, dietitians, and others as needed meets to discuss patients progress. Patients and their families are always an integral part of the rehabilitation team and their input is highly encouraged. Rehabilitation Nursing Rehabilitation nursing is a specialized from of care with the goal of returning patients to a maximum level of independence. Rehabilitation nurses possess unique expertise and are critical members of the rehabilitation team. Rehabilitation nurses perform standard nursing functions. In addition, they assist patients with ambulating, provide education and demonstrate the use of adaptive equipment. Most importantly, rehabilitation nurses collaborate with physicians, physical therapists, occupational therapists, speech/language pathologists, dietitians, social workers, case managers, pharmacists, respiratory therapists, and other members of the multi-disciplinary team to help patients reach their full potential.
