2 Why the Gwen Neilsen Anderson Rehabilitation Center? The Gwen Neilsen Anderson Rehabilitation Center (GNARC) specializes in medical treatment and rehabilitation for people with injury or illness requiring further rehabilitation before returning home. In FY 2016, the Center had 216 total discharges, including 74 discharges from our Stroke Specialty Rehabilitation program. Nearly 90% of these patients were discharged back to the community (15% higher than the national average). The Center served one adolescent during this time period. Research shows that recovery is optimized when rehabilitation care is located close to home. Having loved ones nearby and feeling comfortable in your environment helps the healing process. We believe that the best way to get better is to surround yourself with people who believe in you. Stroke 34 % % 63 Orthopedic Our Patient Population The Center has served patients from five surrounding states and eight Idaho counties, providing excellent care close to home. The Center treats close to 225 inpatients a year, including stroke, brain injury and spinal cord injury patients, as well as medical and surgical patients with complications related to these injuries. More than one-third of patients at the Center participate in our Stroke Specialty Rehabilitation program. We also support the stroke community by hosting a monthly stroke support and education group. As part of a larger medical center and health system, the Gwen Neilsen Anderson Rehabilitation Center provides smooth transitions along the continuum of care. The Gwen Neilsen Anderson Rehabilitation Center is a licensed, 14-bed comprehensive acute inpatient rehabilitation unit. The Center is fully accredited by the Commission on the Accreditation of Rehabilitation Facilities and provides state-of-the-art, evidenced-based rehabilitation care. It has been largely funded by The Craig H. Neilsen Foundation, a private paralysis research foundation, and is named in honor of Craig Neilsen s mother, a longtime Twin Falls resident. stlukesonline.org/gnarc

3 Functional Independence The Gwen Neilsen Anderson Rehabilitation Center at St. Luke s Magic Valley has proven results in helping patients achieve more functional independence and return to their communities. Total FIM Motor Efficiency is a measure of the amount of functional gain in movement the patient demonstrated during his or her rehabilitation stay compared to the time spent in rehabilitation. These measures are completed just after admission to the Center and again at discharge. Tasks measured include ability to walk, transfer, bathe, dress and groom. Discharge to Community is the percentage of patients who return to a community setting, including their home or assisted living residence. Functional Independence FIM Efficiency How We Measure Up Patients at the Center are more likely to experience better functional gains and returns to their community compared to patients at other facilities. The Center overall demonstrated efficiency in improving motor function up to 9% higher than the national average. Stroke patients at the Center achieved a Total Motor FIM Efficiency change up to 7% higher when compared to the national average. The Center s community discharge percentage is 14% higher than the national average. 4 Hospital Acquired Conditions Hospital Acquired Conditions are medical conditions or complications that a patient develops during a hospital stay. We are committed to reducing these complications during care so that patients can focus on their rehabilitation. Keeping patients safe is always a high priority. Falls Zero Complications Zero Central Line Associate Blood Stream Infections! By following evidence-based care and a focus on standardized processes, the Gwen Neilsen Anderson Rehabilitation Center has had no infections in over two years. Zero Acquired Pressure Ulcers in more than 28 months. Zero Falls with Harm in over 15 months. Zero Catheter Acquired Urinary Tract Infections in FY16! The expert nursing team implements standardized measures and processes to protect patients from infection. Continuous Improvement The Center has shown a continuous decline in falls and in falls with harm, due to effective reinforcement of strategy with staff, patients and family. A strong focus on detailed plans prior to discharge have allowed the Center to achieve readmission rates lower than national average. 5 Patient Safety St. Luke s Magic Valley and the Gwen Neilsen Anderson Rehabilitation Center continually strive to achieve excellence in patient safety. To better understand how our frontline clinicians perceive the level of effectiveness that exists at the Center during delivery of care, we voluntarily participate every 18 months in a validated safety culture survey. How We Measure Up Developing a culture that is transparent among caregivers is a continuous focus at St. Luke s Magic Valley. Results of this independent survey demonstrate the perceptions of the clinicians at the Gwen Neilsen Anderson Rehabilitation Center. The 2016 survey results indicate the Center outperformed other departments at St. Luke s Magic Valley, within St. Luke s Health System, and the national industry median for six of the seven survey domains.
