1 REPORT FORM - NDT EXPERIENCE Candidates applying for certification with sufficient qualifying work experience must complete these forms. Candidates applying for examination without sufficient qualifying work experience are not to complete these forms. The candidate must report industrial experience at three levels: as a Trainee, as a Level 1 and as a Level 2 inspector. - A candidate applying for Level 1 certification must report method experience as a trainee. - A candidate applying for Level 2 certification must report method experience both as a trainee and as a Level 1. - A candidate applying for Level 3 certification must report experience time as a trainee, as a Level 1 and as a Level 2. Each level of reported experience must be attested to in writing by the employee s supervisor at the time the candidate gained the claimed experience. In cases where this is not possible, your current supervisor may sign. A sponsor (CGSB certified Level 2 or Level 3) must co-sign the attestation of the various levels of experience. There must be three different signatures on the forms: candidate, supervisor and sponsor. NRCan verifies the claimed experience of 25% of applicants and imposes severe penalties for any falsification. Prior to NRCan certification, industrial experience must meet or exceed the minimums shown in Table 1. After 10 weeks, he has 10 x 60 = 600 hours or 600/175 = 3.4 months of experience. Experience older than 10 years from date on this report form is not acceptable. Definitions used in text: Supervisor: Experience claimed by the candidate must be verified as accurate by the person to whom the candidate normally reports. This person must be an official representative of the employer (company) for whom the candidate works. In the case of a self-employed candidate, the candidate cannot sign as his own Supervisor ; he must obtain the signature of an official of a company for whom he has worked and who is familiar with his work history. Sponsor: Experience claimed by the candidate must be attested to as accurate by an NRCan certified Level 2 or Level 3 inspector who is not the Supervisor but who is familiar with the work history of the candidate. Validate: Act of demonstrating that a verified procedure will work in practice and fulfill its intended function, normally achieved by actual witnessing, demonstration, field or laboratory tests or selected trials. 3 Part 1 - Applicant for Level 1 or Higher Certification (Report of Experience as a Trainee) An individual without certification who is working in the method is considered as a trainee. Trainees are gathering experience, under the supervision of Level 2 or Level 3 personnel, in various aspects of the method. Such experience is assessed against the scope of the competence for Level 1 personnel. At the time of application for examination, a trainee should be able to: a) set up NDT equipment; b) perform the tests; c) record and classify the results of the tests; d) report the results. The candidate must demonstrate these abilities prior to certification. 6 Part 2 - Applicant for Level 2 or Higher Certification (Report of experience as a certified Level 1 or equivalent) An individual, certified to Level 1 or equivalent, is gathering experience, under the supervision of Level 2 or Level 3 personnel, in various aspects of the method for assessment against the scope of the competence for Level 2 personnel. At the time of application for examination, a Level 1 or equivalent should be able to: a) set up and verify equipment settings, b) under supervision, select the NDT technique for the test method to be used c) know the limitations of application of the testing method d) perform the tests e) record and classify the results of the tests f) under supervision - interpret and evaluate results according to applicable codes, standards, specifications or procedures, e) report the results The candidate must demonstrate these abilities prior to certification. NRCan requires a detailed listing of the NDT method used, the specific equipment used, what was inspected, to what code or specification, date of inspection, report designation, who was the employer and the signature of a supervisor. Note: The supervisor must sign (not just initial) each claimed inspection. In each method in which the candidate seeks certification, the candidate must fully document at least five (5) inspections. 9 Part 3 - Applicant for Level 3 Certification (Report of experience as a certified Level 2 or equivalent) An individual, certified to Level 2 or equivalent, is gathering experience in various aspects of the method for assessment against the scope of the competence for Level 3 personnel. At the time of application for Level 3 examination, a Level 2 or equivalent should be able to show experience related to: a) full or partial responsibility for an inspection facility or job site b) a general familiarity with other NDT methods. b) knowledge of materials and process technology to select NDT methods, establish NDT techniques and instructions, c) assist in establishing procedures and acceptance criteria where none are otherwise available d) validate NDT instructions and procedures e) interpret results and codes, standards, specifications and procedures, f) supervise tasks at or below Level 2, g) provide guidance for personnel at or below Level 2 h) report the results of non-destructive tests. The candidate must demonstrate these abilities prior to Level 3 certification. NRCan requires a detailed listing of the NDT method used, the specific equipment used, what was inspected, to what code or specification, date of inspection and signature of supervisor. In each method in which the candidate seeks certification, the candidate must fully document at least three (3) inspections. For more NDT methods, add more sheets as necessary.
