The number of people aged 75+ living in your health board area. The total number of your community rehabilitation teams received in the following financial years , 11-12, 12-13, 13-14? The total average number of your community rehabilitation teams receive per week. Please give numbers for the last six weeks if an average cannot be supplied. How many staff are employed in each of your community rehabilitation teams? Please break down by staff type (eg therapeutic specialism) and specify if full time or part-time. Are there currently any vacancies? Where Community rehabilitation teams refers to teams in the community which help people who may be at risk of hospital admission because of a deterioration in their health but can be helped with input from therapists eg physios and adaptations to their environment. These patients may have been referred by their GP or may have been referred on leaving hospital. I can now respond as follows: The number of people aged 75+ living in your health board area. Moray Occupational Therapy: Buckie CTT 18.75hrs - For inpatients and community rehab. TI3 9 hrs Glassgreen CTT 2011 to beginning of WTE (Band 7) Glassgreen CTT Post Jan WTE (Band 6) Glassgreen CTT 3 Shared support workers with physio , 0.80, 0.20 Glassgreen CTT Part of WTE (Band 6) Forres 18 hrs (Band 7), 15 hours (Band 3) Speyside 18 hrs (Band 7), 15 hours (Band 3) Please note that Glassgreeen CTT is the nearest to a dedicated community rehabilitation team - the others only do community rehabilitation as time allows and tends to be the follow ups post hospital discharge. Aberdeen City CHP: We do not have formal community rehabilitation teams and we have therefore provided information for OT (NHSG) and physiotherapy (PT) to the community services during the period from 2012 (when SAP started) to the end of March OT Staffing: 0.5 Band 3 Clinical Support Worker (part time but half time in Intermediate care) 1.99 Band 4 Clinical Support Worker (all Part Time) 1.5 Band 6 Occupational Therapists (1 x Full Time, 1 x part time but other half in SDT) 1.47 Band 7 Occupational Therapists (all Part Time) Total for OT = 5.46 WTE Physiotherapy Staffing: 2.3 Band 3 Clinical Support Workers 1.5 Band 5 Physiotherapists 4.47 Band 6 Physiotherapists 4 P a g e

Aberdeenshire Community Health Partnership (CHP): NHS Grampian can advise that we are unable to provide this information as the physiotherapists and occupational therapists that work within the community are part of the overall Occupational Therapy and Physiotherapy departments within the Health Board and do not work in specified multi disciplinary rehabilitation teams.
