1 Person-Centered Thinking for Families

2 What is Person Centered Thinking?

3 Person centered thinking is a set of principles and core competencies that is the foundation for person centered planning.

4 Person centered thinking tools provide practical strategies. They help to gather meaningful information and facilitate conversations. They help to make sure that the focus remains on the perspectives of the person affected by the issue or outcome.

5 Many service organizations report that Person Centered Plans are more likely to be implemented and achieve positive outcomes when these skills are being used.

6 PERSON CENTERED THINKING Underlies and guides respectful listening, which leads to respectful action Supports people in having positive control over the life they desire and find satisfying. Where they: Are recognized and valued for their contributions (current and potential) to their communities; and Are supported in a web of relationships, both natural and paid, within their communities

27 Even though the one page profile has the same things I ve already told doctors, it seems to make a real difference having it written down, and in such a positive format.

28 We gave the triage nurse Dan s one page profile and they read through it. After that, the nurse spoke to Dan in a way that he could understand. Without having the one page profile, I don t think that the experience would have gone so smoothly. Dan would have had to listen while we described him to others which he finds very uncomfortable. It is an excellent way for people to understand and get to know Dan quickly.

29 One Page Profiles Things we keep in mind: Identify the purpose - A one-page profile can be a general introduction to the person. With this purpose, the most important information about the person would be included. A one-page profile can also be specific in terms of a place or context. Examples include: hospital, school, employment, home, etc. Keep it up to date - You must keep the information in a one-page profile up to date - this means reviewing it with the person on a regular basis. You can use the working and not working tool, and other personcentered tools to help you to do this.

36 Wyatt s Communication Chart What is happening? Wyatt does this It may mean this We need to do this An adult is giving Wyatt instructions or asking about something that has recently happened. 1. He avoids eye contact, eyes are moving around, eyebrows go up and down, tries not to smile 1. I hear you, but I m feeling ornery. Not going to admit to anything! 1. Tell him something funny then try again. 2. He looks directly at you and his eyebrows go up (and stay up) 2. I hear you, and I m going to do what you ask or tell you the truth about what happened.. 2. Smile and let him know you re pleased with him. Wyatt is playing with his friends He shows his teeth and growls. This typically follows Wyatt using words that others don t understand. He s frustrated with his friends and feels they aren t listening. Remind Wyatt s friends that he is trying to talk to them. Ask Wyatt to try again and be patient with his friends. Anytime Wyatt gets aggressive (hits, kicks, pinches) with his friends or their toys Wyatt is frustrated that his friends don t understand what he s trying to share. Help him talk to his friends, encourage him to apologize, and ask him if he needs some time alone to relax and take care of his body TLCPCP

37 Communication Chart Things we keep in mind: Remember that all behavior is meaningful and we can often learn what is important to and for someone from communications that might be seen as negative. The temptation can be to try and 'fix' behavior rather than listen to it.

42 My 15-year-old daughter has often had to take a back seat. She lets us know the issues affecting her and we work together on what we can do to rectify this. We ve always talked about things but having something to refer to (What s Working / Not Working) has certainly lessened the arguments and stress in the family.

43 By using the What s Working and Not Working I have been able to have better conversations with my son s teachers. It has helped me to frame the things that are not working as a request for a solution that I want to work with professionals to find.

44 What s Working and What s Not Working Things we keep in mind: In the resulting discussion, always start with where there's agreement and common ground Honestly capture people's perspectives, even if you disagree with them It s important to be unconditionally constructive. No matter how tempted you are to take sides, you don't.

46 Person Centered Thinking is Based around a person with the person at the center of the planning process. A commitment to achieve what is important to the person now and in the future. An ongoing process of listening, learning and action that helps a person to have positive control over the life that they choose.
