1 Spiritual Wellness Assessment Welcome to the spiritual wellness assessment, where you will gain a deeper understanding and awareness of what makes you well in this dimension of your life. On completion of this assessment you will know your spiritual wellness quotient. This exercise will help you gain a better understanding of yourself and your values and explore what motivates and propels your choices. Spiritual Wellness Dimension As you explore this dimension, please ponder the following questions: Who am you? What are your values? Do you live with integrity? How well is your spirit? How well are others experiencing you? The spiritual dimension of your life is essential to the health and wellness of your human biology. When others treat us well we feel important, respected and valued, when we are treated poorly, we feel devalued and worthless. When we are well, we are better able to treat others well, which results in mutual wellness. Value Alignment This dimension has a number of areas that are essential to value formation and alignment. The wellness of your Spiritual Dimension will reflect your level of alignment with your values and identify what motivates you. On completion of this questionnaire, you will know your spiritual wellness quotient and know the 1

2 areas where you need to do some work to develop a higher level of spiritual wellness. Assessment Profile Please review your personal details and make changes if they are incorrect, then proceed to the spiritual wellness assessment questionnaire. The key life areas in three categories to help you determine your wellness quotient. Use the five questions below to assess how well you are living in the following key life areas in this dimension. Ask yourself any of the following five questions to help rate yourself: 1. How well am I doing in this key life area? 2. How well am I developing in this key life area? 3. How are those in my personal life experiencing me in this key life area? 4. How well is this key life area in my business or professional life? 5. How do I feel about my wellness in this key life area? Rate your answers on a scale of 1 to 10 with one being not well and 10 being, excellently well. 2

5 Spiritual Wellness Assessment Analysis About the Spiritual Wellness Dimension The spiritual dimension forms the foundation for developing the other wellness dimensions. Your profile score in this dimension will reflect your level of consciousness and value alignment. It will show how well you are connecting your outer physical world with your inner self. This wellness dimension is the place that connects you with your energy field vibrations and draws you inside to explore the inner world such as your thoughts, beliefs and integrity. This is where you visualize and envision the true you. This dimension comprises many key life areas that make up the core of who you are and connects you with the Divine, God or Higher Power. It is where your beliefs come from. This dimension includes your hope, sense of integrity, self-esteem and personal style. It is from your Spiritual Wellness dimension that you develop your core values and a sense of balance. These key life areas blend to create your energy field or the social vibrations you emanate. This dimension determines what you resonate and how others feel and experience you, also referred to as your personal style. Spiritual Wellness Competency Integrity in Value When we are clear on what our values are we are better able to align our choices and actions to reflect those values and live with integrity. This then allows us to experience a purposeful reality. When our actions represent false values we are out of balance and integrity, thus it affects the core of who we are. Even if we are not consciously aware of it, on some level we know when our actions are out of alignment with our values.

6 The more closely you live in alignment with your true values, the greater the likelihood of you living with integrity and balance. This will result in a higher level of self-esteem and sense of well-being and belonging. The values you hold will determine how well you experience your life and also how others experience their relationships with you. What are Values? Values make up the tapestry of our lives. Human values expand and multiply with use. They shape our world view, the way we feel about ourselves and often shape our personal style. Our values come from our beliefs, culture, religion, faith, hopes and social interactions. The values you hold will determine the choices you make and the lifestyle you lead. Our values will determine our personal integrity and our choice to live a balanced, purposeful life. Your wellness profile is a reflection of the scores you have assigned to the key areas you choose to assess and will fall within the range below. These categories are just one way of classifying these key life areas and can be categorized differently in other spheres. They are intended to give you a way to get more meaning and depth from the key life areas in this dimension and how they relate to you in every aspect of your life and relationship. Categories 3

9 Your Key Life Scores The chart below shows the key life areas you have assessed and the scores you ve assigned yourself in each category. It is imperative that you remember to ensure that you are caring for yourself in all of the key life areas to maintain your overall wellness. Relationship with Self Your relationship with self is exemplified in the way you carry and express yourself. It is reflected in your self-esteem and personal style. These key life areas allow you to keep connected to the essence of who you are and with integrity with self; allowing you to be your best self. 10 Your relationship with others is in direct relationship with your relationship with yourself and your relationship with God. This where you come to realize that others are an extension of yourself. It is where you learn to begin to trust yourself and practice integrity in your relationships. Relationship with Others RELATIONSHIP WITH OTHERS Culture Vision Balance Trust Relationship with God Your relationship with God is reflected in your religion or spiritual activities or rituals. Your beliefs, practice, faith and sense of purpose will exemplify your spiritual development. It will serve to motivate your actions, your choice to live with integrity and your purpose in life. It will also impact your relationship with yourself and with others. 6

Your scores indicate that you are living optimally, your values are aligned and you are living with integrity. It also shows that you have a good relationship with yourself, with others and with God. This is critical for inner peace and self-satisfaction. It also serves to boost your self-esteem and sense of self as well as your comfort level with others. You are at optimal wellness in this dimension for the areas you have assessed. You are receiving great energy from the level of value alignment you ve strived to maintain. You are ready to experience the next stage of business and life success. Remember: in order to maintain balance and optimal wellness in this dimension your values, goals and purpose must remain aligned. Pay attention to your action and assess your values often to ensure they remain in alignment with your daily choices and feed your purpose, sense of integrity and trust in yourself. 7

12 Spiritual Wellness Improvement Plan Now is time to create your spiritual wellness improvement goals. Goals are things you plan on attaining or doing with intention and purpose. Your profile analysis report provides you with a view of where you are going and what you want to do. Now you should take some time to set some concrete goals for yourself in the areas you want to improve.
