3 Evaluation of the Bilingual healthy Choices Program, Evaluation Team Member November Present (Project Funded April 1, Present) Sixteenth Street Community Health Center s (SSCHC) Bilingual Healthy Choices Program strives to empower participants to adopt a more active lifestyle and healthier eating patterns. The program assists individuals and families in building capacity for making health-minded decisions for the incorporation of more physical activity in their daily lives and selecting more healthful food without increasing their grocery budget. Evaluation of the Bilingual Healthy Choices Program seeks to (1) strengthen the evaluation of the Healthy Choices curriculum through a collaborative approach that both enhances short-term outcome evaluation and adds medium-term outcome evaluation and (2) use this evaluation to build an evidencebased, family educational model and curriculum for an obesity intervention that can be replicated locally and nationally. The evaluation includes measurement of change in knowledge, attitudes, and behaviors, as well as biometric measurements. Community Partner: Sixteenth Street Community Health Centers PI: Kaija Zusevics, PhD, MPH, CHES Funded by the Wisconsin Partnership Program Dementia Wellness Project for Underserved African American Elders, Evaluation Team Member April 2015-Present (Project funded April 1, Present) This Community-Academic Partnership program seeks to improve the health and well-being of underserved urban elders and their families by addressing three of the four health factors presented in the What Works for Wisconsin s Health population health model, including health behaviors (diet, exercise, stress management, smoking, and medication adherence), access to care (medical homebased coordinated care by Milwaukee Health Services, Inc.), and social and economic factors (culturally tailored education and family support). This large implementation grant expands and extends the activities of the Wisconsin Partnership Program funded Milwaukee Health Services, Inc. dementia diagnostic clinic developed in consultation with the Wisconsin Alzheimer s Institute. The existing partnership has been expanded to include the Center for Urban Population Health to further support the development and evaluation of the community health education and screening program. Community Partner: Milwaukee Health Services, Inc. Academic Partner: Mark Sager, MD Lead Evaluator: Michelle Corbett, MPH, CHES Funded by the Wisconsin Partnership Program Wisconsin s Collaborative Approach to Increase Colorectal Cancer Screening, Evaluation Team Member June Present (Project funded June 30, Present) This project s purpose is to increase colorectal cancer screening rates among participating Federally Qualified Health Centers patient populations aged years by implementing evidence-based interventions and other strategies. This partnership between the Wisconsin Comprehensive Cancer Control Program, the American Cancer Society, the Center for Urban Population Health, and Milwaukee area Federally Qualified Health Centers is implementing the following interventions to increase colorectal screening rates: Provider Assessment, Feedback, and Professional Development Training; Provider Reminders; Patient Reminders and Education; and Patient Navigation. Grantee: Wisconsin Comprehensive Cancer Control Program through the UW-Madison Carbone Cancer Center PI: Noelle LoConte, MD Funded by the Centers for Disease Control and Prevention 3

4 PAST PROJECTS Project LAUNCH (Linking Actions for Unmet Needs in Children s Health)- Wisconsin, Evaluation Team Member, September September 2015 (Project funded in Wisconsin October 2009-September 2015) Wisconsin Project LAUNCH primarily focuses on children ages 0 to 8 and their families in 12 high-risk ZIP codes in Milwaukee. In partnership with the state of Wisconsin s Department of Health Services and Department of Children and Families, the local Project LAUNCH program at the City of Milwaukee Health Department coordinates a comprehensive early childhood system of care to ensure that Milwaukee families have access to programs that foster wellness in all areas of a young child s life. The project focuses on connecting services and systems. Project roles: data collection from community partners, qualitative and quantitative data analysis, and reporting to national, state, and local partners. Funded by the Substance Abuse and Mental Health Services Administration. Normalizing Breastfeeding: Building Social Support and Community Capacity, Evaluation Team Member, July June 2015 This initiative aims to increase breastfeeding initiation, duration, and exclusivity rates by engaging pregnant women, expectant fathers, and their families through health promotion, health education, and social support programs.
