Page 2 Dear Physician Partner, As Chief Medical Officer and President of Citrus Valley Physicians Group (CVPG), I would like to welcome you to our network of health care providers. CVPG was established in 1992 to service the San Gabriel and Pomona Valley. Over the past 20 years, our organization has developed a proven track record in managed care and provider service. We are currently expanding in other geographical areas that include the greater San Fernando Valley, Long Beach and Glendale in preparation for upcoming changes resulting from health care reform. We are committed in assisting you in navigating through the changes occurring in the environment such as the Cal Medi-Connect (Duals Demonstration Pilot) and the Covered California (Exchange) program. Our team of experts are ready to work with your practice to develop retention strategies to anchor your existing patient base and to capture new market share through the various health care program options available through CVPG. 13 Page 13 EZ Net User Manual New User Registration: From the home screen, click on New User Registration, fill out the registration form and submit. An will automatically be sent to both the user and our IT Departent. The new user will be required to verify the account through the link sent to their . IT will then enable the username/ password required for the User to login. Login: Enter username and password and click Login. You will be prompted with a dialog box to confirm address. Forgot Password: Click on forgot username/password, fill out the requested information and click on Next. A confirmation will be sent to your address. EZ Net Home Page - Authorizations: Look Up Authorizations: On the main menu tab over to Auths/Referrals and select Inquiry. Submit an Authorization: Fill out the bold items to submit the authorization. Document Management: Click on the File icon on the top-right corner of the Authorization Submission screen. Click on Add New Document. Browse and select the files to attach. Complete the Reference ID and a Description before clicking Upload. Click upload; the document management icon will not have a red check indicating that files are attached. Citrus Valley Physicians Group is happy to provide Urgent Care Centers for patients to access emergent care after normal business hours. Save time and money as your wait time is usually much shorter and your co-pay less than an E.R. visit.
