Grounded in that motto and impelled by our fundamental mission to improve the quality, effectiveness, and efficiency of cancer care so patients can live better lives, our alliance of Member Institutions and our staff members challenge themselves each and every day to make the NCCN Clinical Practice Guidelines in Oncology (NCCN Guidelines ) and the care recommended therein more accessible. 4 Through expert clinical recommendations, continuing education programs, global outreach, research initiatives, policy work, and our multitude of multistakeholder collaborations, NCCN has set the standard of cancer care for nearly a quarter century. Our legacy speaks for itself. In October 2017, we hit a milestone, onemillion registered users of the NCCN Guidelines! Moreover, the NCCN Guidelines were downloaded more than eight million times in 2017 a 16% increase over 2016 and a 25% increase across two years. It s the human story behind these numbers that really counts. Today, there are more than 15.5 million cancer survivors living in the United States alone. By providing free access to the recommendations in the NCCN Guidelines to people around the globe, our vision is to see that the number of cancer survivors keeps growing worldwide. NCCN s efforts to increase accessibility also include 142 translations of the NCCN Guidelines, 24 regional adaptations, as well as resource-stratified guidelines tailored to low- and mid- resource regions throughout the world. NCCN Broadening the access to appropriate treatment and supportive care, together with the Africa Cancer Coalition, American Cancer Society, and Clinton Health Access Initiative, we launched the NCCN Harmonized Guidelines for Sub-Saharan Africa in November. These Guidelines represent both the optimal care these countries aspire to provide and pragmatic approaches outlining effective treatment options for resourceconstrained settings. 4

5 Indeed, patients are paramount to our mission. Complementing the eight million downloads of NCCN Guidelines in 2017, NCCN, with support from the NCCN Foundation published 24 new and updated NCCN Guidelines for Patients, bringing the total library to 39. More than 395,000 people accessed these patient-friendly translations of our clinical guidelines in 2017 with more than eight million page views. From the more than 500,000 registered users in the United States to the five in Togo, we are honored to play a part in upholding NCCN s mission to improve the quality, effectiveness, and efficiency of cancer care so that patients can live better lives. 8 GUIDELINES WE LIVE BY Paramount to ensuring patient access to quality care is empowering patients and providers with evidence-based information for use in the decision-making process. Our mission to improve the quality, effectiveness, and efficiency of cancer care so that patients can live better lives can only be realized through unwavering dedication to providing all oncology stakeholders with the utmost reliable and upto-date treatment information. NCCN makes this possible through the publication and upkeep of the library of NCCN Guidelines and derivative products. First developed by visionary leaders in 1995 for eight cancer types, NCCN Guidelines provide sequential management decisions and interventions for the treatment of 97% of cancers affecting patients in the United States. Now, as before, the goal of these Guidelines is to ensure that all patients have access to the optimal outcomes for their disease. Today, we offer 68 NCCN Guidelines documenting evidencebased treatment interventions and management, as well as Guidelines for detection, prevention, and risk reduction; supportive care; and age-related recommendations. Developed by more than 1,200 NCCN Member Institution volunteers seated on 53 panels, each NCCN Guidelines document is updated at least annually to reflect the latest advances in treatment. In 2017, we reached new records, with as many as nine interim updates to NCCN Guidelines for certain diseases. Our panel members are multidisciplinary, disease-specific sub-specialists who are both clinicians and researchers, as well as patients and patient advocates. Grounded in our mission, NCCN develops and maintains a host of derivative resources of the NCCN Guidelines. Products like the library of NCCN Compendia, NCCN Guidelines with NCCN Evidence Blocks tm, and the NCCN Chemotherapy Order Templates (NCCN Templates ) aid in decision-making. Moreover, throughout the year, NCCN offers in-person and online education programs, delivering knowledge of the latest advancements in cancer care to nearly 14,000 learners. Through efforts of the NCCN Foundation, patients are able to access adapted, patient-friendly versions of NCCN Guidelines recommendations, empowering them to make informed decisions about their treatment plans. 11 ACCESS to life-saving treatment guidelines By compiling every recommendation for radiation therapy in one place, we ve made it significantly easier for radiation oncologists to stay upto-date on the very latest recommendations, regardless of how many different cancer types they treat. - Robert W. Carlson, MD, Chief Executive Officer, NCCN A complete compendium to aid in cancer care Nearly two-thirds of patients with cancer receive radiation therapy one of the most common treatments for cancer in the United States. To provide a definitive source for radiation therapy recommendations, NCCN announced the availability of the NCCN Radiation Therapy Compendium in early This compendium is part of NCCN's ongoing effort to always provide the most pertinent data on emerging treatment practices in the clearest, most efficient way possible. Hosting a single access point for all radiation therapy recommendations within the library of NCCN Guidelines, the newest addition to the our Library of Compendia was completed in October 2017 just seven months from the publication of the first chapters. Today, 42 Guidelines with radiation therapy recommendations are featured within the NCCN Radiation Therapy Compendium, providing radiation oncologists with the specific, cutting-edge information they need, without requiring them to sift through any extraneous information. 12 Forward progress in the face of cancer The library of NCCN Guidelines cover more than 97% percent of cases in the United States, yet we remain steadfast in our mission to ensure that patients have access to the best care for their disease type, no matter how complicated or rare. In 2017, NCCN published new NCCN Guidelines for AIDS-Related Kaposi Sarcoma, bringing the total library to 68 Guidelines. The treatment recommendations for AIDS-Related Kaposi Sarcoma were derived from one of the eight original NCCN Harmonized Guidelines for Sub-Saharan Africa. Subsequent to the growth of the NCCN Guidelines library, derivative products update throughout the year to reflect the changes in the Guidelines algorithms. In addition to the complete NCCN Radiation Therapy Compendium, in 2017 NCCN maintained 40 out of 48 possible NCCN Guidelines with NCCN Evidence Blocks, more than 3,200 NCCN Compendium Listings, 1,170 NCCN Templates, and 1,170 NCCN Imaging AUC entries for 51 cancer types. NCCN One in a million Of significance in 2017, was achieving the milestone of one million registered users of the NCCN Guidelines on NCCN.org. These users downloaded the NCCN Guidelines more than eight million times from NCCN.org and the Virtual Library of NCCN Guidelines mobile app a 16% increase over 2016 and a two-year increase of 25%. 12

13 NCCN GUIDELINES DOWNLOADS IN 2017 (MILLIONS) NCCN In 2017, NCCN Guidelines were downloaded by more than 2.6 million unique users. This increase speaks to the growing influence of and demand for NCCN Content worldwide. Defining optimal care with Catgories of Preference In the 2017 NCCN Guidelines User Survey, 85% of U.S. respondents said they would prefer to have optimal treatments outlined in the Guidelines. In September, NCCN began publishing Categories of Preference for drugs and biologics within the Guidelines, beginning with Multiple Myeloma. While all recommendations within the Guidelines are considered appropriate, Categories of Preference distinguish between preferred interventions, other recommended interventions, and those useful in certain circumstances. 13

14 The goal of the Categories is to stratify Guidelines to clarify panel and institutional preferences for interventions, to provide guidance to users of the Guidelines on which recommendations are considered optimal, and to continue to provide a wide range of recommendations to meet varying clinical circumstances and patient preference. To view Dr. Carlson, visit NCCN.org/annualreport A 360 view of cancer care Launched during the 2016 Annual Conference, JNCCN360 is a family of tumor-specific microsites providing users with carefully curated content tailored specifically to NCCN s audience. JNCCN360 is the latest resource from JNCCN Journal of the National Comprehensive Cancer Network. In 2017, JNCCN360 microsites were available for Multiple Myeloma and Breast, Kidney, and Non-Small Cell Lung Cancers. JNCCN is a peer-reviewed, Pub-Med indexed journal with a circulation of more than 25,000, featuring original health services research from investigators from NCCN Member Institutions and around the world, as well as insights on NCCN Guidelines updates, and insightful commentary from leading experts in oncology. 17 ACCESS to ground-breaking research By highlighting technological advances and multidisciplinary collaboration, we hope to encourage the widespread adoption of best practices for reducing costs and improving quality in breast cancer patient care. - Susan Most, RN, MBA, Director of Clinical Operations, NCCN ORP Funding quality improvement projects in breast cancer In November 2017, the NCCN Oncology Research Program (ORP), in collaboration with Pfizer, funded nine projects representing four NCCN Member Institutions and other cancer centers throughout the country for projects that use clinical care pathways to implement quality improvement initiatives along the continuum of care for underserved patients with breast cancer. A unique collaboration between ORP and Pfizer Global Medical Grants, the project sought proposals from both NCCN Member Institutions, as well as other academic and community hospitals throughout the United States. More than 90 letters of intent were received. NCCN In 2017, Elizabeth Gerstner, MD, of Massachusetts General Hospital Cancer Center, dosed the first patient in a clinical trial funded through ORP to study the effectiveness of bavituximab in patients with glioblastoma. Annual poster session continues to grow In 2017, the General Poster Session held during the NCCN Annual Conference had yet another year of record participation; 85 presenters, including six NCCN Young Investigator Awardees from The 2017 studies included research on the following topics: 19

NCCN Sharing resources among Member Institutions NCCN ORP promotes collaborative research among NCCN Member Institutions in various ways, including the management of the NCCN Shared Resources Database. Scientists and investigators at 18 NCCN participating cancer centers access the database and the resources therein that will improve the ability to conduct meaningful research. Through the database, investigators share more than 230 unique assets, such as complex technologies, instrumentation facilities, human tissue specimens, animal models, and specialized databases. 20

23 Providing ACCESS to recommendations at point of care Collaborations to ensure access Paramount to ensuring that patients have access to high-quality cancer care is providing access to up-to-date cancer treatment recommendations for all stakeholders in oncology. NCCN To that end, NCCN upholds long-standing relationships and continues to build new collaborations with some of the most influential health information technology (HIT) vendors around the world. NCCN encourages and applauds the inclusion of NCCN Guidelines and derivative content in HIT and decision-support tools in order to not only improve care for patients, but also to provide for quality assessment initiatives. In 2017, NCCN embarked on several new and expanded collaborations. Among these are new collaborations with Evinance Innovation, Inc., 23

In continuation of its support of the U.S. Department of Health and Human Services call for standardization of cancer care information across all electronic platforms, NCCN, a Centers for Medicare & Medicaid Services (CMS) recognized provider-led entity for development of imaging AUC, continues to work multiple organizations for inclusion of the NCCN Imaging AUC tm in HIT, such as with: AIM Speciality Health, Intermountain Insights, Inc., evicore Healthcare, McKesson Speciality Health, MedCurrent, National Decision Support Company, and New Century Health. Distribution of NCCN Content NCCN NCCN Guidelines are the recognized standard for clinical policy in cancer care and are the most thorough and frequently updated clinical practice guidelines available in any area of medicine. In addition to the inclusion of NCCN Content in HIT, NCCN recognizes and appreciates the significance of ensuring that providers are able to access Guidelines recommendations in whatever format they use. 24

25 To do so, NCCN works with outside organizations for dissemination of NCCN Content through print copies of the NCCN Guidelines, NCCN Pocket Guidelines, NCCN Flash Cards of select content from the NCCN Guidelines and Compendia, and sponsorship of the NCCN Virtual Library of NCCN Guidelines Mobile Application. Through such collaborations, in 2017, 82,000 sponsored versions of the Guidelines were distributed. NCCN Corporate Council The NCCN Corporate Council is an important resource for the development and expansion of NCCN programs and resources. NCCN offers membership in the NCCN Corporate Council to significantly enhance collaborative efforts between pharmaceutical, biotechnology, and diagnostic companies and NCCN, and thus better serve patients. NCCN In 2017, NCCN hosted two NCCN Corporate Council meetings that engaged representatives from industry in order to deliberate patient and provider needs and identify avenues of support for such initiatives. Patient safety we ve got that in the bag In continuance of the international campaign for patient safety, the NCCN Just Bag It! campaign moved forward in 2017, bringing to light the importance of delivering vincristine in IV mini-bag to eliminate the risk of a fatal medical error. In August 2017, NCCN announced it reached its goal of 25

26 100 cancer centers and hospitals signing on to the campaign and adopting policies in line with the NCCN Guidelines. MiKaela Olsen, MS, APRN- CNS, AOCNS, The Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins Moreover, MiKaela Olsen, MS, APRN-CNS, AOCNS, an oncology nurse at The Sidney Kimmel at Johns Hopkins, one of the NCCN Member Institutions, spearheaded a center-wide effort to administer vincristine via mini IV-drip bags, the results of which she presented at the Oncology Nursing Society Annual Congress in May. After successful implementation in pediatric oncology at Johns Hopkins Hospital, the change was put into effect in adult oncology following thoughtful design of the step-by-step procedure, policy revisions, and collaboration between nursing and pharmacy. Our staff feel confident that this new procedure is safe and that it is absolutely the right thing to do to prevent patient harm. Once we made the change, we did not look back. Eliminating the risk of harm was our number one priority, said MiKaela Olsen, MS, APRN-CNS, AOCNS, The Sidney Kimmel at Johns Hopkins on implementing the NCCN Just Bag It! campaign. 29 Ensuring ACCESS to appropriate care around the globe Taking a stand against poor outcomes in Africa On November 8, NCCN debuted its new NCCN Harmonized Guidelines for Sub-Saharan Africa the outcome of a much-anticipated and groundbreaking collaboration between the Africa Cancer Coalition, American Cancer Society, Clinton Health Access Initiative, IBM, and NCCN. 30 It was a gratifying and inspiring experience to collaborate with, and learn from, our Sub- Saharan colleagues, who do hands-on work solving problems for patients every day. Working together, we were able to distill the most important elements of care management, and prioritize treatments that provide effective outcomes even when resources are limited. - Wui-Jin Koh, MD, Seattle Cancer Care Alliance In many ways, this is an initiative modeled after the success of past HIV efforts, and also necessary because of them. As life expectancy increases in the area, more and more people are being diagnosed with various cancers. This project also inspired the creation of brand new NCCN Guidelines for AIDS-related Kaposi sarcoma, which accounts for 12% of cancer diagnoses for people living with HIV in the United States, but occurs much more frequently in Sub-Saharan Africa. NCCN continues to work with oncology specialists around the world to identify the need for harmonized guidelines throughout other low-resource areas. Providing a forum for global deliberation NCCN hosted a Global Patient Advocacy Engagement Roundtable before ESMO 2017 in Madrid, Spain. This multi-stakeholder Global Roundtable assembled patient advocates, physicians, and policy experts to discuss best practices in global patient advocacy efforts to ensure that patients are involved at all levels of health care decision-making. Moderated by Robert W. Carlson, MD, the panel discussed variations in patient advocacy efforts and patient access to quality cancer care. NCCN In October, in conjunction with the NCCN 12 th Annual Congress: Hematologic Malignancies tm in San Francisco, California, NCCN held its inaugural NCCN Global Academy for Excellence & Leadership in Oncology tm : School of Pharmaceutical & Biotech Business. In 2017, NCCN hosted its annual Global Corporate Council meeting in conjunction with the NCCN Annual Conference in Orlando. Growing global influence More than 46% of the one million registered users of NCCN Guidelines on NCCN.org report that they live outside the United States. Indeed, the influence of NCCN Content continued to gather momentum throughout all regions of the world, with more than 2.4 million Guidelines downloaded from NCCN.org (excluding the mobile app) from outside of the States in NCCN Dr. Carlson moderates NCCN Global Academy Roundtable featuring experts involved in development of NCCN Guidelines and their global adaptations. To ensure that providers and patients around the world have access to the most up-to-date standards in cancer care, NCCN is committed to maintaining and growing a library of resource stratified guidelines, global adaptations, harmonizations, and translations of NCCN Guidelines. NCCN published seven new and updated NCCN Framework. Originally launched in late 2015, these tools outline the most optimal treatment options for patients with cancer, based on four levels of available resources. In 2017, the library of NCCN Framework grew to 16, covering 75% of global cancer cases. 31

The word survivor may imply that treatment has concluded, but for many cancers, survivors actually need ongoing treatment for the rest of their lives. Some people find the word too passive. They may, understandably, prefer to call themselves warriors. - Yalek Biru, the event s keynote speaker who was diagnosed with Multiple Myeloma 22 years ago. Exploring disparities in survivorship Living through and beyond a cancer diagnosis has profound effects on a person s well-being physically, psychologically, socially, economically, and spiritually. On December 1, 2017, NCCN convened various stakeholders to explore the significant and unique challenges facing survivors today, as well as those who care them particularly in a time of such uncertainty in health care policy. The Annual Patient Advocacy Summit featured two roundtable panels, as well as informational presentations by national patient advocates, featuring resources available to patients today. Of particular note was the topic of disparities, specifically regarding the different needs of urban and rural populations and people across socioeconomic statuses. When it comes to racial minorities, panelists were concerned about gaps in data, and agreed more outreach and study is needed for underserved communities. The panelists also discussed the impact of aging baby boomers and the growing survivorship population. NCCN 37

38 It s very important to determine who handles the ongoing coordination of care for survivors. Care management generally shifts to primary care physicians, but it needs to be done in an integrated way so that survivors have all of their health care needs addressed, said Crystal Denlinger, MD, FACP, Chair of the NCCN Guidelines Panel for Survivorship. 91 Empowering patients through ACCESS to comprehensive resources Patient and caregiver resources The NCCN Foundation, founded by NCCN in 2010 to empower people living with cancer and advance oncology innovation, supports the publication of a rapidly expanding library of patient education resources and funds promising young investigators at the forefront of innovation in cancer research. The library of NCCN Guidelines for Patients and NCCN Quick Guide sheets saw growth in 2017, with the publication of 24 new and updated patient resources.
