In September 2010, North Ohio Heart, Inc. and its primary care division, Ohio Medical Group, joined services with EMH Regional Healthcare System under the new entity North Ohio Heart, Inc. 6 Scheduling Your First Appointment You may request an appointment online or by calling the office of that location and pressing option 3. When scheduling an appointment, please inform the medical secretary of the nature of your problem so we can set aside the proper amount of time for your visit. We will ask that you confirm your address, phone number and insurance information at each visit. Please bring your insurance card with you. If you are unable to provide insurance information, you will be considered self-pay and will be responsible for payment. Cancellations/No shows: If you must cancel an appointment, we ask that you notify us at least 24 hours in advance. This allows our physician to offer appointments to other patients in need of care. If you miss your appointment without giving adequate notice, you will be charged a $25.00 no show fee for that appointment. If you fail to keep three appointments without notifying us, you will be asked to seek your medical care at another facility. Boy, when they tell you an appointment time they mean it! I never have to wait even a minute! - OMG Office Patient Insurance & Billing: Your insurance coverage is a contract between you and your insurance carrier, not between your insurance carrier and North Ohio Heart/Ohio Medical Group. We cannot waive co-pays, deductibles and uncovered services as they are requirements placed on you by your insurance company. We are also unable to change the visit types based on your coverage. If you have any questions about your coverage or what is covered, you should contact your carrier at the customer service number listed on your card. Because there are many insurance carriers with many plans and individual requirements, it is the patient s responsibility to know and understand their own plan. We will bill your insurance company for you, but ultimately, you are financially responsible for any costs incurred in your office visit. Collection of your co-pay at the time of your service is a requirement of your insurance contract. Balances on your account will also be collected at your visit.

7 How to Get the MOST Out of Your Office Visit We want the time you spend with your doctor to be as meaningful as possible. To get the most out of your visit we suggest you plan and prepare in a variety of ways. Here are a few ways you can ensure your office visit is more efficient: Be Prepared Before you visit, prepare a list of questions and concerns to discuss with your physician. Bring your medications with you, in their bottles. It is very important to provide your physician with an accurate account of what medicine you are taking. You should also have a written medical history, including recent diagnostic tests, lab results, chest x-rays, or EKGs and records from your referring physician. It is important to bring your insurance card (or spouse s card if necessary) along with you and a referral from your primary care physician. Educate Yourself Become better informed about your health to make your time with the doctor more effective. The more you know, the better we can all work together. There is literature in our offices about many conditions. If you do not see what you need, inquire at the desk and the staff can assist you. In addition, visit for important health information for you as well as links to other resources. I feel like I am part of the OMG family! - Middleburg Heights Patient Write Things Down Sometimes a lot of information is given during a visit and it can be difficult to remember everything said. Be prepared to take notes or bring a family member with you who will write it down for you. You can also write down other questions you want answered before you leave. Our clinical staff is happy to give you the information you need. Explain Your Concerns Be prepared to answer your doctor when he asks, Why are you here? Start with what you think is the most important then discuss your top two or three concerns and what assistance you are seeking.
