2 CAROLYN M. D AMBROSIO, MD, MS, FCCP Carolyn D'Ambrosio, is the Director of the pulmonary and Critical Care Fellowship at Harvard-Brigham and Women s Hospital in Boston, past director of the Center for Sleep Medicine at Tufts-New England Medical Center in Boston, where she also served as Associate Program Director for the Internal Medicine Residency Program. She received her medical degree from George Washington School of Medicine, and completed postdoctoral training as a fellow in pulmonary and critical care medicine at Yale University School of Medicine. She was also Chief Resident for internal medicine at Strong Memorial Hospital in Rochester, New York. She currently holds a teaching appointment as Associate Professor of Medicine in the Pulmonary, Critical Care and Sleep Medicine Division at Tufts University School of Medicine, where she received the esteemed Award to Faculty for Notable Teaching. Her major research interests include the study of sleep apnea as a risk factor for perioperative morbidity and mortality, and the role of fragmented sleep on patients with acute respiratory failure and mitochondrial disease. In addition to her accomplishments in teaching, she has presented her work on sleep apnea at numerous national conferences. Her research has been featured in many peer-reviewed books and journal articles, and she is an editorial reviewer for journals including Intensive Care Medicine, CHEST, SLEEP, and the Journal of Clinical Sleep Medicine. She has served as Chair for the Sleep-Related Breathing Disorders Section of the American Academy of Sleep Medicine, and is an active member of professional societies including the Massachusetts Sleep Society and the American Thoracic Society.

3 KHALID ISMAIL, MD KHALID ISMAIL, MD Director, Sleep Medicine Fellowship Program; Co-Director, Mycobacterial Disease Clinic; Pulmonary Attending Physician; Assistant Professor, Tufts University School of Medicine. After Tinishing his pulmonary and critical fellowship at New York University (NYU), he joined the sleep fellowship program at Lahey clinic medical center. Dr. Ismail eventually joined the faculty at Tufts medical center back in 2009 as an assistant professor of medicine, became the program director for the sleep medicine fellowship, and recently the director for the Sleep Center at Tufts medical center. His areas of research interest include the interaction between sleep disordered breathing and pulmonary hypertension. FERNANDA ALMEIDA, DDS, PhD FERNANDA ALMEIDA, DDS, PhD Dr. Almeida received her Doctor of Dental Surgery degree from the University of Sao Paulo, Brazil, and her Master of Science from the Federal University of Sao Paulo, Brazil. She is an alumna of UBC, where she completed her Doctor of Philosophy (Dental Sciences). Dr. Almeida has been involved in oral appliance research and the treatment of patients with obstructive sleep apnea since 1996, and her research is focused on oral appliance side effects, compliance, titration modalities, imaging and treatment outcome. With other UBC Dentistry faculty, Almeida has designed an interdisciplinary program in dental sleep medicine for the clinical specialty graduate programs in orthodontics and pediatric dentistry.

4 GREG ESSICK, DDS, PhD Greg Essick, DDS, PhD Dr. Greg Essick is Professor in the Department of Prosthodontics and in the Center for Pain Research and Innovation at the University of North Carolina at Chapel Hill. He completed his D.D.S and his Ph.D. training in neurophysiology at the University and post-doctoral work at the Salk Institute, La Jolla, California. Dr. Essick s NIH-supported research includes studies of chronic orofacial pain and of trigeminal nerve injury. His recent research pertains to the associ- ations between obstructive sleep apnea and temporomandibular disorders (TMD), and periodontitis. He authors or coauthors more than 100 journal articles and book chapters. His faculty practice focuses on dental sleep medicine, which he is imple- menting into the curriculum at the UNC School of Dentistry. He provides the school s continuing education in sleep medicine to practicing dentists in North Carolina. ANTONIO ROMERO, DDS, PhD. Dr. Romero graduated from University of Valencia, Doctor cum laude PhD, Postgraduate Degree in Statistical Analysis from Health Sciences. University of Valencia, Diplomate by the American Board of Orofacial Pain. Member of the Research Committee of the American Academy of Dental Sleep Medicine, member Spanish Society of Craniomandibular Dysfunciton, Spanish Pain Society and Spanish Society of Dental Sleep Medicine, private practice exclusively limited to TMD, Orofacial Pain and Dental Sleep Medicine in Valencia (Spain). Dr. Antonio Romero has lectured widely on the topics of TMD, Bruxism, Orofacial Pain and Dental Sleep Medicine both in Spain and Internationally.

5 ORFEU BUXTON, PhD. Associate Professor Biobehavioral Health Penn State University, Assistant Professor Division of Sleep Medicine Harvard Medical School, Associate Neuroscientist Department of Medicine Brigham and Women's Hospital, Adjunct Associate Professor Department of Social and Behavioral Sciences Harvard School of Public Health, Faculty AfTiliate of Penn State s Population Research Institute (PRI), Member of the Graduate Faculty in Neuroscience. Dr. Buxton s research primarily focuses on 1) the causes of chronic sleep deticiency in the workplace, home, and society, and 2) the health consequences of chronic sleep deticiency, especially cardiometabolic outcomes, and the physiologic and social mechanisms by which these outcomes arise. Successful aging is a central focus of this work. Ongoing interdisciplinary human studies involve sleep loss, aging, and insomnia, as well as health disparities. Dr. Buxton serves on the Internal Advisory Board for the Harvard School of Public Health Center for Work, Health, and Wellbeing. Dr. Buxton is a member of the Work, Family, and Health Network, cochairs the Steering Committe, and leads the Biomarker and Actigraphy Data Coordinating Center (BADCC) for the Work, Family, and Health Study, among others. After earning a B.S. in Behavioral Neuroscience from the University of Pittsburgh and a stint as a small business owner, Dr. Buxton earned a doctoral degree in Neuroscience from Northwestern University, Evanston, Illinois, working with Professors Eve Van Cauter and Fred Turek. He obtained post-doctoral training in sleep and endocrinology with Dr. Van Cauter at the University of Chicago. Dr. Buxton co-founded the National Postdoctoral Association, a member-driven organization that provides a unique, national voice for postdoctoral scholars.

6 JOHN REMMERS, MD. Dr. Remmers is a pulmonologist & clinical professor of internal medicine at the University of Calgary with an active sleep practice in the sleep lab at the Foothills Medical Centre in Calgary. He is a recognized physiologist, physician, pioneer & innovator in sleep medicine, having invented products ranging from portable diagnostic sleep monitors to auto-titrating CPAP devices. A world renowned expert in the Tield of snoring & obstructive sleep apnea (OSA), being the Tirst researcher to elucidate the pathogenesis of sleep apnea & to demonstrate that sleep apnea is caused by an anatomical narrowing of the pharynx. Dr. Remmers is a well known respiratory physiologist, who contributes to the Tields of the neurobiology of respiratory rhythmogenesis, chemoreception, & the pathophysiology of the control of breathing. Published over one hundred peer reviewed publications in the area of respiratory physiology & served two terms as the Editor-in-Chief of the Journal of Applied Physiology. His recent research has been dedicated to oral appliance therapy (OAT) to address the needs of OSA patients who are dissatistied & non-compliant with CPAP. A key factor inhibiting the widespread use of OAT is the challenge of determining which patients will respond to this therapy. With the expertise of the R&D team at Zephyr Sleep Technologies, the revolutionary product MATRx was developed & released to market to allow sleep labs to perform remote-controlled oral appliance titration during polysomnography. The MATRx test identities favorable oral appliance candidates, determines an efticacious protrusive position & provides patients with a therapeutic alternative to CPAP. MATRx will allow sleep physicians & dentists to improve the standard of care for their OSA patients. He is an international speaker presenting honorary research lectures, symposia & poster presentations to the ATS, APS, ACCP, AASM, WASM, NVTS, AADSM, the Pankey Institute & Spear.

7 ALAN A. LOWE, DMD, PhD..Alan Arthur Lowe graduated with a DMD from The University of British Columbia in Vancouver and completed a Diploma in Orthodontics and a Ph.D. entitled "The Neural Control of Tongue Posture" at the University of Toronto. Currently, he is Professor and Chair, Division of Orthodontics in the Department of Oral Health Sciences and has maintained a private orthodontic specialty practice in Vancouver for 35 years. Dr. Lowe has lectured extensively both nationally and internationally in the areas of neurophysiology and orthodontics with a particular emphasis on the etiology and treatment of snoring and Obstructive Sleep Apnea. His research activities have been funded by both provincial and federal governments and he has published over 120 papers, 150 abstracts and 15 book chapters. In addition, he has successfully Tiled Canadian, U.S. and international patents for each of three inventions. Recently, the Canadian Institutes of Health Research awarded $1.4 million over Tive years to the UBC Respiratory Sleep Disorders Research Team of which Dr. Lowe is a coinvestigator. Dr. Lowe was one of the founding members of the American Academy of Dental Sleep Medicine and has served for many years as a member of their Board of Directors, a Research Advisor to the Board and Chair of their Award s Committee. Dr. Lowe has been the recipient of the American Academy of Dental Sleep Medicine Pierre Robin Academic Award, the College of Dental Surgeons of British Columbia Distinguished Service Award and the German Society of Dental Sleep Medicine Meirer-Ewert Award for Career Achievement. JONATHAN PARKER, DDS.Dr. Parker is a graduate of the University of Minnesota School of Dentistry. He has treated patients with snoring and sleep apnea for over 24 years and has had a full-time practice in dental sleep medicine in Minneapolis, Minnesota since He is a Diplomate of the American Board of Dental Sleep Medicine and the American Board of Orofacial Pain. He is a charter member of the American Academy of Dental Sleep Medicine. He was the recipient of the Academy s prestigious Distinguished Service Award in He is immediate past President of the American Board of Dental Sleep Medicine. Dr. Parker has authored a number of research articles and abstracts evaluating the effectiveness of oral appliance therapy for obstructive sleep apnea (OSA). Dr. Parker has lectured internationally on evaluation and treatment of snoring and OSA. He is well known for developing innovative techniques and protocols to improve patient care and treatment success in dental sleep medicine.

8 ROBERT ROGERS, DMD Dr. Rogers has had a special interest in the treatment of sleep-disordered breathing since 1990 and treats patients in conjunction with many regional sleep centers. Presently, he is President and Director of Clinical Services for Pittsburgh Dental Sleep Medicine, PC and limits his practice to dental sleep medicine. Dr. Rogers is the founding president of the American Academy of Dental Sleep Medicine (AADSM) and served again as president in 1995 and In addition to being a long-term member of the Board of Directors, he has participated in committee work on a consistent basis. Dr. Rogers is a Diplomate of the American Board of Dental Sleep Medicine and is the recipient of the AADSM Distinguished Service Award. Dr. Rogers was the author/editor of the original AADSM educational slide series and is a contributing author to the graduate dental text, Clark s Clinical Dentistry. He is currently the dental consultant to Philips-Respironics, Inc. Dr. Rogers was a member of the task force for the revision of the American Academy of Sleep Medicine Position Paper and Practice Parameters on Oral Appliance Therapy. He also co-authored the American Academy of Sleep Medicine Guidelines for the Evaluation, Management and Long-term Care of Adult Obstructive Sleep Apnea. In addition, he is a consultant for the National Institutes of Health regarding oral appliances as related to the treatment of sleep-disordered breathing. Dr. Rogers is a frequent speaker at the AADSM Annual Meetings and has presented lectures on oral appliance therapy to physicians, dentists and patient groups throughout the United States and Europe. MARYBETH ROGERS, B.S. Mary Beth Rogers graduated from the University of Pittsburgh with a Bachelor of Science degree in education and taught school in the Pittsburgh area for several years. After teaching she managed a busy dental practice for her husband, Rob Rogers for many years and is experienced in dental assisting, front oftice management, bookkeeping and accounting. In 1991, Mary Beth became the executive director of the newly formed Sleep Disorders Dental Society, a non-protit professional educational association of dentists and physicians. Under her direction, the Society came together to promote the legitimate acceptance of oral appliance therapy by the medical profession and expand the role of dentists in the practice of sleep medicine. The organization is presently one of the fastest growing associations in the sleep medicine Tield and offers board certitication to serious dental practitioners. Now known as the American Academy of Dental Sleep Medicine, Mary Beth was the Tirst recipient of the academy s Distinguished Service Award. Presently, she is the vice president of Pittsburgh Dental Sleep Medicine and manages all administrative services including insurance reimbursement and contracting. Mary Beth works closely with Medicare as a member of the Provider Outreach Education Advisory Group and consults with many insurance companies to help establish medical policy regarding oral appliances

9 JAYME MATCHINSKI, J.D. Ms. Matchinski concentrates her practice on health care and corporate law, with a particular focus in the area of sleep medicine. She assists physicians and health care systems with compliance, reimbursement, licensure and certitication issues, as well as the purchase, sale and formation of health care entities. Her clients include sleep disorder centers, durable medical equipment companies, rehabilitation hospitals and facilities, ambulatory surgery centers, long-term acute care hospitals, skilled nursing facilities, nursing homes and assisted living facilities. Previously, Ms. Matchinski served as the vice president of a national health care consulting Tirm. Ms. Matchinski is a member of the editorial advisory board of Sleep Diagnosis and Therapy Journal, and an advisory member of the board for the Sleep Center Management Institute in Atlanta, GA. She also serves on the board of Volunteer Optometric Services to Humanity Illinois Chapter and helps provide pro bono representation to individuals seeking political asylum through the Heartland Alliance for Human Needs & Human Rights. She earned her J.D. from Valparaiso University School of Law and holds a Bachelor of Science from Northern Michigan University. NOAH SIEGEL, MD Noah S. Siegel is board certitied in both Sleep Medicine and Otolaryngology Head and Neck Surgery (ENT). Dr. Siegel is the Medical Director of Otolaryngology for Massachusetts Eye and Ear s Longwood facility in Boston. He is a member of the Faculties at Harvard Medical School, and Tufts University School of Medicine. Dr. Siegel attended the University of Michigan in Ann Arbor for both his undergraduate and Medical School training where he graduated Magna Cum Laude in He completed his general surgery internship at Northwestern University in Chicago and then completed his residency at Harvard University in Otolaryngology- Head and Neck Surgery. Dr. Siegel has an active medical practice with primary interest in sleep disorders, allergy and pediatric otolaryngology. In addition, he regularly teaches courses and classes at national conferences and for academic medical/dental training programs.

10 NOSHIR R. MEHTA, DMD, MS. Dr. Mehta is the Senior Advisor for the Craniofacial Pain, Headache and Sleep Center and Associate Dean of Global Relations, at Tufts University School of Dental Medicine. He is a Diplomate of the American Board of Orofacial Pain, and American Board of Dental Sleep Medicine, Fellow of the International College of Dentists and Fellow of the Pierre Fauchard Academy. Dr. Mehta is author of numerous scientitic articles and chapters in major textbooks on orofacial pain, Dr. Mehta has and continues to lecture nationally and internationally on his areas of interest including dental occlusion, bruxism and dental sleep. LEOPOLDO P. CORREA, BDS, MS Dr. Correa is an Associate Professor, Division Head of Dental Sleep Medicine, and Director of Dental Sleep Medicine Fellowship Program at Tufts University School of Dental Medicine in Boston Massachusetts. He is diplomate of the American Board of Dental Sleep Medicine, received his Master Degree in orofacial pain and temporomandibular disorders at Tufts University where he has treated sleep apnea and TMD patients for over 15 years with his mentor Dr. Noshir R. Mehta. Dr. Correa teaches Dental Sleep Medicine into the pre and postgraduate programs at Tufts University, and also participate as speaker in national and international seminars.

11 ROBERT TALLEY, DDS. Dr. Talley manages a private practice that has been limited to conservative care through interdisciplinary management of patients with sleep disorders, TMD and craniofacial pain since Founding member and 2x past-president of the AACP, he is a Diplomate of the American Board of Dental Sleep Medicine, the American Board of Craniofacial Pain, and the American Board of Orofacial Pain. Author of the Dental Sleep Medicine chapter of Craniofacial Pain: A Handbook for Assessment, Diagnosis & Management and an AACP Institute faculty member, Dr. Talley is a graduate of the University of Missouri-Kansas City School of Dentistry (1971). JAMISON SPENCER, DMD, MS. Dr. Spencer is a Diplomate of the American Board of Craniofacial Pain and the American Board of Dental Sleep Medicine. Dr. Spencer is past President of the AACP and an AACP Institute faculty member. He completed a 3-year distance learning MS degree program through Tufts University. Dr. Spencer s private practice is limited to dental sleep medicine and craniofacial pain. Dr. Spencer lectures nationally and internationally on the topics of anatomy, craniofacial pain and dental sleep.
