1 Updated June 2, 2017 Case Number /31/2017 at 11:50 am Green Bay and Alden Arroyo, Adam A., male, age 26, 5544 S. Meade, Chicago, IL Driving While License Suspended, Possession of Cannabis $ /18/2017 at 3:00 pm Police on routine patrol stopped a silver Honda for speeding 46/30 on Green Bay Road. Officers stopped the vehicle and spoke to the driver, Arroyo, who it was later determined was driving on a suspended license. Arroyo admitted to officers there was cannabis located in the glove box and officers were able to locate the cannabis upon searching the vehicle. Arroyo was arrested, charged with Driving While License Suspended, Possession of Cannabis and speeding. He was processed, released on bond and given a July court date. Case Number /29/2017 at 10:50 pm Forest Park Beach Minor Consumption of Alcohol Juvenile, males, (4) Grayslake, Lake Forest & Lake Bluff Juvenile, females, (3) Lake Forest 06/14/2017 at 9:00 am Lake Forest Beach employees called 911 after observing eight to ten 16 year olds with alcohol under the North Pavilion at Forest Park Beach. Police met with the juveniles, who admitted to consuming alcohol and had alcohol in their possession. Officers issued administrative hearing tickets to all individuals with a June hearing date. Case Number /29/2017 at 5:00 pm Lake Forest Boat Ramp Theft $120 06/20/2017 at 1:30 pm Juvenile, male, Mundelein, IL Juvenile, male, Mundelein, IL Police responded to Forest Park after a report of two bikes taken from that location. Police spoke to the victims and beach guards who provided descriptions of the suspects and stolen bikes. Police eventually learned the offenders may be headed to Culvers in Libertyville. Lake Forest Police officers drove there and located the stolen bikes and two juvenile suspects. Both subjects were transported to the Lake Forest Police Department, their parents were notified of the incident and the two stolen bikes were recovered. Both juveniles were issued local ordinance violations for Theft, released to a parent and given June court dates.
2 Case Number /27/2017 at 11:57 pm Westleigh Road and Ridge Road Driving While License Suspended $1500 Cole, Justin K., male, age 28, 1700 Kayla Ln., Waukegan, IL /18/2017 at 3:00 pm Police on routine patrol conducted a traffic stop on a tan Chevrolet Impala after noting the registration plates were incorrect. Police spoke to the driver, Cole, and determined his driver s license was suspended at the time of the incident. Cole was arrested, processed and released on bond. He was given a July court date. Case Number /27/2017 at 12:45 am Rte. 60 and Ridge Road Driving While License Suspended, Operating an Uninsured Motor Vehicle Peterson, Sydnie D, female, age 23, 5038 S. Carpenter, Chicago, IL $2000 6/20/2017 at 3:00 pm Police on routine patrol conducted a traffic stop on a blue Nissan after officers observed the driver talking on a cell phone while driving. Police spoke with the driver, identified as Peterson, and determined her driver s license was suspended and she did not have insurance on her vehicle. Peterson was arrested, processed and released after posting bond. She was given a June court date. Updated May 26, 2017 Case Number /23/2017 at 12:57 am Rte. 60 and Old Bridge Road No Valid Driver s License $1500 Cardine, Niyah, L., female, age 19, 4326 W. Van Buren, Chicago, IL /18/2017 at 1:30 pm Police on routine patrol conducted a traffic stop on a Buick sedan for an equipment violation and when officers spoke to the driver, Cardine, they discovered she did not have a valid driver s license. Cardine was arrested for No Valid License, transported to the Lake Forest Police Department where she was processed, released on bond and given a July court date. Case Number /22/2017 at 5:57pm 700 blk. N. Western Ave, Lake Forest Aggravated Domestic Battery (Class 2 Felony) and $ /24/2017 at 9:00
3 Domestic Battery Bodily Harm Zacarias, Luis J, male, age 38, 770 N. Western Ave, Lake Forest, IL C 120 Waukegan Lake Forest Police investigated a reported incident of domestic battery in which Zacarias was suspected of causing bodily harm to the victim. After an investigation was completed, Lake Forest Officers consulted with the Lake County States Attorney s office and a warrant was obtained for Zacarias for Aggravated Domestic Battery (Class 2 Felony) and Domestic Battery Bodily Harm. Police located a vehicle being driven by Zacarias and after conducting a traffic stop was conducted, Zacarias was taken into custody on the active warrant and transported to Lake County Jail on the warrant. Case Number /21/2017 at 1:57 pm Rte. 41 and Westleigh Possession of a Stolen Vehicle, No Valid DL $1500 Juvenile, male, age 16, Waukegan, IL 07/18/2017 at 3:00 pm Police patrolling the area of Rte. 41 and Westleigh Road overheard an ISPERN broadcast of a stolen vehicle described as a silver Chevrolet Cruze being tracked by ONSTAR that was southbound on US Rte. 41 from Waukegan IL. Officers observed the vehicle in the ISPERN broadcast and with assistance from Highland Park PD, stopped the vehicle on Rte. 41. Police were able to speak with and detain the driver, who they determined to be a juvenile, and confirmed the vehicle was reported stolen from Waukegan. Officers arrested the juvenile, transported him to the Lake Forest Police Department where he was held pending the arrival of Waukegan Police. The juvenile was cited for No Valid DL by Lake Forest Police, given a July court date and released to Waukegan Police. Case Number /20/2017 at 1:50am Waukegan Rd. and Conway Road Zero Tolerance, Possession of Drug Paraphernalia $1200 6/20/2017 at 3:00pm Rubin, Miller S., male, age 19, 4808 Preserve Pkwy. Long Grove, IL Police on routine patrol conducted a traffic stop on a silver Volvo that was observed speeding on Waukegan Road at Conway Road. Police stopped the vehicle and spoke to the driver, identified as Miller Rubin. While talking with Rubin officers could smell an odor of an alcoholic beverage coming from inside the vehicle and they observed Rubin s eyes to be red and watery. Officer s asked Rubin how much he had to drink that night and he admitted, stating, maybe a beer. Officers requested Rubin submit to some field sobriety test which he agreed to. Subsequent to police completing their investigation, Rubin was arrested for Zero Tolerance. During a search of Rubin s vehicle officers located a small quantity of drug paraphernalia. Rubin was transported to the Public Safety Building, processed and released on bond. He was given a June court date.
4 Updated May 18, 2017 Case Number /16/2017 at 8:21 pm Forest Park Beach Minor Consumption of Alcohol, Possession of Drug Paraphernalia, Possession of Cannabis Forbes, Kelly, female, age 19, 1378 Eddy Ln., Lake Zurich, IL Schmitt, MacKenzie, female, age 19, Ravine Road, Lake Zurich, IL Walter, Shea, female, female, age Red Haw Lane, Lake Zurich, IL Castellani, Daniel, male, age 19, 820 Ravenswood Ct., Lake Zurich, IL /14/2017 at 9:00 am Officer s conducting a well being check for a female who was reportedly ill at the north end of the beach lot, located several subjects who had been drinking. One teen was transported by ambulance to LF Hospital. The remaining teens all issued tickets for Minor Consumption of Alcohol and one was also charged with Possession of Drug Paraphernalia and Possession of Cannabis. All were given June dates and released. Case Number /11/2017 at 9:43 am 1200 block of Green Bay Road Criminal Trespass to Motor Vehicle $1500 Williams, Gary M., male, age 18, 1637 Lewis Ave, North Chicago, IL Gordon, LaRoyce, male, age 19, 2011 Green Bay Road, North Chicago, IL /05/17 at 8:45am C 150 Waukegan Police determined in the early morning hours of 5/11/17; Lake Forest, Highland Park, and Lake Bluff sustained a multitude of car burglaries in addition to two stolen Mercedes Benz sedans. Lake Forest Police responded to a residence in the 1200 block of Green Bay Road after the resident found his vehicle missing from his driveway. Police determined the 2004 Mercedes Benz had been left unlocked in the driveway and the vehicle keys were inside the vehicle. Lake Forest Detectives were able to locate the stolen Mercedes in North Chicago and, with the help of Waukegan Police, detectives stopped the vehicle and arrested Williams and Gordon. Police consulted with the Lake County States Attorney s Office and subsequently both Williams and Gordon were charged with Criminal Trespass to Motor Vehicle, processed and released on bond pending a June court date. 5 Foley, Stephen X., male, age 19, 485 Holland Court, Lake Forest, IL Foley, Michael B., male, age 19, 335 N. Green Bay Road, Lake Forest, IL Avery, Charles F., male, age 19, 219 Gillick, Park Ridge, IL Police on routine patrol observed three teens after curfew hours walking eastbound Deerpath Rd. from Golf Ln. Due to three appearing under age, the recent vehicle burglaries in the area, police made contact with the individuals. While speaking with the teens, officers detected a strong odor of an alcoholic beverage emanating from each of the three individuals. When asked if they had been drinking, all three denied drinking alcohol but when requested to perform a test to verify this statement, all three refused. After confirming the age of all three boys, police charged all three with minor consumption of alcohol, issued each an administrative hearing citation and released each individual to a parent. Updated May 12, 2017 Case Number /10/2017 at 12:45 pm Rte. 60 and Suffolk Garcia, Luis C., male, age 26, th Ave. Zion, IL Illegal Transportation of Alcohol, Possession of Cannabis and Possession of Drug Paraphernalia $120 06/06/2017 at 3:00pm Officers on patrol conducted a routine traffic stop on a blue Dodge truck in the area of Rte. 60 and Suffolk after an officer observed the driver smoking cannabis from a glass pipe. Police stopped the vehicle and spoke to the driver, Garcia, and during the conversation police smelled the odor of marijuana coming from the vehicle and observed a small plastic bag containing cannabis in the vehicle near the center console. A glass pipe, still warm and containing cannabis, was recovered from the center console of the truck. Police also located an opened and partially full container of alcohol the driver also admitted to drinking. Garcia was cited for Illegal Transportation of Alcohol, Possession of Cannabis and Possession of Drug Paraphernalia. He was cited, released and given a June court date. Case Number /05/ block of McKinley Minor Consumption of Alcohol 06/14/2017 at 9:00 am Multiple juveniles, male and females, Lake Forest and Lake Bluff Police were dispatched to 1500 block of N. McKinley Rd. Lake Forest IL for a noise complaint at that residence. Upon my arrival, police observed two females standing on the front porch of the residence and one female was observed vomiting over the railing into the bushes and the other female was holding back her hair. Police identified the two females, determined they were underage to be drinking alcohol. As police spoke to the females, they observed the occupants of the residence close the door and lock it. They observed the occupants cleaning up beer cans, cups,
6 and other garbage they had created in the residence. Police were eventually able to get in touch with the resident and officers eventually located all occupants of the house. 12 citations were issued for minor consumption of alcohol. All subjects were cited, released to parents, as necessary, and given a June date. Case Number /05/2017 at 2:15 pm Rte 41 and Gage Lane Speeding 83/55, Driving While License Suspended $1500 Johnson, Jordan O., male, age 20, 144 S. Jackson St., Waukegan, IL /06/2017 at 3:00 pm Police on routine patrol stopped a 2012 Chrysler for speeding 83 mph in a posted 55 zone. Police spoke to the driver, Johnson, and determined he was driving on a suspended license. Johnson was arrested, charged with Speeding 83/55 and Driving While License Suspended, processed and released on bond after being given a June court date. Case Number /03/2017 at 10;10 pm Waukegan Road and Westleigh Road Speeding 63/45, Possession of Drug Paraphernalia $120 Guevara Rivera, Luis J, male, age 20, 305 Lorraine Ave. Waukegan, IL /06/2017 at 1:30 pm Police conducting routine patrol in the area of Waukegan Road stopped a 2005 Honda Civic for speeding 63mph in a posted 45 mph zone. Officers spoke to the driver, Guevara Rivera, and after talking to him officers questioned him about possible drug paraphernalia in the vehicle. Police located a glass pipe with cannabis residue inside it. Guevara was charged with Speeding 63/45 and Possession of Drug Paraphernalia, cited and released with a June court date. Updated May 5, 2017 Case Number /02/2017 at 1:24 am Rte. 60 and Ridge Road Possession of Cannabis and Possession of Drug Paraphernalia 06/14/2017 at 9:00 am Santillanes, Rogelio NMI, male, age 33, 241 N. Kostner Ave., Chicago IL Police on routine patrol observed a vehicle operated by Santillanes commit a traffic violation. Police stopped the vehicle and spoke with the driver, identified as Santillanes. While speaking to Santillanes, officers on scene could small marijuana coming from inside the vehicle. When
7 asked, Santillanes admitted to possessing drug paraphernalia and a small amount of cannabis, which he surrendered to officers. Santillanes was later charged with Possession of Cannabis and Possession of Drug Paraphernalia. He was cited, released and given a June hearing date. Case Number /29/2017 at 3:58 am 100 block of Deerpath Cell Phone Use While Driving, Illegal Transportation of Alcohol, Possession of Cannabis and Possession of Drug Paraphernalia Da Salvia, Craig A., male, age 23, 211 Savana Ct., Lake Forest, IL /10/2017 at 9:00 am Police on routine patrol observed a silver Toyota being operated while the driver was using a cell phone. Police conducted a traffic stop on the vehicle and spoke with the driver, identified as Da Silvia, and during the conversation police could smell the odor of marijuana coming from the vehicle. Da Silva originally denied he had marijuana in the vehicle but when he was informed that based on the smell, the vehicle was going to be searched, he then admitted to possessing marijuana and drug paraphernalia in the vehicle. Police located all drug related items plus a flask with some alcohol inside. Da Silva was subsequently arrested for Cell Phone Use While Driving, Illegal Transportation of Alcohol, Possession of Cannabis and Possession of Drug Paraphernalia. He was cited, released and given a May hearing date. Case Number /28/2017 at 1:19 am Deerpath and Western Ave. Driving Under the Influence, Use of a Cell Phone While Driving, Illegal Transportation of Alcohol Smetak, Matthew B., male, age 31, Walnut Park, Austin TX $ /23/2017 at 9:00 am C 405 Waukegan Police on routine patrol observed a green Jeep driving erratically in the area of the East Business District and the driver was using a cellphone at the time. Police conducted a traffic on the vehicle and spoke to the driver, Smetak, concerning what police had observed. While speaking with police, officers noted an odor of alcohol coming from Smetak, who admitted to being at a local bar shortly before being stopped by police. Police requested Smetak perform some standard field sobriety test to determine if he was impaired. Based on Smetak s performance and observations by officers, Smetak was arrested for Driving Under the Influence of Alcohol, he was transported to the Public Safety Building for processing. Police also located an unsealed bottle of Captain Morgan rum behind the driver s seat. Smetak was processed and later provided a breath sample with a BAC. Smetak was released on bond and given a June court date. Case Number /27/2017 at 12:27 am 1200 block of Edgewood Driving Under the Influence of Alcohol $ /02/2017 at 9:00 am C 405 Waukegan
8 Trujillo, Erik nmi, male, age 41, 980 N Green Bay Rd., Lake Forest, IL Police responded to the area of North Edgewood Road on a report of a single vehicle crash with possible injuries. Police responded to the scene and located a black SUV with damage and air bag deployment in the 1200 block of Edgewood Road. Police also located the occupants of the vehicle and based on statements at the scene, determined Trujillo was the driver and he was demonstrating signs of alcohol impairment. These findings were further corroborated by witnesses who observed him drinking in a local bar before the crash. Trujillo was transported to Northwestern Lake Forest Hospital for treatment and police continued their investigation. Based in information obtained by police, Trujillo s blood alcohol level at the time of the crash was estimated to be BAC. Trujillo was later charged with Driving Under the Influence of Alcohol, No Proof of Insurance and Improper Lane Use. He was processed, released on bond and given a June court date. Case Number /26/2017 at 5:53 pm Ridge Road and Old Elm Road Driving While License Suspended, Illegal Transportation of Alcohol $1500 6/6/2017 at 3:00 pm Burns, Thomas C., male, age 53, 1301 N. Western Ave., Lake Forest, IL Police responded to the area of Ridge and Old Elm after receiving a 911 call concerning a suspicious vehicle in a nearby neighborhood. Police conducted a traffic stop on the vehicle after observing the driver not wearing a seatbelt. Police conducted a traffic stop on Burn s vehicle and observed him pour an unknown liquid out of the vehicle prior to stopping his van. Police were informed that Burn s driving privilege was suspended for DUI and located in the vehicle was an unsealed 1.75L bottle of vodka. Burns was subsequently arrested for Driving While License Suspended, No Seat Belt and Illegal Transportation of Alcohol. He was processed, released on bond and given a June court date.
