4 Your Guide to the Immune System So What are the Different Immune Responses?

5 IgE - Classic Allergy Immunoglobulin E (IgE) is a class of antibody that has only been found in mammals. It plays an important role in allergy, and is especially associated with type 1 hypersensitivity. It is capable of triggering the most powerful immune reactions.

7 IgG Hypersensitivity Immunoglobulin G (IgG) is a immunoglobulin, each IgG has two antigen binding sites. It is the most abundant immunoglobulin and is approximately equally distributed in blood and in tissue liquids, constituting 75% of serum immunoglobulins in humans.[1] IgG antibodies are predominately in the secondary antibody response, which occurs approximately one month following antigen recognition This is the only isotype that can pass through the human placenta, thereby providing protection to the fetus in utero. It can bind to many kinds of pathogens, for example viruses, bacteria, and fungi

9 IgA Immunoglobulin A (IgA) is an antibody playing a critical role in mucosal immunity. More IgA is produced in mucosal linings than all other types of antibody combined Secretory IgA can survive in harsh environments such as the digestive and respiratory tracts, to provide protection against microbes that multiply in body secretions. IgA1 is found in serum and made by bone marrow B cells. Decreased or absent IgA, termed selective IgA deficiency, can be a clinically significant immunodeficiency. IgA levels are tied to genetics and have a high incidence of immuno deficiency in people of European descent.

10 IgM Immunoglobulin M, or IgM for short, is a basic antibody that is present on B cells. It is the primary antibody against A and B antigens on red blood cells. IgM is by far the physically largest antibody in the human circulatory system. IgM antibodies appear early in the course of an infection and usually reappear, to a less extent, after further exposure. IgM antibodies do not pass across the human placenta. These two biological properties of IgM make it useful in the diagnosis of infectious diseases. Demonstrating IgM antibodies in a patient's serum indicates recent infection, or in a neonate's serum indicates intrauterine infection

17 Testing for Food Allergies/Sensitivity IgE n Type one toxic reactions (IgE reactions) are obvious. You know when you have one because your body suffers a notable and almost instant reaction (some people can die from strawberries or crab meat). Most of the time these are KNOWN by accident IgG n n Type two reactions are not immediate making them almost impossible to detect without sophisticated testing. Or potentially limiting dietary changes for weeks. There is a 26 day half life for IgG antibodies so elimination diets need to take an average of 120 days to completely see reduction in all symptoms

18 What about Celiac and Gluten Sensitivity Autoimmune & Disease Risk? Blood Testing Non-Standard Tests - Beyond the Diagnosis Label n IgG and IgA serum n Endomysial Antibody n Tissue Transglutaminase (ttg) n Cross Reactivity of Foods with Molecular Mimicry n Zonulin/Occludin and LPS endotoxemia n Brain Derived Antibodies

19 The Gold Standard Endoscope n Assumes damage is equally spread through the small intestine n Damage to the intestine is often patchy and not equal throughout the length of the small intestines Potential to have false negatives Irratic and Irregular Damage Damage that may not be visable in the small intestine microscopic colitis and other gluten sensitive enteropathies

20 Specificity and Sensitivity of Testing IgG anti-gliadin antibodies are more sensitive but are less specific markers for disease compared with IgA class antibodies. n The IgG anti-gliadin antibodies are 91% specific and have an 87% sensitivity. This means that they will show positive results more readily but there is not as strong a correlation with celiac disease. It is less specific. But is often elevated in other gastrointestinal diseases. IgA anti-gliadin antibodies are less sensitive but are more specific. n In clinical trials, the IgA antibodies have a specificity of 97% but the sensitivity is only 71%. That means that, if a patient is IgA positive, there is a 97% probabilit y that they have celiac disease. n Conversely, if the patient is IgA negative, there is only a 71% probability that the patient is truly negative for celiac disease.

21 Specificity and Sensitivity Continued IgA class anti-endomysial antibodies (AEA) n n n Are very specific, occurring only in celiac disease and Dermatitis Herpeformis. These antibodies are found in approximately 80% of patients with DH and in essentially 100% of patients with active celiac disease. IgA endomysial antibodies are more sensitive and specific than gliadin antibodies for diagnosis of celiac disease. Tissue Transglutaminase (ttg) n ttg ELISA is measuring the same thing that the endomysial is more sensitive and specific than anti-endomysial antibodies testing. This has become the standard screening test before performing an invasive, expensive and potentially error prone endocscope biopsy.

22 Celiac Statistics According to the Journal of Neurosurgical Psychiatry, 2002; 72: ; Studies have shown that the prevalence of Celiac Disease is much higher than previously thought as much as 1 in 100 people. Most of such patients have NO Gastrointestinal Symptoms Silent Celiac. According to Gut Magazine 2006; 55: , the current ratio of clinically diagnosed to undetected cases of CD is 1 to 8. According to the Lancet: Vol.358, August 4, 2001; Death was the most significantly affected by the diagnostic delay, pattern of presentation, & adherence to the GFD Nonadnherence to the GFD (defined as eating gluten once-permonth increased the relative risk of death by 600%)

23 Statistics Continued People with undiagnosed Celiac disease have an increased risk for all diseases n CD patients (not gluten-free) have a 60% higher risk for cardiovascular disease (archives of Internal Medicine Vol. 163, July 14, 2003) n CD patients (not gluten-free) have a 10 times higher risk for other autoimmune diseases like MS, Lupus, Rumatoid Arthritisis than the general population (Cellular Molecular Life Sciences 62 (2005) )
