2 GOAL: REDUCE SUICIDE ATTEMPTS AND DEATHS IN TENNESSEE Enhanced Follow-Up Services (EFS) Program-Statewide Centerstone of Tennessee Suicide Prevention Training Program-Statewide Tennessee Suicide Prevention Network Evaluation Centerstone Research Institute
3 ENHANCED FOLLOW-UP SERVICES (EFS) PROGRAM All clients will receive intensive follow-up services for up to 30 days. This includes (at minimum): Contact within 2 hours of receipt of referral Weekly contact with Follow-Up Specialist Each contact will include the following: Risk assessment will be completed Crisis Management Plan review/development Means restriction education Referral/Linkage to additional needed services TARGET clients (ages 25-6) will receive additional contact with Follow-Up Specialist for up to one year in the form of a phone check-in at months 3/6/9/12.
