2 Outline Lesions of the lacrimal caruncle are rare and vary widely due to its unique composition. Correct clinical preoperative diagnosis is very difficult and is reached only in about half of the cases Rarely, general pathologists and even specialized ophthalmic pathologists encounter lesions taken from this location. The anatomy and histology of the lacrimal caruncle is unique in its composition of both dermal and conjunctival structures that may all be involved in pathological change 15. The aim of this presentation is to introduce the lesions of the caruncle based on a retrospective review of caruncular lesions of the eye examined at the largest tertiary ophthalmic pathology laboratory of the Netherlands. A retrospective search of all lesions of the caruncle examined at the laboratory of ophthalmopathology from 1987 to 2012 was performed. One hundred and sixty lesions of the caruncle were identified. The classical textbooks often only depict oncocytoma of the caruncle which is the classical lesion for this site, but is only sixth in occurrence in our experience. Nevi (36.25%) were the most common lesions observed, squamous papillomas were the second most common entity (15.25%). Together these two entities represented 51.5% of the lesions found in the caruncle. A total of 25 different classifying histopathological diagnostic entities were identified reflecting the complex composition of the caruncle and fifteen more diagnoses can be added by a review of the literature on caruncular lesions Fourteen lesions in our series were malignant (9%). Most lesions present in the caruncle are benign. We present an educative series of clinical and histopathological photographs of the lesions encountered in our practice. We will emphasise the difficulty of clinical identification of histopathologically confirmed lesions and the need for confirmation
